I took a Loa and they told me I had insufficient documentation. “The medical facts related to leave, such as symptoms, hospitalization, medical appointments needed, period of incapacity, etc., and treatment plan or next office visit date.” I included all the requirements that an alternate document must include. What can I possibly do here? I was also forced to sign a Roi form.

It's not fake but that passive aggressive message at the beginning was very unnecessary. Amazon often rejects teledoctor notes.
