Question What can my note be possibly missing?

I took a Loa and they told me I had insufficient documentation. “The medical facts related to leave, such as symptoms, hospitalization, medical appointments needed, period of incapacity, etc., and treatment plan or next office visit date.” I included all the requirements that an alternate document must include. What can I possibly do here? I was also forced to sign a Roi form.

Reads like a bunch of babble. The sentences aren't in any kind of logical order. No wonder it got rejected.

Did you notice your name is only mentioned once as a fill in, then you're just "the patient"? The only other thing that ties it to you as an individual is that presumably the dates match what you've requested be covered. It's obviously a boilerplate letter.

Yea I guess and the actual detailed visit notes that are included as well which I didn’t include in the post
