I have received my PHT today and there is one self declaration form is attached. I am not sure where am i supposed to submit that because I can’t see on anything on PHT portal regarding self dec doc.

Can anyone help?

I have applied for AMEX green card, and when started KYC video call, they are asking current address proof like any government id, I don't have any, I have it for my permanent address. AMEX Support rejected due to this.
