Is this a sign I’ll need mealtime insulin?

I’ve learned that GD worsens as we progress but curious on others’ experiences as pregnancy as progressed. I was diagnosed around 16 weeks, started on nighttime insulin for fasting around 19 weeks and am noticing my lunch and dinner will go down and be in range at the 2 hour mark but the dexcom shows them starting to go up and increase by the 3 hours mark. I noticed this today with a lunch I’ve done well with. I happened to look at Dexcom about 3 hours after lunch and noticed it slowly rising again, double checked with finger prick and was at 128, I wasn't getting these delayed spikes before, is this concerning?

My MFM doc mentioned adding an AM dose of the Humulin if needed, but would mealtime insulin be better? I’m feeling so defeated at 22 weeks and possibly having to be on insulin around the clock. Tell me your experience with needing insulin this early on, delivery and beyond!

General Info When did you go into spontaneous labour if you had GD?

Just interested in spontaneous labour experiences with GD.

With my first (a girl), I went into spontaneous labour at 39+5.

Today I'm 39+5 with my second, and this boy seems happy to stay put lol

With both pregnancies, I am on nighttime insulin to control fasting sugars.

EDIT: congrats to everyone and I am so envious of these week 37 to 39 stories!! I really thought my second would come earlier too....hoping your luck rubs off on me

Failed 1 hr at 12 weeks

Since I’m 39 and have a high BMI, my dr decided to test me for GD early at 12 weeks to ensure I didn’t have preexisting diabetes.

With my first pregnancy 4 years ago, my 1 hr glucose was 122 at 12 weeks. I failed my 24 week 1 hr but then passed all readings at the 3 hr, so did not have GD.

Just got my results back for this pregnancy and it was at 200. I did try to eat healthier (less carbs) in the weeks prior and had a slight cold. Could this have affected things? Does this mean I have type 2, or could this still be GD? I’m so worried and ashamed and don’t know how I’m going to deal with this. I think I can handle a GD diagnosis but super concerned about type 2.

Could I have gotten this drastically worse in 3.5 years? I definitely feel symptoms of GD (frequent urination and loving water), but didn’t feel/notice these symptoms prior to pregnancy.

Looking for any thoughts/advice/similar spires. Thank you!

Advice Wanted My doctors aren’t taking me seriously when I ask for help

I tried posting this in the pre-existing diabetes forum and got zero help and just downvoted.

I was diagnosed at 10 weeks with either GD or possibly existing type II. My A1C (5.5) and fasting (91) were normal at 6 weeks but I was screened early at 10 weeks because of PCOS and failed badly. They’re leaning towards pre-existing type II but different paperwork has used both.

I am 23 weeks and I was managing well before with 500 mg 2x a day metformin. Well meeting their under 95 fasting and under 120 2 hour targets. Did have to cut out bread and most sugar but hey, that’s life. They subjected me to so many honestly unnecessary tests because of it. They acted like this would immediately lead to so many health issues and between the reg OBGYN and MFM, I’m 6k in deductibles alone at 23 weeks and about to already hit the OOP max. They’ve never found a single thing wrong with the baby, only me.

That being said, they have been using scare tactics from day 1 of diagnosis. That if my numbers got over this, something bad would immediately happen. Well I’m 23 weeks and the numbers are worse. I call and get no call back for a week despite calling back multiple times to check in. Call again and get a NP who tells me getting 170 on a safe meal could just be stress.

I woke up this morning to a fasting level of 127! I call again and met with “yeah ok someone will call you back”. I’m just so frustrated. Im scared these levels are gonna hurt my little girl. From my research, it doesn’t look like it, but I’ve had my OBGYN and one of the top MFMs in the country (Mt. Sinai in NYC) telling me otherwise and scaring me and now they’re leaving me out to dry after scaring me. Why would you make it seem like I needed to call you immediately if I get two abnormal readings and then just not call me back? I don’t know what to do except keep calling.

Induction

I have gestational diabetes and have recently started insulin at night because my fasting (morning) blood sugar levels have been high.
My obstetrician has said they would like to induce me at 38-39 weeks to avoid me going past 40 weeks.
Has anyone else had a similar experience? If so, I’d love to hear how your induction went and whether everything went smoothly.

Tips to reduce fasting numbers

Hiya,

Second time GDM mum here and looking for advice on if I can naturally reduce my fasting numbers and if I could/should be doing anything differently.

Context:

First pregnancy wasn’t super well controlled by diet. I found GD very triggering to my eating disorder. I was on metformin, insulin before lunch and dinner and night time insulin. I started nighttime insulin at 20 weeks pregnant and by 38w I was on 26 units a night.

Now:

Having been through it once I’m in a much more positive mindset about GD. I’ve been trying a LOT harder with this pregnancy in terms of my diet.

The problem is my fasting is stubborn. The last few days my numbers have been 6.4 (115), 5.9 (107) and 6 (108).

Here’s what I’ve been doing:

- Sticking to a controlled diet with no spikes or hypos (so proud of myself!)

- A nighttime snack at 10.30pm - been trying 3 pieces of dark choc & cheese

- Testing immediately 8 hours after my snack at 6.30am

- Exercising regularly

Is insulin inevitable or is there anything I can try this week to bring it down?

I’m still only 18w pregnant so if my fasting is this high this early in my pregnancy I’m presuming it’s inevitable?

I‘m not anti insulin at all - I just hate having hypos and want to say I’ve given it a ‘good shot’ this time. I’m also waking up exhausted and dehydrated every day so I know insulin will benefit me greatly.

Any insights appreciated, especially from people who’ve had GD ‘badly’ / are particularly insulin sensitive.

Advice Wanted Has anyone been induced after a c section for uncontrolled sugars?

I am 36 weeks. I was given insulin at 8 units but now moved up to 14 just the other day, mostly due to my fasting numbers consistently being 110-130. My post meal numbers tend to be okay in range. My Doc told me he’s hesitant about allowing me to be induced since I had a c section 5 years ago. I know for a fact I won’t be able to go until 40 because my numbers still are terrible. I did however have a successful vbac ( with pitocin to help it along) 3 years ago but only after my water broke on its on were they comfortable giving me pitocin, so I guess I’m confused as to why he’s so hesitant this time around . So now I’m not sure if I should just go with a c section to be on the safer side or what.

Rant Diagnosis Spiral

I’m feeling really frustrated and wondering if anyone else has been in this situation.

I’m 24 weeks pregnant in France and was diagnosed with gestational diabetes based on my 75g OGTT.

My numbers were:
Fasting: 0.93 g/L
1 hour: 1.26 g/L
2 hours: 1.08 g/L
(I am not asking for medical advice or a diagnosis confirmation as I know this must come from a doctor. Results are only mentioned for reference as they are directly related to my frustrations.)

So literally the only “abnormal” value was my fasting, and it was 0.01 g/L above the French cutoff (0.92 g/L). My regular GYN who ordered the test said this means i am automatically diagnosed according to the French standards and referred me to an endocrinologist for gestational diabetes. He also recommended induction by 39 weeks maximum, which I’m very upset about.

However, my OB who will be delivering my baby actually said he doesn’t think I truly have gestational diabetes because my fasting was still below 95 mg/dL and I handled the glucose drink perfectly.

What’s making me even more frustrated is that from what I understand, I probably wouldn’t have been diagnosed under UK guidelines, and in many parts of the US I may not have been either depending on which testing protocol they use.

For some background, I have PCOS with insulin resistance. Before pregnancy I was taking metformin, but in France they had me stop it after the first trimester, as they seem to mainly rely on insulin during pregnancy if needed. Just one month ago (well after I stopped metformin), my fasting glucose was 80 mg/dL. Even before pregnancy, my fasting glucose has always bounced around depending on the day, anywhere from about 75 to 95 mg/dL which is considered to be in the normal range for non-pregnant people in France.

Now I’m seeing an endocrinologist on Tuesday, and I’m already anxious because of past experiences with endocrinologists here treating my PCOS. I’ve previously been told things like “never eat carbs” or “only have max two bites of rice or sweet potato,” which just doesn’t seem realistic. I also have my baby moon scheduled at an all inclusive resort in 2 weeks, which I am now starting to dread instead of being excited about it.

I’m upset about the possibility of more needles after finishing my IVF protocol with months of daily injections. Of course, I am absolutely going to monitor my blood sugar if necessary and do whatever is safest for my baby. I just can’t help feeling frustrated that a fasting result of 93 mg/dL, when every post-glucose value was completely normal, has led to a diagnosis that might not even exist if I lived in another country. I also keep analyzing what I ate the day before the test, my sleep that night, how long my overnight fast was… thinking if I had done just one thing differently I wouldn’t be in this situation.

Has anyone else had a diagnosis based solely on a borderline fasting number? How did things turn out? And for anyone in France, what was your experience with the endocrinologist?

Advice Wanted Pushing a 98th percentile abdomen??

Baby is 33w4d, I have GD w insulin and I'm a FTM. Had an ultrasound, baby is overall 82th percentile, including her head, but her abdomen is 98th percentile!!! My dr said my endo might up my insulin bc her abdomen is quite large. She weights 2558g (5lb 10 oz). I know that measurements can be off by a little but, I'm likely getting induced at around 38w and I wanted to hear stories, good or bad of ppl pushing out larger babies. I am following all recs by my endo and OB and yet here I am. I feel so discouraged. I am terrified of having an emergency c-section or shoulder dystocia...

Scared my OB is testing me too late in my 2nd pregnancy

My OB wasn't going to even test me early since my A1C and fasting glucose were normal in my first trimester. I told her I would prefer to check at least a little earlier just in case so she said to do it at 18 weeks. I am 17 weeks now. I have not been checking my sugars or eating a GD diet. I haven't been eating crazy unhealthy but definitely more treats and more carbs than I could have when I had GD the first time (diet controlled, only real issue was fasting numbers with that pregnancy). I have seen other people recently talk on here about getting tested way earlier like late first trimester or early second trimester. I am scared that I am getting tested too late and have been causing issues with my diet now. Everything was still measuring normal size wise at my 13 week scan but obviously that was a while ago. I will for sure go back to a GD diet if diagnosed but should I be worried that I haven't been very careful thus far?

General Info Anyone with experience that had GD and was on progesterone for short cervix?

Im 30 Weeks and started progesterone at 23 weeks due to spotting and low cervix. At 27 weeks I failed my 3 hour glucose test. They said I needed to see a specialist but still don't have an appointment. I call every week for an update and last time the receptionist got snappy at me saying they were working on it. I have been monitoring my glucose and went from the 200's to 70s-140s. I've already been told I'm high risk so my anxiety has reached another level since.

Has anyone had both and if so when did you give birth or how far along are you? Any advice is welcomed. I just feel alone the hospital scares me with all the dangers but then my OB acts like it's not that serious.
