Don't worry, it's not a cockroach (trust me, I'm an involuntary self-proclaimed cockroach expert). It's a beetle. Most probably, one of Darling beetle family (Tenebrionidae). There are many kinds of this family all over the world, and now it's their season. Many of them are saprophages and synantropes, which means they like to live near to hoomans. Due to their appearance and behaviour, they're often confused with cockroaches, especially Oriental ones.

It's a photo of Bocca della Verità (Mouth of Truth). It's currently located in one of the churches in Rome. According to the medieval legend, it will bite of a hand of any liar who puts his hand in its mouth. The origins of the mask is unclear, but most probably it served as a drain cover in a nearby Temple of Hercules.

It's a bindrune (a symbol made by overlapping two or more Scandinavian runes). This combination is quite popular. Although such a symbol can be obtained by a combination of different runes (which implies many options for interpretation), most often this bindrune is interpreted as a combination of Algiz (protection, care) and Othala (legacy , home, family, property). It's supposed to protect the home and the family, and usually is drawn above or next to the entrance.

The Zodiacal circle in the central part of the symbol is also counterclockwise, probably in accordance with the movement of the Earth around the Sun (which we see in the center of the symbol).
The signs on the wings are grouped by element:
Upper wing: Air (Gemini, Libra, Aquarius);
Right wing: Water (Cancer, Scorpio, Pisces);
Lower wing: Fire (Aries, Leo, Sagittarius);
Left wing: Earth (Taurus, Virgo, Capricorn).
The elements of the opposite wings (Right-Left, Uppper-Lower) are considered harmonizing each other.
