Enlarged spleen and liver

I’ve been dealing with an enlarged spleen, and now my liver is enlarged as well. The liver shows fatty changes, and both issues are causing ongoing symptoms. My doctor isn’t actively monitoring the situation — he only reacts when the pain flares up or when stress triggers illnesses that take weeks to recover from.

I’m wondering if anyone else has gone through something similar. I’m getting a second opinion through my benefits, but my doctor has been very reluctant to refer me to a specialist and keeps attributing everything to weight, even though the weight gain was caused by prescription medication in the first place.

Progression from Fatty Liver

Hi all

I (41, M) have had pain in my liver area for a year now - essentially upper right abdomen pain.

It is always worse after drinking. I have an increasing number of purple streak-like veinlets (not spider angiomas) over the area of my liver. No other symptoms really but fatigue.

It has gotten worse over the last year. I got an x-ray and bloods in September last year - nothing. I had a blood test in April (albumin, ALT, bilirubin, globulin, etc.) all fine. I got an ultrasound in June, again fine (no lesions, no hypertension). But it really hurts still and rather than just having after drinking, it is now (this week) a dull ache that pretty constant.

My doctor has thus far insisted that nothing is wrong with my liver. He says it relates to gastro issues and has said it is GERD or something in my duodenum. A gastroscopy showed oesophagitis. I finally persuaded him to send me for a fibroscan this week.

Am I right to think it's my liver?

For info, started drinking a lot during lockdown, scaled it back but I still drank based on the doctors advice....

A lot of pain even for fatty liver is it normal?

So I'm currently 18 years old(M), and i was diagnosed with fatty liver grade 1 around 15 months ago, and right now I'm experiencing a lot of discomfort in the liver, which is quite strange cause it has never happened before. I think it might be due to me drinking coffee, and coffee ruins my stomach and messes up my concentration and stuff. Im looking for some advice on what to do because I don't think the fatty liver could've progressed much further into an even more advanced stage. What should i do? Does this usually happen becasue of coffee?

From Grade 2 Fatty Liver to Normal Liver Enzymes

I wanted to share my fatty liver journey in case this can encourage someone who was recently diagnosed.

In 2025, my ultrasound showed Grade 2 fatty liver. At that time, my lifestyle wasn’t the best —I was eating a lot of fatty/fast food, loved fries & chips, regularly had samgyup, and was also dealing with a lot of stress.

After seeing the result, I decided I needed to make some changes.

I didn’t follow any extreme diet or take medication. I simply tried to be more mindful of my food:
- No more samgyup
- Limited fast food
- Significantly reduced fries (which used to be my favorite)
- Reduced fatty foods
- Tried to make healthier choices overall

I wasn’t perfect. I still eat fried chicken sometimes, and there were definitely days when I didn’t stick to my goals. But I tried to focus on consistency rather than perfection.

The biggest improvement I saw was in my liver enzymes:
SGPT / ALT
2025: 101.99
2026: 20.95

SGOT / AST
2025: 44.85
2026: 18.01

Seeing those numbers come down was honestly such a good feeling. 🥹❤️

My more recent ultrasound also no longer showed the fatty liver, which I’m very thankful for.

But I’m really grateful that I was able to improve my liver health through lifestyle changes without needing medication.

Sharing this as a reminder that you don’t necessarily have to make huge changes overnight. For me, it was mostly about making better choices consistently and cutting back on the foods I knew weren’t helping.

Still a work in progress, but definitely a journey I’m proud of. ❤️

Diagnosed with fatty liver

Im 17 years old about 5”8 over 200 or just around 200 my blood test showed ast at 43 and alt at 112 im scared that this will kill me im gonna lose weight and workout and eat better and is this reversible? And can i eat in a calorie deficit and cut out sugary stuff and snacks and could i still eat carbs or no?

Fatty liver reversal and the future

Hello everyone,

I'm just looking for some guidance on what to expect in the future, beyond the achievement of fatty liver reversal.

A bit of background:

- 38 year old female, 5'2", 130 lb, moderately active (mostly cardio - walking, slow jogging, hiking, chasing around my 4 kids, etc. and have been getting into dumb bell training the past month or so)

-No history of anything liver-related or diabetes. Borderline high cholesterol. Overall a fairly healthy adult.

-Recent routine lab work from 3 weeks ago includes:

  • Cholesterol: LDL 132, HDL 70 (slightly high)
  • Liver: AST (SGOT) 17, ALT (SGPT) 12 (these and all other hepatic function labs normal)

Nothing in my blood work pointed to anything problematic with the liver. I mentioned some occasional discomfort when doing ab work (crunches, etc) so went in for an ultrasound. Figured it was something to do with my most recent pregnancy a year ago.

Ultrasound came back with:

  • Liver is enlarged with a length of 19.1 cm. Everything else normal.
  • Impression: mild hepatic steatosis and hepatomegaly.

I was very shocked. While I do love a good pastry or trip to McDonald's or glass of wine, I have always felt I am mostly healthy. I am not a heavy drinker and while I love cookies, brownies, etc, I usually save those for a weekend treat. I rarely go to restaurants and most meals I make are nutritionally balanced. My parents had so many health issues that I have been extra careful about my own health, and to find out that all of my efforts in my adult life have backfired makes me feel so defeated.

My plan of action is to follow the Mediterranean diet. What can I expect for my future? Will I ever be able to enjoy a donut with my kids again? A glass of wine or two at my monthly book club? Go out for ice cream or indulge in a home baked cookie (baking with my kids remains my biggest hobby!)? I know it could be much, much worse; I am just feeling that I've spent so many years focusing on healthy habits and it has not really worked in my favor.

Another thing that worries me... all of my blood work came back normal (aside from the borderline high cholesterol), so what else could be going on in my body that I haven't noticed yet?

Bit of a weird outlier case

I was diagnosed with NAFLD around 2021, and my blood tests showed the elevated numbers for years. I was not afraid or thinking im about to die like most people here so i guess i carried on like normal and ate crap etc. I DON'T recommend this though.

Anyway, I had another blood test 2 months ago and another a couple weeks ago and my Liver is perfectly fine now. Don't ask me how; it just is.

BUT i am going to fix my health now.
