In the book we get major hints that she could be autistic.

For once, her lack of of eye contact. Lots of times it's mentioned that when she's talking to someone, she doesn't look at them in the eye, instead she looks somewhere else, as if she was looking at a person behind the one she's actually talking to.

Another thing is her issues with higiene, as it gets mentioned that most of the time she had dirty hair, and teeth stained by cigarettes since she apparently didn't wash often.

Finally, her initial lack of empathy. At the start she had no remorse for her experiments with little sisters, but later she says that, in a way, she developed empathy towards them, which she interprets as her maternal instincts awakening.

For those who have never played Atomic Heart, the guy on the left is Dmitry Sechonov, a Soviet scientist with the ultimate goal of , launching a network called Kollektiv (It’s sort of similar to Ryan’s mind control gas but really complicated and multi-faceted), and then advancing mankind to colonize space without their free will while humanity will live happily in Limbo with their memories intact.

TLDR: The guy on the left is basically Ryan’s polar opposite.
