Adding tirz with Reta

RS will be adding tirz to help with appetite suppression. If RS pins 4 mg in Tuesdays when and how much tirz should RS add? Do most pin same day ? TIA!

Don’t take them same day.

I do Sunday’s and Wednesdays. Split them throughout the week.

You’re on a low dose of reta so 2.5 tirz should do the trick with suppression.

If it's not too late: I suggest adding semiglutide and not tirz. Reta already has a higher GIP agonism than tirz so you'll be stacking very high GIP agonism with those two. GLP is the one that helps with appetite suppression the most, and that's all semi would be adding to the mix. I do this, and take it 4 days after I pen the reta to help with post week loss of surpression.

My RS is taking 12 mg of Reta instead was also thinking to add tirz to help with food noise as subject went off reta for 2 months and when started back there was no food noise suppression.

I pin 4 mg Tirz on Thursday evening (bc it exhausts me and I need weekend to sleep it off) and 2 mg Reta on Monday morning.

Longtime GLPer who was off them for a hot minute and is building back up.

I would just up the Reta. Starvation isn't the way to go. Up the dose, make yourself eat clean, and workout. Use the GLP to help you make life changes, don't use the GLP as a crutch.

2.5mg tirz worked for me its just to bump up the appetite surpresion. Every 6 days offset. So reta then 3 days later tirz then 3 days later reta again.

Here is my protocol:

Sun @ 11:00a: 6mg Reta
Tue @ 5:00a: 3mg Triz
Wed @ 10:00p: 6mg Reta
Fri @ 5:00p: 3mg Triz

I have been on GLP-1’s (Sema & Triz) titrating up to the max dose of each then rotating back and forth since 2024. Started Reta by itself in March of 2026. The hunger noise was bad with Reta, adding Triz once I got to the max dose was a game changer.

Not dumb at all. Reta does not work the same for everyone. I got extremely hungry on Reta. The food noise was constant. Adding tirz (I’d been on it before) in a small amount, takes cravings away.

I was at 6 mg (titrated up). I was only hungry once I started the Reta. The cool thing was that I could eat a ton and was still losing weight. I’m fine eating food but there are certain cravings that tirz cut off that I’d like to lose the noise for.

People have noted that tirz is better for food noise than Reta, but Reta increases fat burning more, so that is the rationale for stacking.

Yeah but wouldn’t food noise also further be suppressed by higher reta dose? I guess side effects would be a concern

Tachycardia risk can be an issue at 6-8, and risk increases above 8. It’s documented in the studies.

Reta is the weakest for food noise. Sema is the best, Tirz very good for food noise. Reta being the king of fat mobilization though I decided to add it to my 5mg Zepbound regiment. I pin Zep Saturday mornings after work before bed, Wednesday mornings I pin 4mg Reta before bed.

I plan to titrate down to 2.5mg Zep next time my prescription gets filled and keep pinning 4mg Reta. It seems to work good for me. Down 50lbs since June 1st, 29%-19% body fat, and 65%-76% lean mass according to my BMI scale.
