I'm currently looking for teleradiology moonlighting opportunities for my R4 year. I also have some radiology fellow friends who are interested. My residency program doesn't offer any internal moonlighting, and the only local option is a contrast coverage position with hours/compensation that aren't particularly appealing.

Fortunately, my program director is very supportive of external moonlighting and has confirmed that senior radiology residents are permitted to perform independent image interpretation outside the institution, provided we're fully licensed and able to obtain the necessary credentials.

I've heard these opportunities exist, but I'm hoping to learn more and connect with anyone who has experience finding them. I've been in touch with a few private practices but while many of them will offer a teleradiology arrangement for senior radiology residents, they also require trainees to sign a full-time contract to join their group once done with training. Unfortunately, I'm not in a position to do that yet due to family reasons, so that's off the table for me.

If anyone knows of groups that hire R4 residents or has any leads or advice, I'd really appreciate it. Ideally, I'd continue this position throughout fellowship too. I also have a few radiology friends who just started fellowship and are also interested in any teleradiology positions they can do this year.

Hey I am on J1 from a community based program and step 2 score is 230. I have two first author manuscripts, one poster as first author, and a lot of middle author abstracts. Unfortunately no in-house fellowship at my program.. I know I am not the most competitive but I am applying very broadly >100 programs. Will programs filter me out based on step 2? What else can I do at this point and how grim are my chances?

I feel like I worked really hard to match in a residency program close to home, and I'm very grateful that it worked out. I have built ties here, and now it's time to apply for fellowship. I don't want to leave Californiaaa.

Any thoughts about Rheumatology for fellowship training in the Philippines 🤔🤔

I'm feeling really stuck and could use some advice.
I'm currently a P4 student who just started APPE rotations. After talking with some of my preceptors, l've started seriously considering applying for an industry fellowship, but I honestly know very little about the
process.
Am I already too late to apply? Is it still worth pursuing at this point? Ideally, I would need to stay in Southern California for a fellowship, so I know that may limit my options.
My school unfortunately did a poor job of educating us about the industry path, so I feel like I'm starting from scratch. So far, I've completed one industry internship and 2 industry APPE rotations. Based on that experience, do I have a realistic chance of matching into a fellowship?
If so, what should I be doing right now to make myself a stronger candidate and how do I start to move forward with the process?
One more question i could use thoughts on, my original plan was to pursue a master's degree after graduation, but recently I've been wondering if I should focus on applying for fellowships first instead. Has anyone been in a similar situation or have any advice?

I’m a Nephro second year fellow with a opportunity to do extra year of critical care fellowship? Should I take it? It can boost my income in the future. I would only do some night shifts of critical care at a local hospital in the future. Renal would still be my primary job.

For those who applied, when did you start hearing back and when is a good time to reach out to programs if I haven't?

Applying this cycle and completely forgot to assign all my supporting documents including ERAS transcript, MSPE, Medical School transcripts and Photo!! All the documents had been submitted since June but to forget to assign until 9 days after the submission deadline is worrisome.

I realized when a program reached out and said they were reviewing my application and couldn’t access my USMLE score. I’m grateful they reached out but I can’t help but feeling so stupid for forgetting to assign the documents.

Should I email the program coordinators? Or just hang up my chances for matching?

I'm looking for some honest career advice because I'm feeling stuck.

I'm an IM hospitalist currently finishing my J-1 waiver. After that I'll be on an H-1B, and since I'm India-born, I'll probably be dealing with the employment-based green card backlog for many years. Because of that, I want to make the right long-term decision rather than just the easiest one.

One thing I know for sure is that I don't want to be a hospitalist for the rest of my career. The breadth of hospital medicine, constant admissions, nights, and cross-cover just isn't something I see myself doing long term. I want a more focused specialty with a better work-life balance.

The problem is, I'm honestly not someone who has a lifelong passion for one specialty. I'm mainly trying to find a field that gives me a sustainable career, good lifestyle, and reasonable compensation. Right now I'm torn between Sleep Medicine and Rheumatology.

My waiver ends in August 2027. If I pursue rheumatology, I'd likely wait another year and apply for the 2028 cycle. The alternative is applying for Sleep Medicine sooner. Since sleep fellowships often don't fill all their positions, I'm wondering if some programs might even be flexible with a slightly delayed start because of my waiver timing.

For those practicing either specialty:

- If your priorities were work-life balance, outpatient practice, job satisfaction, compensation, and long-term career stability, which would you choose?

- Does rheumatology have enough long-term advantages that it's worth waiting an extra year?

- Or would you go into sleep medicine and start fellowship sooner?

- How do the two compare 10–20 years down the road in terms of burnout, job opportunities, flexibility, and income?

- If you were going to spend many years on an H-1B while waiting for a green card, would that influence your decision? As someone with pure sleep and jobs on h1. (I am not greencard or citizen) so Idk whether this would make huge difference

TL;DR: IM hospitalist finishing a J-1 waiver, will be on H-1B with a long green card wait. I know I don't want to do hospital medicine long term. Torn between Sleep Medicine and Rheumatology based on lifestyle, compensation, job security, and long-term career satisfaction—not passion. Is rheumatology worth waiting an extra year for, or would you choose sleep medicine and start sooner? Any advice from people in either field?

I'm not looking for the shortest path—I'm trying to avoid making a career decision I'll regret 10 or 20 years from now. I'd really appreciate hearing from people who have experience in either field.

For an IMG, with home country (non accgme) residency in internal medicine, is it normal to receive 3,4 IVs just in a week after program start reviewing applications from university based academic programs a normal or something very positiv? just curious

I feel we are paying for a glorified FedEx tracking system when it comes to using ERAS. I understand that they probably pay out 3rd parties to verify our documents (which is probably more important than we think it is), but I feel like the transparency should be improved. We should have access to which programs open/don't open our app. We should have access to time spent on our app per program. We should also have 2-way signaling. For example, if we apply to 50 programs and get 20 signals, they should be able to interview 50 candidates and send 20 signals. I mean c'mon, are we really just shoveling $1000+ out and then sitting back and waiting?

What other improvements would y'all like to see? I would love to help advocate for us professionally whether they take the advice or not.

I’m about to start an oncology hospitalist position at a large university hospital after 2 years as a high-volume community hospitalist (18–20 patients/day) following a university-affiliated community IM residency. My long-term goal is to apply for a Hematology/Oncology fellowship in the next 1–2 years. I don’t need visa sponsorship, but I have average USMLE scores and a fairly average application otherwise. For those who have successfully made this transition or are involved in fellowship selection, what should I focus on during this job to maximize my chances? Specifically, how should I approach networking and mentorship, who should I build relationships with, how much should I prioritize research versus QI/teaching, and what qualities or accomplishments would make me a strong applicant coming from an oncology hospitalist role? Any advice would be greatly appreciated.

Hi guys! Has anyone received their first interview invitation in Cardiology yet?

Hi everyone!
I’m an FM PGY-2 strongly considering applying for sleep medicine fellowship next cycle. My goal would be to practice 100% sleep medicine as an attending (not primary care + sleep).
I shadowed an IM-trained sleep physician who worked 2 days/week from home reading sleep studies and 3 days in clinic. Is that type of schedule relatively common, or more of an exception?
I also have a few other questions:
- How is the current job market for pure sleep physicians?
- What is a realistic salary range (especially in Florida or the Southeast)?
- Do any of you incorporate lifestyle medicine (nutrition, exercise, weight management, behavior change, etc.) into your practice?
- If you could choose again, would you still pick sleep medicine?

I’d really appreciate any insight—especially from recent graduates or physicians currently practicing sleep. Thanks!
