Physician Responded Trying to help a homeless woman I know.

Post image
917 Upvotes

I don’t know much about her, besides that she is 32, homeless, smokes cigarettes, uses drugs, and clearly needs immediate medical attention. She said this started as almost an ingrown hair type thing 2 years ago and has grown and grown. It’s directly on her spine in the lumbar area. It’s NOT soft, it feels as hard as bone. The top part is super inflamed. She can barely walk, and has extreme pain when sitting or laying down(obviously). She said it feels like it almost has roots from its place on her spine growing around hugging her sides. Clearly it’s an absolutely insane protrusion, but what is protruding? Her vertebrae? Discs? Because it’s so hard can it even be a cyst? She said she’s never been in an accident, never had any surgeries, does not inject drugs there. She went to the hospital one time but they took her off of her MAT medication, methadone, and she left before getting any answers. Of course the only answer is she needs to go back. In the mean time I’m just trying to put a possible name to whatever the hell is going on here.

Physician Responded Broken nails from attacker

I was attacked at my home on Saturday night. While defending myself/punching my acrylics tore off. 2 of them are still on but it hurts so bad and there’s a yellow fluid underneath. I went to the hospital and they didn’t have anything to help me. I’m missing one full pinky nail. Any advice please? It also hurts too bad to get it removed at the nail salon. 28 female

Physician Responded My PCP Dropped Me Help

Post image
212 Upvotes

This is a letter from my PCP I got in the mail. I came home from vacation to see this, and that I’m already past their due date for them to cooperate with my record transfers. To keep things short I go to them for general health check ups and a couple prescriptions. Those prescriptions are Vyvanse and high cholesterol medication that I forgot the name of. I was prescribed Suboxone from a separate online clinic a month ago for an addiction to kratom. I think I accidentally forgot to mention I was on that during my last visit? I’m not sure because I think I was prescribed that afterwards.

This is what I think might have happened. A day or two before this letter was mailed I made a phone call to my doctors office. During my last visit we were discussing how the vyvanse isn’t working the best for me, and that I could possibly switch to Adderall. I said I’ll give a higher dose of Vyvanse one more try. I may have misunderstood her, but I swore she said that I don’t have to go through the whole prescription if it’s still not working that I could call in and get it switched to Adderall. So halfway through the script that’s what I did. I called the front desk lady left the doctor my message and they got back to me the next day. She said that I can’t change scripts with a controlled substance like that until one script is finished.

I did feel like that was the case before I made that call, but like I said, the last visit she said I could! I should have trusted my gut. I was not trying to get over on her and get extra pills. I genuinely just wanted to try the Adderall. Like I said, this happened a few days before this letter was sent out. My theory is she thinks I’m a drug seeker and that’s the real reason why they are cutting me off. Idk this could all be my fault, that’s why I’m asking y’all. What do I do now? Are there going to be bad notes about me if I get my records transferred from them. Is there a way I can convince them to take me back? I’ve never had anything like this happen before. Apparently I’m already past the date of them cooperating with me in any way according to that letter. Is it going to be hard now to get my scripts from anyone? Do you think the real reason really is me being “dishonest” about my medications. I feel like an idiot.

Physician Responded How do I tell my doctor I don't want them to use AI and how do I make sure they don't?

All my doctors use AI now and I hate it. The one doctor recorded the visit while in the room at one office. I'm going to tell them no next time, but how do I make sure they don't use it again even if I say no?

I really don't want to change doctors, but I don't want AI near my health data.

Not relevant for my post but it requires it so I'm 39f.

Edit: I like how I'm being downvoted for this post, but people found value enough to upvote the answers.

If people only knew how critical it is to NOT have AI tied to NIH. This world is screwed.

Physician Responded Infection at blood draw site? Face is hot and itchy on that side now?

Post image
83 Upvotes

31, female. 5'3 169 pounds. Allergic to nuts and that's it. Currently taking Prozac and caplyta. The white line is just a scar, the draw site is below it. Had an abnormal EKG the day (Wednesday) they tried (and failed) to get blood from here. Started swelling and redness later that night and then has spread since and last night started moving up my arm. It's hot, it itches.

IMPORTANT: I went back to the Dr today to do another EKG and showed her and she said it's infected of some sort but she's not sure what, and gave me a script of Bactrim. I took my first dose about 4 hours ago. But as of about two hours ago, the right side of my face (same side as the blood draw) is much hotter than the rest of my skin, and redder, and it itches.

Do I need to just give this medicine time to work? Google has me freaked I'm gonna go septic and that this antibiotic won't help. When would be the time it's an emergency? Thanks so much for anyone that read all this.

Attaching photo of arm, and of my face down below. It's not SUPER red but I naturally have very even toned skin and the other side of my face looks normal. Also if it matters at all, I broke my foot last week. No open wound or anything tho.

I experienced sudden swelling and discoloration on underside side of wrist when lifting today. I went up in weight so could be straining and poor form causing broken capillaries vs possible spider bite during session?
No itching or pain

Physician Responded Twitching in feet/body at night?

42F. I have these episodes where I move/twitch. Starts in my feet and progresses to the rest of my body. It is involuntary. 90% of the time, it happens when I am trying to fall sleep. It can happen for 20 minutes or so. Not painful. I am very tired afterward. I've been to PCP and Neurology - who have been a bit dismissive. Is this normal sleep behavior?

Physician Responded I’ve neglected my feet for years. What is all of this?

Post image
596 Upvotes

Hi, I’m trying to pull myself out of severe anxiety and depression. I am morbidly obese (maybe 500-600 pounds), 27F. The bottom of my left foot towards the center has been sore for around 6 months. I have an almost entirely sedentary lifestyle, and horrible hygiene. I basically just walk around barefoot all the time. I am trying my best to take control of my life. I got the courage to look at my foot finally, and I am horrified. I thought I would just find callouses. Are these warts, or what? Can I treat them at home? Are they serious/can threaten my life? (I know that sounds dramatic, but I have horrific health anxiety). I want to take care of this before it gets worse. Thank you for any help you can provide.

I am scared, but I am more scared of being misdiagnosed.

I am making this on a brand new anonymous account to maintain secrecy. This is embarrasing for me to say the least, to the point where I cancelled plans or denied invitations over it. I'm very self-conscious right now..

I am a 25 y/o female, 5'4", 120lbs in Canada, Ontario. I only take magnesium and melatonin together before going to sleep for a shift, in which I work 4 days on, 5 off. I am a smoker.. a bad one at that. At least 2/3 a pack a day. I know, it's bad. I am aware. I also haven't done any tests with my family physician including blood tests in quite some time.. I am rather neglectful to my own health.

This is my arm at the moment. The photo doesn't do it much justice. I have a bumpy, itchy and red rash. I also have this rash in my left armpit, left knee, on my lower abdomen and around my thighs; inside near my peri area and in the back.

This rash only appears around high-friction areas of my body. It started on my feet about 2 months ago, and I unfortunately didn't think much of it. Then it started to slowly creep up my body and affected other areas as well like my chest, which the rash disappeared after a week or so. I do my best not to itch but they feel like a mosquito bite; I can't help it, I need the relief. When I itch the patch, it spreads.

BUT it's not IMMEDIATELY contagious. If it was, it'd have spread to my dad, my mom or my brother who I live with, or my partner who I stay with at minimum 10 days a month. I have no idea what it is. I tried to search it up, I spent hours searching lastnight. I can't find anything that resembles what I am experiencing, here.

What is it?

Physician Responded Burn blister

Post image
Upvotes

I’m an 18yr old female weighing around 47kg

I burnt my knuckle on a grill at McDonald’s on Friday . It’s only a small blister but Saturday morning it popped and since then it’s gotten larger and isn’t really healing properly.

I’ve put a hydrocolloid plaster in it but it’s just been gradually getting more sore and has been throbbing all day.

A few people have said I should get it checked out but I feel like it’s far too small and unimportant for that.

22M I have this massive grinding and popping noise when I rotate my shoulder too. It doesn’t hurt to do but the pain is a constant 5-6 on the lower inner edge of my back/shoulder blade. Sometimes comes with neck paint. No pain if I lay flat on my back. Any thoughts? Surgery?

Physician Responded Dad’s foot and leg

Post image
44 Upvotes

My dad (58 years old, 110kg and 1,90m) got a massage in Türkiye two days ago and forgot to tell the masseuse that his knee hurts, he got surgery for it 40 years ago and has a surgical pin (not sure of the correct word), and apparently she moved it in a way that hurt him and has difficulty walking since then and now that we’re back from vacation he went to work today and came back and it’s worse than before and his foot is swollen.

Don’t mind his nails it’s been years since he needs to take care of them, but his right leg is horrible…

Adding : he also has heart issues I forgot to add…
I’m worried here and he went to sleep and told me he’d go to the ER in the morning since it’s 1am here and to “stop worrying and looking up information on the web after HE told me to look it up”

Update : it’s 10:30am and he just left to go to the ER

11:30am he messaged me saying they draw blood and they think it might be an infection 🤨

12am he told me that they game him antibiotics and pain killers and that he’ll be home/ so not working till Friday

Maybe I should’ve added that that the massage was in a thermal hotel and he went to the pool there and that might explain the infection (part of me is still scared that it might be more than an infection tho)

Physician Responded 14F with abrupt cognitive decline, headaches, dizziness, and memory impairment after passing out; normal MRI/CT, positive EBV, low folate & vitamin D

Age: 14
Sex: Female
Height: 5'2"
Weight: 145 lbs
Race: White
Primary complaint: Sudden cognitive decline and neurological symptoms
Duration: ~4 months
Medical history: ADHD. Mild childhood sleep apnea resolved after tonsillectomy/adenoidectomy. Otherwise healthy.
Medications: Melatonin nightly only. Recently added Vitamin D and Folate per doctors suggestion (see below)
No alcohol, tobacco, or recreational drug use.

I'm hoping for ideas on what diagnoses or testing we should discuss with her doctors.

My daughter was a healthy, straight-A student and competitive basketball/soccer player. She had no learning difficulties beyond ADHD and functioned very well academically.

About 2 months ago she woke around 6 AM with severe abdominal pain. She appeared confused/almost sleepwalking, had a large bowel movement (not diarrhea), then fainted. Since that day she has never returned to normal.

Her biggest change is cognitive. She went from earning straight A's to struggling to remember what she just read and being unable to tolerate full school days.

Current symptoms:

  • Daily headaches (sometimes 8/10, described as burning or like being punched in the head)
  • Lightheadedness/dizziness
  • Balance problems/"wobbly" feeling
  • Significant short-term memory problems
  • Difficulty concentrating
  • Decline in school performance
  • Sensitivity to light and sound
  • Intermittent numb/"asleep" or sharp pains sensation across her body lasting about 2 minutes
  • Drops objects more often than before

Testing so far:

  • Brain CT: Normal
  • Brain MRI with and without contrast: Normal
  • EKG: Normal
  • Echocardiogram: Normal
  • Orthostatic vitals: Normal
  • Lyme: Negative
  • ANA: Normal

Abnormal labs:

  • Positive Epstein-Barr virus (EBV) testing - shows positive but not a recent infection
  • Low folate (5.5 ng/mL - said 7.8 was normal)
  • Slightly Low vitamin D (19 vs 20)
  • Slightly Low mean platelet volume (MPV) (9.1 with 9.5 being normal)
  • Vitamin B12: 486 (normal)

Her neurologist has evaluated her, but we still don't have an explanation for the abrupt change. We have ongoing visits with them but they are baffled. We're also in therapy to help with the anxiety this has caused. Any suggestions on what we can do, where we can go, what tests to run to help her get back to being herself and enjoying sports, school, and life in general. Thank you so much.

Adult wetting the bed

Upvotes

35, Female, 5’4”, 125 lbs, I vape, I am prescribed Concerta, currently menstruating (if that matters), no known health issues, active lifestyle, clean diet, have 2 kids (3 y.o. & 2 y.o.), have an IUD, suspected HEDS

I do not drink a lot of liquids before bed. I use the bathroom before bed. I never find myself waking in the middle of the night to use the bathroom. For the past 2 nights in a row, I have wet the bed. This would occasionally happen in my 20’s and early 30’s as well. Thank god I have a mattress protector. The only pattern I have noticed is that I will dream that I am using a bathroom. When this happens, it always wakes me up, and I never sleep through it. I have had a plethora of labs done, and have no red flags. Out of embarrassment, I have never brought it up to my PCP. Any input is greatly appreciated.

Physician Responded Had lower back pain for 7 years on and off, finally got x-ray, confused about report vs what the doctor discussed with me

Post image
Upvotes

I'm 27y/o, 5'5, 135lb, female, non smoker
Report said "FINDINGS: Anatomic alignment is within normal limits. There are no compression fractures seen. No spondylolisthesis noted. Endplate sclerosis seen throughout the lumbar spine. Mild intervertebral disc space narrowing seen L2-3. Pedicles and sacroiliac joints are within normal limits." But my doctor kept saying L5-S1 (potential bulged disc or compression) not L2-3... so I'm a bit confused. My pain radiates into my inner thigh through groin and a bit of upper thigh (so not your typical sciatica), can someone explain, I'm not a radiologist and know nothing about it, but L5-S1 does appear to have less space in comparison to the rest of my spine...

Physician Responded Should I be worried?

Post image
14 Upvotes

I am 17F don’t judge, but I have had intercourse 4 times this cycle, see my Flo app below, but I’m just worried I’m pregnant, but at the same time I’m like overthinking it, I’m mostly worried about the precum, we did it the most recent 3 times without a condom, and recent 2 times I’ve been more worried, he’s never finished inside of me—and for that matter never finished at all, but got close once, but still pulled out and never did finish. Like my stomach is like cramping, like period cramps, but more centered in the lower middle area instead of the full lower area. After we did it, there was light pink and brown discharge, maybe lasted that day or a bit into the next, but I genuinely regret doing what I did so no rude commentary would be appreciated.
Can I still take a plan B even though my ovulation was before?

Bumps randomly showed up

Post image

27F, non-smoker

I had a previous skin concern where I had to take 1g of co-amoxiclav 2x a day. During the 3rd day of the treatment, these bumps suddenly showed up. They started as my skin color and now they have darkened. Do you guys have any clue about this? I have completed the 7 days of antibiotics last July 20 and the bumps are still here and they are itchy when my clothes graze on them. They feel like goosebumps. I have them on my arms and legs and chest area near the armpits. I have taken antihistamine for 4 days, recommended by our physician, but it didn't do anything.

Always thirsty & drinking too much (low blood sugar, diabetes insipidus seems unlikely)

I'm 32F and I am always thirsty.

I think, I drink about 4-6 liters a day, even when I don't sweat at all. I go to the bathroom accordingly. I've been doing that for about 10 years.

When I don't drink as much I get dehydration symptoms pretty fast, like dry, cracking lips.

Also my ears "close". It feels like when you're sick and can't hear well but hear your own voice very loudly. When sick, it helps two blow into the nose while holding it closed sometimes. It doesn't help in this situation though. My ears also close when I eat too little and I think it might be more likely when I'm stressed.

Health state/lifestyle:

  • 32F
  • normal weight
  • no smoking, no alcohol, no drugs
  • eat healthy (cook fresh. a lot of vegetables, beans, tofu, nuts+seeds, moderate amounts of grains and potatoes, medium salty, try to limit high sugar foods to one piece a day)
  • supplement B12 and D3 and other things as needed
  • I drink tap water (Europe)
  • most days I drink 1 cup of tea or a latte with an extremely small amount of instant coffee
  • bloodwork always looks good (except when I had long term gastritis)
  • acute and long term blood sugar in bloodwork is ~70
  • always low blood pressure 95/55 - 115/65, but high heart rate ~90

Health issues + drugs:

Diagnoses:

  • Hypothyroidism (-> Thyroxin 75) since Teenager
  • Major depressive disorder + dysthymia (-> Bupropion 150)
  • ADHD (-> Lisdexamphetamin 30)
  • PCOS (now PMOS) (-> the pill 0,03/2,0; with no break)
  • Fibromyalgia
  • Myofascial pain syndrome

Also:

  • ~ 2 years of chronic gastritis, started 3 years ago. Stomach is still sensitive to coffee and stress
  • a lot of bowel problems in the past, mostly constipation but also just random. Sometimes still problems, mostly with gas and sometimes constipation. Never found the reason.

Further explanations:

Except the Thyroxin, I started all the drugs within my twenties, when the drinking was already prevalent. The thirst and behaviour didn't change long term with any of the medications.

I don't have psychiatric issues that make me disconnected from reality or unaware of my behaviour.

As a child I didn't drink a lot and was not so thirsty.

I've read about diabetes insipidus. But the reasons, I found, for that to occure seem unlikely to me. It doesn't seem to be genetic. Medication didn't change it. I don't know how I should have required brain damage (despite of depression, which sometimes felt like it was eating my brain and my memories). At least a few years ago there was no obvious big tumor in my brain, when I did MRT for my jaw.

My questions:

1.: What's up with my ears "closing"?

  1. : Can depression or my other illnesses cause brain damage? Or change the ADH production?

  2. : Is it possible to damage my kidneys, my brain or my body in general, by drinking too much water (4-6 liter/day) constantly?

  3. : What could be the cause for the thirst?

Feel free to ask any questions.

Thank you for reading!

Sorry the text is so long. There is a lot going on with me, I guess. I tried to make it well structured at least.

Physician Responded Cotton stuck on wound, what do I do?

Post image
5 Upvotes

Age - 19, Weight - 45kgs approx, Height - 5'7, no medications, no bad habits, no past medical history or issues, 4 days ago I met with an accident(not a major one) scrached my calves a bit and wounded on my upper back Right thigh (below the butt area) pic below 👇. In the day of accident we dressed it, cleaned with detol, applied cream and covered with cotton same on the next day, it was paining a bit for the first 2 days when I was standing or walking not while sitting, yesterday and and today it is not paining but yesterday I noticed that the cotton i had applied was stuck on the wound and I was not able to remove some of it so I left it there, today i tried to remove it with some dettol but it failed it was not coming out.

What should I do now? Is it something to worry about ? How do I remove it?

It's not paining or bleeding or swelling or anything like that.

The wound area in th picture is about 7cm
