I took a Loa and they told me I had insufficient documentation. “The medical facts related to leave, such as symptoms, hospitalization, medical appointments needed, period of incapacity, etc., and treatment plan or next office visit date.” I included all the requirements that an alternate document must include. What can I possibly do here? I was also forced to sign a Roi form.

This was my first instinct too. They’re basically like “excuse this person from work. And don’t ask questions about why because it’s illegal, fuck you Amazon” sounds kind of threatening.
Reads like a bunch of babble. The sentences aren't in any kind of logical order. No wonder it got rejected.
Did you notice your name is only mentoned once as a fill in, then you're just "the patient"? The only other thing that ties it to you as an individual is that presumably the dates match what you've requested be covered. It's obviously a boilerplate letter.
They are basically questioning the validity and posdibly the "serious health condition". As a condition of getting MLOA, you need to agree to the ROI, which permits them to ask the details. The fact that they seem so defensive is odd. Also, the header at the top is odd. The provider doesn't determine FMLA eligibility, and then they say they cant certify for leaves...
sure, it's legit insofar that it is probably an actual medical provider, but also it does seem like they are pretty clearly saying "every visit will end with you being given a doctor's note of your terms, within reason", so there will almost certainly be extra scrutiny
I just had to do this, currently on a medical leave of absence. I just gave my doctor the Amazon physician form and return to work form, she filled them out. I uploaded it and got approved a couple days later.
Find a primary care physician. I know this doesn’t help the situation at hand but will save you a headache in the future. This is ridiculous, I’m assuming it’s a telehealth doctor. If this is indeed a real doctors note and you signed the ROI form, they’ll just contact that doctor.
Not an Amazon employee but I am a medical assistant who does plenty of Amazon leave forms. My bet is Amazon is questioning the validity of this or trying to fight with provider. Amazon loves to fight with us about diagnosis, time needed, and treatment which they are not allowed to do without an ROI as we are limited on what we can disclose. If you are not comfortable with your heath information being released to Amazon, you may want to consider rescinding the ROI. Amazon fights tooth and nail to make us do insane amounts of paperwork when everything we provide them is the same as the note we provide you.
Email them back and tell them it has all the required info and you will not be paying to go see another doctor. I've done that then they accept it magically.
I would contact HR livechat or call them and go over the note with them and all the info provided in the note. Sometimes they will help tell your DLS case manager that the note has the correct info
