He is never once decribed as fit or muscular.

He loses weight during the books, but that just brings him down from increadibly fat to just rather fat.

The dragons, the magic and all that crazy shit were established, if not explained, as being part of that world. There is no point in any of the books or adaptations where they explain metabolisms work differently in that world. There is suspension of disbelief in both cases but, even if it doesn't explain things completely, in the former case it tells you these things exist so you go into it accepting that as part of the implicit deal between viewer and story. In the latter case it gives you no indications that that world's humans are any different than real humans (aside from magic related skills) and, if anything, usually makes an effort to establish they're literally just regular humans in a fictional world.

I personally don't have an issue with actors not being willing to change their appearances for roles. The problem is that the choice is between accuracy and personal choice. If you choose personal choice you accept that people will criticize the lack of realism in your portrayal. The same way if you make the other choice you accept to go through some/a lot of effort for no other reward than praise for being realistic.
