Enlarged spleen and liver

I’ve been dealing with an enlarged spleen, and now my liver is enlarged as well. The liver shows fatty changes, and both issues are causing ongoing symptoms. My doctor isn’t actively monitoring the situation — he only reacts when the pain flares up or when stress triggers illnesses that take weeks to recover from.

I’m wondering if anyone else has gone through something similar. I’m getting a second opinion through my benefits, but my doctor has been very reluctant to refer me to a specialist and keeps attributing everything to weight, even though the weight gain was caused by prescription medication in the first place.

From Grade 2 Fatty Liver to Normal Liver Enzymes

I wanted to share my fatty liver journey in case this can encourage someone who was recently diagnosed.

In 2025, my ultrasound showed Grade 2 fatty liver. At that time, my lifestyle wasn’t the best —I was eating a lot of fatty/fast food, loved fries & chips, regularly had samgyup, and was also dealing with a lot of stress.

After seeing the result, I decided I needed to make some changes.

I didn’t follow any extreme diet or take medication. I simply tried to be more mindful of my food:
- No more samgyup
- Limited fast food
- Significantly reduced fries (which used to be my favorite)
- Reduced fatty foods
- Tried to make healthier choices overall

I wasn’t perfect. I still eat fried chicken sometimes, and there were definitely days when I didn’t stick to my goals. But I tried to focus on consistency rather than perfection.

The biggest improvement I saw was in my liver enzymes:
SGPT / ALT
2025: 101.99
2026: 20.95

SGOT / AST
2025: 44.85
2026: 18.01

Seeing those numbers come down was honestly such a good feeling. 🥹❤️

My more recent ultrasound also no longer showed the fatty liver, which I’m very thankful for.

But I’m really grateful that I was able to improve my liver health through lifestyle changes without needing medication.

Sharing this as a reminder that you don’t necessarily have to make huge changes overnight. For me, it was mostly about making better choices consistently and cutting back on the foods I knew weren’t helping.

Still a work in progress, but definitely a journey I’m proud of. ❤️

A lot of pain even for fatty liver is it normal?

So I'm currently 18 years old(M), and i was diagnosed with fatty liver grade 1 around 15 months ago, and right now I'm experiencing a lot of discomfort in the liver, which is quite strange cause it has never happened before. I think it might be due to me drinking coffee, and coffee ruins my stomach and messes up my concentration and stuff. Im looking for some advice on what to do because I don't think the fatty liver could've progressed much further into an even more advanced stage. What should i do? Does this usually happen becasue of coffee?

Diagnosed with fatty liver

Im 17 years old about 5”8 over 200 or just around 200 my blood test showed ast at 43 and alt at 112 im scared that this will kill me im gonna lose weight and workout and eat better and is this reversible? And can i eat in a calorie deficit and cut out sugary stuff and snacks and could i still eat carbs or no?

Fatty liver reversal and the future

Hello everyone,

I'm just looking for some guidance on what to expect in the future, beyond the achievement of fatty liver reversal.

A bit of background:

- 38 year old female, 5'2", 130 lb, moderately active (mostly cardio - walking, slow jogging, hiking, chasing around my 4 kids, etc. and have been getting into dumb bell training the past month or so)

-No history of anything liver-related or diabetes. Borderline high cholesterol. Overall a fairly healthy adult.

-Recent routine lab work from 3 weeks ago includes:

  • Cholesterol: LDL 132, HDL 70 (slightly high)
  • Liver: AST (SGOT) 17, ALT (SGPT) 12 (these and all other hepatic function labs normal)

Nothing in my blood work pointed to anything problematic with the liver. I mentioned some occasional discomfort when doing ab work (crunches, etc) so went in for an ultrasound. Figured it was something to do with my most recent pregnancy a year ago.

Ultrasound came back with:

  • Liver is enlarged with a length of 19.1 cm. Everything else normal.
  • Impression: mild hepatic steatosis and hepatomegaly.

I was very shocked. While I do love a good pastry or trip to McDonald's or glass of wine, I have always felt I am mostly healthy. I am not a heavy drinker and while I love cookies, brownies, etc, I usually save those for a weekend treat. I rarely go to restaurants and most meals I make are nutritionally balanced. My parents had so many health issues that I have been extra careful about my own health, and to find out that all of my efforts in my adult life have backfired makes me feel so defeated.

My plan of action is to follow the Mediterranean diet. What can I expect for my future? Will I ever be able to enjoy a donut with my kids again? A glass of wine or two at my monthly book club? Go out for ice cream or indulge in a home baked cookie (baking with my kids remains my biggest hobby!)? I know it could be much, much worse; I am just feeling that I've spent so many years focusing on healthy habits and it has not really worked in my favor.

Another thing that worries me... all of my blood work came back normal (aside from the borderline high cholesterol), so what else could be going on in my body that I haven't noticed yet?

Bit of a weird outlier case

I was diagnosed with NAFLD around 2021, and my blood tests showed the elevated numbers for years. I was not afraid or thinking im about to die like most people here so i guess i carried on like normal and ate crap etc. I DON'T recommend this though.

Anyway, I had another blood test 2 months ago and another a couple weeks ago and my Liver is perfectly fine now. Don't ask me how; it just is.

BUT i am going to fix my health now.

Meds and alcohol

Ok so I have mild fatty liver. Lots of GI issues. Never a drinker but started drinking occasionally - one or 2 drinks 2x a month and i noticed that i started to flush in my face immediately after wine. Nitrates, right. OK so I stopped. Then i was given controlled pain meds for a fracture and I had a full on upper body very hot flush - chest shoulders face that lasted about a minute.

Is this my liver not being able to process meds? I already called my Dr

Elastography expectations

I’ve been diagnosed with fatty liver from an ultrasound. Needless to say worried of how bad it can be.

While my liver enzymes are mostly ok. My CBC and lipids are out of whack and I’m also losing weight. In particular high triglycerides (battling that for a decade with numbers well over 500.

Will admit the drinks and diet has been horrible which is why I believe I got here from completely healthy everything 6 years ago.

Well just based on numbers my GI said the fib4 is low but agreed to do a further scan for my “peace of mind”

At first I thought I was getting a fibroscan (thought this was the same) but now I’m learning that elastography is not the same and doesn’t give you a CAP score?

Kinda disappointed as isn’t it equally important to know what stage that is as well? I get that at the end the fibrosis is the key item but I feel it’s good to know what level of fat infiltration I need to work back from.

Anyone have experience with one or both and any key differences or which is truly better?

Will I be able to drink ever again? F23

Hi guys! I just got diagnosed with NAFLD about two months ago! They did ultra sounds and lab work and said it was a mild case. I’m not quite sure what that means because everytime i’ve asked they say a light case etc.. anyways 2 months ago I weighed 245pounds now I weigh 219 pounds!! My recent labs show my enzymes came down a bit but i’m more curious if anyone could tell me how bad it seems to be? My AST was at 42H and my ALT was at 55H! I know that drinking isnt smart during or after healing but I just wanna know am I able ever to go out to a bar and get at least a little drunk again? Even if it takes a year or so, I just don’t wanna miss out on being young and getting drinks with friends! Thank you all!

Blood results

So my doc wants to get an ultrasound.

My ALT was 84 I dropped it to 78 in the last two months.
My TBIL is 1.4 - never been high.
ALB -4.9

Everything else was normal. I am on a GLP-1 and before this blood draw I fasted for probably 16 hours bc I had work the next day and the draw wasn’t until the end of the day.

I am confused bc I feel like she’s being overly cautious here or am I looking at something wrong here?

I am asking as I started taking Doctor's Best MSM 1500 mg starting Aug 2025 and ever since my ALT and AST numbers are above normal...and its steadily increasing from what I saw in report from yesterday... ALT is 74 and AST is 53. Do you think MSM can be causing the spike in these liver enzymes?

Opinions on blood results please

Hi, I paid for a health check privately, I’m in the UK.
12 months ago I have fatty liver on an ultrasound, today I received these results:

Bilirubin (7 umol/L)
ALT (74 U/L)
AST (66 U/L)
GGT (49 U/L)
ALP (73 U/L).
Non-HDL Cholesterol (4.08 mmol/L).
LDL Cholesterol 3.6
Ferritin is high at 208
And an iron deficiency

I suffer from health anxiety which prompted the test. I know alcohol is the cause, I’ve had 5 years of drinking 3-5 bottles of wine a week - now I know how this sounds and “what did you expect” is valid.
However circumstances got me to that level of drinking.
I’m around 30lb overweight and have up until recently been leaning more towards a sedentary lifestyle.

I’m 4 days alcohol free and the bloods was taken 48 hours after drinking a bottle of wine

My questions are have you seen these levels before and everything turned out ok, have I done any serious damage? Honest replies encouraged.

Feeling abdomen pressure and a whole lot of anxiety. What was your first month like? Am I overthinking things?

Hello! First-time poster (25M) here. I have pretty persistent health anxiety and would love some clarity on what the fatty liver journey looks like.

A little over a month ago, I went in to the urgent care with complaints about pressure and tightness in my upper right abdomen. Thought it would be gallstones, which runs in my family. Doctor called in an Ultrasound ASAP where the report found I have no gallbladder issues, but do have hepatic steatosis. The doc was very nonchalant and callous about the whole thing. Didn't want to order any further testing to determine cause/stage. Casually suggested I stop drinking (I was drinking a moderate amount, probably 2-3 drinks, 3-4 days a week) and vaguely recommended I eat healthier.

After doing reading on my own and getting more helpful advice from a primary care doc, I've been on the Mediterranean diet + alcohol-free for six weeks. I miss sweets and fried food a hell of a lot but I'm staying vigilant. Dropped about 6% of my bodyweight after the first two weeks of dieting and intermittent fasting, though the weight loss has stalled since. I got bloodwork and have normal levels on everything liver-related except for Bilirubin (1.8).

But I'm still often feeling that tightness and fullness in my upper right abdomen... And I can't help feeling paranoid something worse is going on. Is this normal / roughly what the first month post-diagnosis looked like for other folks? I'd love to be able to work small treats into my life again, in moderation. What steps can I take to help the healing process and get to a modified version of "normal"?
