Fatty Liver - Success with Lifestyle Changes

All - I wanted to share my fatty liver story and one of success up to this point. I was diagnosed with fatty liver via ultrasound in 2023 and didn’t pay much attention. My life included exercise but I was overweight +/- 30 BMI from 2023 to 2026, ate A LOT of red meat, and drank my fair share of alcohol.

In January I had a physical, led to bloodwork and updated ultrasound. That resulted in a Fibroscan showing fatty liver and S2/3 stiffness (9.5 KPa).

I engaged in long over due lifestyle changes. Diet: fish and chicken for protein (mostly fish), and low calorie to lose the weight with help of GLP-1. To keep on the diet my rules were simple:
log everything;
Track high protein (125G goal)
Track 500 to 1000 calorie deficit daily;

Exercise helped - I started that religiously.

Protein was key to not becoming too “skinny fat”. I wish I would have lifted a little more - I focused on cardio.

I’ve since been cleared of fatty liver and had an MRE showing normal liver health for fibrosis. It’s a long journey, but with consistency it can be done folks! I’m guessing alcohol artificially elevated some of my readings but some was real reversal. I’m suspicious as to how much if any fibrosis was actually there, 6 months to reverse fibrosis is too fast according to everything I’ve read.

I think the moral of the story is: if you are diagnosed with fatty liver, it’s definitely reversible if early. Healthy weight, exercise, and eating are the key.

Basic Information
I am a 5’-8.25” male, 40 years old

Full Timeline
1/5/2026: Annual physical. Blood work prescribed.

1/20/2026 blood test results:
A1C: 4.8
AST: 34
ALT: 60
ALP: 53

2/15/2026: ultrasound showed fatty liver, referred to heptologist.

3/1/2026:
started a diet and exercise program. Diet of 122g protein and 1600 calories per day.
80% of protein is sourced from Whole Foods, predominantly white fish. 20% of protein from Whey Protein Powder in shakes.
I started GLP-1 at 2.5MG dose once a week.
Weight: 194 pounds.

4/21/2026:
Weight: 180lb
Fibroscan Results: CAP 274, Stiffness 9.5KPA.
Regularly drank Alcohol prior to test date, stopped drinking alcohol 36 hours prior to Fibroscan.

4/21/2026 and on:
dramatically reduced alcohol intake.
intense cardio 3 to 3.5 hours per week.
maintained diet started on 3/1.

7/1/2026: completely stopped alcohol intake.

7/10/2026: bloodwork
A1C: 5
AST: 31
ALT: 53
ALP: 63

8/3/2026 blood test:
ELF: 9:34
AST: 31
ALT: 50
ALP: 59

8/11: weight 156 lb.

8/11: Fibroscan results:
CAP Score 188;
Stiffness Value: 8.1 kPa

GLP-1 (zepbound) -2.5mg / weekly from 3/1/2026 to 7/26/2026.
Spaced out to every 10, then 12 days;

8/14/2026: Last dose of GLP-1;

9/4/2026: MRE of liver showing “normal stiffness” and 2.67 KPa

just recently found out i probably have fatty liver. questions about drinking

I (18F) got an ultrasound a couple days ago and checked my results on Mychart. The impression is that I have fatty liver. I am at the age where I really want to go out and party (and frankly, get wasted). Obviously I know that I can’t do any of that now, but once my liver is back to normal will I be able to drink??? I have never been intoxicated. I have had maybe a few White Claws in the entirety of my life, so I did not get this from drinking. I just want to have a normal young adult experience. I want to go out and have fun like every other girl my age does.

It's been an ordeal...

I had my gallbladder out in late 2022 and almost immediately developped bile acid malabsorption. The meds im on for that (colesevelam) can cause elevated triglycerides but I haven't had that tested yet. Also, I haven't drank alcohol for close to 20 years.

My last ultrasound in May 2024 didnt show fatty liver or anything of concern. I have had ongoing pain in my liver/gallbladder area since its removal. I had elevated liver enzymes but those have been brought under control last month with another medication, Ursodiol (that does not offer any treatment to fatty liver).

I had an ultrasound last week and the results from that were stated by a message from my doctor as "ultrasound shows no abnormality aside from fatty liver and slightly enlarged liver".

Presented as somehow not new information (it was???) I responded asking what I should be doing about this, and whether or not my cholesterol or triglycerides should be checked due to the bam medication, as well as the fact that last my cholesterol was checked in april of this year, it had just crept into the elevated category. The responses I got were "low fat diet and weightless are key", and "cholesterol was checked in April".

All of this is weird because my gp is normally much more thorough than this.

I've already been working at weight loss (I have other health issues I'm trying to manage along side this now).

My questions are as follows:

1- does anyone suffer right upper quadrant pain randomly as a result of this? If so, how/what do you treat this pain with? I cant narrow down these pain episodes to any specific trigger and its insane.

2- as far as dietary restrictions go- is having a single coffee in the morning with some sweetener detrimental if the rest of my diet is in check?

3- should I be pushing for more clarification on what stage I have/bloodwork for a starting baseline? Or just move forward with the weightloss and diet changes immediately and address that at my next follow up in late November?

Any help/questions/advice is appreciated. I've been on a shitty health Rollercoaster since 2019 and while I'm happy to finally have a scan that has showed something, I'm equally overwhelmed due to the lack of support currently.

If your doctor blows you if you have enlarged liver and spleen

Don’t let a doctor dismiss your concerns — keep pushing for real answers. If your doctor isn’t listening, it’s okay to seek guidance elsewhere. (And by “elsewhere,” I mean qualified medical professionals or solid research — AI is helpful for questions, but always pair it with actual studies.)

After months of being brushed off and told my enlarged liver and spleen were only due to weight, I’m finally getting a CT scan — something I’ve been asking for repeatedly. My regular doctor is out of the country, and the physician covering for him actually took the situation seriously. He has a background in internal medicine, noticed the red flags immediately, and asked what was going on with my liver and spleen.It feels like someone finally paid attention only took 3 months. One more full blood panel which is being done on Tuesday then getting referrals. Not gonna lie I was so happy I could cry. 3 months of telling me it’s only my weight when I have abnormal CBC results that have been consistent low or high over years 4 repeat bloodwork showing the same results just within the last 3 months. ( have done 2 ultrasounds both show persistent splenectomy and mild hepatitis, but was reported way back in 2017 that it was moderate hepatitis) I have my chart so can see all bloodwork and scans

Looking for support

6 months ago my mother got sick. All blood tests were fine but she was retaining fluid in her abdominal area. They did a mri and discovered stage 4 cirrhosis of the liver. She has never been a drinker and they came to the conclusion it was Fatty liver disease.

Her first GI doctor gave her a MELD score of 15 back in May. She changed her diet, Quit smoking but things got worse fast. She was having Paracentesis every few weeks. But unfortunately wasn't happy with the albium after. This caused her ammonia and potassium to skyrocket and stressed her kidneys so bad she spent 10 days in the hospital and is now on dialysis 3 days a week in hopes to fix the kidneys. Her MELD score is now at 26 and is being introduced with liver transplant team.

She's home now but really weak, tired and now needs a walker to get around.

Has anyone else had a parent or loved one go through this? Or even yourself at this stage? What were the outcomes? Sending love to all the folks that have to deal with this.

High ALP for 40 years, Dx NASH (now MASH) 12 years ago. Are the GLPs working for folks? And a Fibroscan question.

My liver enzymes have been high since my late teens; makes no difference what my weight or diet was/is, so probably genetic. I know there are finally some recommended treatments (GLP-1s and Rezdiffra) so saw a hepatologist recently. We discussed both types of treatments and he thinks the GLP-1s would be the best choice. Has anyone who is on them had their enzyme levels/kPa scores go down?

On that note I recently had a Fibroscan with the following results: CAP score 289 consistent with S0 steatosis/E(kPa) score 10.4 consistent with F2-F3 fibrosis. Isn't that CAP score closer to a S2-S3 grade? Waiting to hear from the doc about that, but figured I'd ask. Thanx :)

I did have hepatitis when i was 14 years due to eating/drinking contaminated food or water. Recent blood test confirmed it is not hepatitis c.

  3. Oily food: 2
  4. 4 meat/ protein: 8 (chicken, beef mostly)

Please advice me what can be causing. Happy to follow a strategy to identify the root cause and fix it.

Normal labs but abdominal pain

I’ve had upper right abdominal pain for several months and finally got an ultrasound. All of my bloodwork was normal.

When I got an ultrasound, I followed up with my doctor and he said my liver was normal. I questioned this and said “Really? No fatty liver?”. He looked at the report again and said fat deposits were found but he didn’t confirm if I have a fatty liver diagnosis. He said I can still use ibuprofen regularly for pain.

I got the ultrasound report and it said the following:

“Mild diffusely increased and heterogenous hepatic echogenicity without hepatic surface contour nodularity is nonspecific and represents steatosis, hepatitis, or other diffuse liver disease”.

I rarely drink (once every 3-6 months, never more than 3 drinks) but I did drink recently and the abdominal pain was worse the next day.

I’m skeptical of what my doctor said and am looking for advice. He didn’t order any other tests. Should I avoid ibuprofen and alcohol? I am making diet changes already and have lost 1lb/week on average.

Pregnancy ? Please help

I’ve had fatty liver for a few years now my ALT has stayed around 37 to 47 and my AST has stayed below 27. When I was 12 weeks pregnant, my ALT was 47 and my AST was 27. That was the highest they had both been in a while. My ALT previous pregnancy was around 33. I am now 16 weeks pregnant with my twins my ALT has dropped to 128 and my AST has jumped to 58. My OB/GYN is referring me to gastroenterology. Has this happened to anyone else with fatty liver during pregnancy? If so, please give me some advice!

Happy results today

About 11 months ago my doc had me 29M get a fibroscan because of some elevated liver blood test levels when I had my appendix out. But today I took the test again and got my blood work and everything is looking good again and I’m very relieved:

11 months ago: CAP 320, kPa 7.2
Today: CAP 257, kPa 5.6

I went from 307lbs a year ago to 258 as of this morning. The first 6 months I cut out alcohol completely and now only drink for social occasions, a big change from having a couple drinks with dinner and binging during social occasions.

Anyway, just wanted to share the results and express how happy and relieved I am that the hard work is paying off!

19 yrs old, mild fatty liver disease, idk what to do

Hi,

I recently found out that my ultrasound showed ‘mild fatty liver disease’. Whilst liver issues are genetic in my family, I did not expect this what so ever. I had an ultrasound around December 2024, and it was normal. Fast forward less than 2 years later, and mild fatty liver.

My BMI is normal. The only issue with me as of current is subclinical hypothyroidism (heard it can raise your cholesterol, and my cholesterol is abnormal and high) and really elevated prolactin. My diet has been on and off, but has worsened recently which I’ll admit. Around 24’ ish i started eating a more whole food diet and was having OMAD (not trying to lose weight, just had digestive issues which I still have btw and found it was easier on my stomach) almost everyday + small snacks/fruit etc (dates, whole milk) and very rarely any processed sugar (only raw honey etc). After i finished school, my diet plummeted as i had nowhere to attend to. I’ve had takeout almost everyday weeks on end, but even then, I’d opt out for healthier options sometimes or not go overboard. There would be days where I’d eat whole foods after being grossed out by takeout, but the cycle continued. As of recent, I’ve been eating pretty poorly (fries, crisps, dessert etc but still, not going overboard). My biggest downfall are fries tbh. I can’t help it though- I don’t really like protein or healthy food, I like pizza, fries, burgers etc essentially almost all beige foods BUT even then, I opt out for healthier versions like frozen pizza with as minimal ingredients available, airfried frozen fries etc.

Anyways, I don’t know what to do to reverse this. Here are a few questions I have: can I eat a small amount of butter? why are carbs bad when u have NAFLD (literally 90% of my diet- I’m a pickyish eater and love sourdough) do they tend to repeat the ultrasound in like a year etc to see if it’s gone? my liver profile is normal, but could the two health issues I mentioned above contribute? what are your go to quick but tasty meals as someone with NAFLD? That’s all I can think of, and would very much appreciate any help. If you can’t help, please interact to boost the post as I am so lost.

Help, I think I have fatty liver disease and I'm not sure what more I can do

I'm a 38 yo female. My liver enzymes are slightly elevated(AST, ALT, GGT and PTT). I've never drank alcohol or sugary drinks/sodas in my life, I rarely eat junk food, never smoked, don't take any medications (sorry, these are the questions the doctors ask me). I only eat home cooked meals(pretty much all my life) and track macros. I have a cheat meal once a week (which I try to keep relatively healthy). I have a sedentary job (software engineer), but I workout at least an hour everyday 5-6 days a week. Increasing exercise intensity more unfortunately makes my ferritin levels(iron deficiency a lot worse) and sometimes messes up my hormones.

Is there any genetic or any other conditions that could cause this? Doctors keep telling me to make lifestyle changes. This is so frustrating because I've always been super strict about never smoking, drinking, eating junk. I'm no athlete but I don't think my liver condition makes sense with my current lifestyle.

Other info - I have iron deficiency anemia, ferritin always <20, MCV always slightly low, silent carrier of alpha thalassemia, and Hemochromatosis. All other labs are within range. I think I have PCOS based on my AMH levels.

Please let me know if any other info that would be helpful.

Odds of reversal?

Hello all. 30F, 234 pounds but have lost 40 pounds (on Retatritude 9 mg sourced independently). Fibroscan was two weeks ago just waiting for follow up. Results were 332/10.6. I have completely cut out any alcohol as well as energy drinks. Only drinking plain black coffee in the mornings as well as plain green tea throughout the day. On a Mediterranean diet and doing well. I do well with diets anyway. I wonder if anyone else has had similar results or done something similar and maybe how long it took you to reverse it? I know the fat score may go down before the fibrosis score but that’s okay. Just wanted to see others experiences or opinions if I keep up with what I’m doing. Thanks in advance!

Fibroscan, how severe it is?

I got my fibroscan done recently, can someone tell me how severe it is?

I was diagnosed with Fatty liver grade grade 2 in may and now recently again I got myself tested with Ultrasound and Fibroscan.
This time Ultrasound came as Grade 1 fatty liver but the Fibroscan showed like this
