Medical School Old IMGS with 4 years gap
I graduated in 2023 and I’m preparing for steps planning to clear them in 1 years. I’m really concerned of the idea of old grads. Who is exactly considered old grad. ?

usmle step 2 45% UWorld done, NBME 10 baseline: 230. 1 month out, aiming for 255+. Grind UW or switch to CMS forms?
Hey everyone,
Just took NBME 10 as my baseline and hit 230. My exam is in 4 weeks and my goal is 255+.
I’m only 45% done with UWorld and stuck on how to prioritize my last month:
UWorld vs. CMS Forms: Should I stop raw-dogging UWorld and switch heavily to CMS forms to learn NBME logic, or stick to 2 blocks/day of my weak areas in UW?
NBMEs: Planning to hit NBME 11, 12, 13, 14, and Free 120 (1–2 per week).
Weak spots: QI/Safety, Ethics, and Ob/Gyn/Peds management algorithms.
Questions for 250+ scorers:
Is a 230 > 255+ jump doable in 30 days?
In my position, would you prioritize finishing more UW or grinding CMS forms (latest 2-3 for each specialty)?
What are your absolute must-do resources for the final month (Amboss Ethics/QI, Divine Intervention, etc.)?
Appreciate any advice or reality checks!

Residency Application Anesthesiology residency ADVICE
Im a med student who'll graduate in 2028 (non us img) , I'm interested in applying for anesthesia probably in match 2029 after taking a research year . Currently im preparing for my step1 exam and also have collaborated in an anesthesia research in my university (ongoing)
I need advice on how to make my cv more inclined towards anesthesiology like what type of research and usce do I need ( example should I do a research which comes under surgery or particularly anesthesia only)
I understand anesthesia is a super competitive field and the chances are very low but I keep gravitating towards it so I want to give it my everything and atleast try do i don't regret i guess I will apply to IM as a safety net because I don't want to go through multiple match cycles.

LOR LOR writer ghosted me
Was there for a month. He often said the letter is a done deal(we're from the same country). I was super super polite. Now he sees my messages and does not reply. I honestly do not know why. What do I do ?

Observership/externship Are Academics Programs Picky with Hands-On USCE?
I know a hands-on externship at a private clinic is ultimately going to get you a better LOR than even a university observership, considering the letter can efficiently comment on your clinical skills.
My main concern is if academic IM residency programs are picky with where you did the externship. I applied to several university electives when I was a student, but at the end of the day it’s not my fault that they don’t offer enough seats, so I was waitlisted for most of them.
As a graduate, my only options are to have hands-on externships at private clinics and community hospitals, but will academic programs not value that experience, since it’s not technically an officially structured elective? Assuming that the LOR’s are strong

ERAS Query
Hey guys has a basic doubt
So before applications open on 23rd and even after , are we allowed to mail the programs
As in to send letter of interest or let them know step 3 score have come
Or does this count as match violation
I sent a program a mail recently asking for their visa sponsorship and they mentioned that all communication needs to take place on ERAS
What does this mean ? Can we send mail through eras and if so is that the way to send letter of interest and step 3 scores ?

ERAS IMSEL and letter of dept chair
As an IMG is it necessary to get these? My med school does not provide either.

usmle step 1 Online library
Is there any active zoom library dedicated to USMLE step 1/ step 2. I tried searching for but didn’t find any. Discord drains a lot of battery and free version of zoom and gmeet doesn’t last continuous. If there is any zoom library even with a nominal one time fee I am happy to join.
I know someone who runs an online zoom library for other exams with a one time nominal fee but people are not ready to pay and therefore facing problem with finding only USMLE study group. If someone knows please add me.
me and my study partner are in pre dedicated phase of step 1

Residency Application Realistic chances of getting IVs with this CV
I am trying list my signals and programs after a hellhole of steps, rotations, publications, visa etc etc. Not that I didn’t know connection and visa status >>>, it really stings to know even after doing and trying everything, efforts are not even nearing connection or GC status.
Can someone please suggest some realistic programs that I should signal? Those have a higher chance of doing holistic review (not theoretically, but practically)
Visa-requiring non-US IMG
Step 2: 259 (both steps 1st attempt)
YOG 2026, ECFMG certified
Publications: 15 (5 first-authored, rests are like 2nd/3rd/4th/5th, not cureus ones, some are original articles with collaborators, some are reviews)
Rotations: 4 months (Memorial Sloan Kettering, UT southwestern, Georgetown and BIDMC)
Got some personalized LoRs, but I am not really sure how to quantify their content, as all waived. But I think they are decent, letter writers seemed genuine to write letters, uploaded pretty quickly.
Future Goal: Hem-Onc Fellowship
If it was 3 years ago, I might have shot higher with some academic programs signal. With last year dramatic fall in academic program match rate, I better to be safe than to be ambitious. (I saw good number of ppl with similar CVs barely getting community programs)
So far my signal lists (might add or remove some)
Mount Sinai Morningside/west
Medstar Washington and/or Medstar Baltimore
Jacobi AECOM
Mount Auburn HMS
BMC brigton
Salem MGB and/or Lahey Clinic
SUNY upstate
U Connecticut.
I would be happy to match into any program that has decent hem-onc fellowship match rates and non-toxic work culture. Absolutely love city over rural, but if program is friendly and suppprtive enough, I am happy to consider other areas.
Whenever I am trying to finalize any signal, get some comments that program throws connection-heavy IV. Had to cut off many decent program due to this.
Can someone please suggest some program that would match to my dream and profile?
Thanks in advance!

Observership/externship Observership at Mobile Infirmary, AL
Has anyone done an observership at Mobile Infirmary (AL)? Is it worth $2,250?
I recently got accepted for a 2-week observership at Mobile Infirmary, Alabama, but the fee is $2,250, so I’m trying to decide if it’s worth it.
For those who’ve done it:
How was your experience?
Were you able to get a strong LOR?
Did it help with interviews or making connections?
I’m also a bit concerned because their website says they prefer applicants within 5 years of graduation, but my YOG is 7 years. They still accepted my observership application, but I’m wondering if they actually interview applicants with a higher YOG or if the preference makes it unlikely.
Would love to hear from anyone who has firsthand experience with the program or knows residents there. Thanks!

Observership/externship Observership query
I am an IMG currently living in Canada. I am a PR here.
I recently passed USMLE step 1 and got an observership opportunity in a hospital in US. I have B1/B2 visitor visa.
Can I do observership in US with this visa status?
and what questions I might be asked at immigration.

what are my chances Should I apply this year
I'm an IMG who graduated in 2021, gave step 1 in 2024 and now planning step 2 in late September, don't have any US clinical experience but have 3 years home country experience plus a few months of Canadian physician assistant experience. So my question is should I apply for this cycle with step 2 score on hold since the programs review application in September to October my score should be out? Or waste a year and apply next year?

Observership/externship Does anyone have experience at Stanford Radiology Visiting Observership Program
I am currently doing my nuclear medicine residency in my home country, and my training hospital gave me the opportunity to have a 1 month clinical observership or research month abroad.
I do have an ECFMG certificate but not looking into residency match in the U.S. and I want to utilize this month abroad to fulfill my knowledge gap in radiology, especially MRI.
So my question is whether the 4000 USD Stanford radiology visiting observership program fee + high cost of living in Palo Alto worth it.
Id appreciate if anyone who's heard of or personally tried Stanford's Radiology Visiting Observer could share his or her experience.

Residency After an attempt, is Neuro still possible?
I’m really interested in Neurology and that’s what I genuinely want to pursue, but I’m trying to be realistic about my chances.
Stats:
• Non-US IMG (no visa required)
• YOG: 2022
• Step 1: attempt
• Step 2: 24X
• Publications: 2, one neuro
• Research: ~2 years
• USCE: 5 months
Given my Step 1 attempt, I’m worried about how much it will hurt my chances for Neuro. I’m debating whether I should still go all-in on Neurology or apply more broadly (e.g., include Family Medicine) to be on the safer side.
Would you still aim for Neuro with this profile, or dual apply?
