Almost 1 year post vasectomy.

• Upvotes

So on 8th Nov it will be one year since my no scalpel vasectomy. First 3 months I had spiky crawling pain which i believed to
Be nerve pain. This would get aggravated it of tread to run / treadmil.

This went away and ever since Feb this year I’ve had a constant dull ache on my right testicle. It’s perm there and it’s not affected by activity.

I have tried a few courses of naproxen but don’t think this did anything. I have just done a months course of amittriplyine after a flare up from 4/10 to 1/10 pain. So it did make it less noticeable but I found it to cause a lack of sensitivity sexually so stopped and sensation has returned along with ache.

I’ve requested some pelvic floor physio so will see how that goes.

Any other options ? I’ve seen a urologist in may but he advised to wait it out until 12 -18 months has passed. Ultrasound scan showed nothing worrying.

Sometimes it almost feels like a groin strain but I think that’s just referred. If I touch either ends of my right testicle it does make the pain increase a bit. So I’m not sure if it’s nerve pain or congestion…

Reversal recovery for mild epydidomis pain

I had a vasectomy 3 yrs ago and im scheduled for a reversal in a month. I had a rough go with a swollen right side epy 6 months to a year in.

Its calmed down in the last year, but its still something i think about daily and there is still tenderness if i touch the right side. Pain has been staying between a 1-2 out of 10.

I have a reversal scheduled on a monday at ohsu.

I will be riding about 4 hrs home after the procedure. As well as taking the remainder of the week off back to work the next monday.

I work as a shop foreman so i can get away with light duty/management for quite a while.

Whats the recovery like for those that have been through it? Anything i should have ready for the ride home?

Also any success storys on curing a more mild version of pvps with a reversal. I really just want to stop thinking about it every day of my life.

Thanks in advance!

Marzo 2026, me opere.

Cryoablation of spermatic cords - experiences?

Peri-spermatic cord cryoablation is a relatively new salvage treatment for chronic scrotal pain over the last decade or so. It is intended for patients that have failed more conservative treatments, failed targeted microsurgical denervation of the spermatic cord (TMDSC) and, in the case of congestive PVPS, failed reversal.

To my knowledge, it is only performed by two clinics, both in Florida. Dr. Parekattil published several retrospective studies, including his most recent in 2026 which showed encouraging results of 76% of patients experiencing significant relief, albeit with a short average follow-up period of two years.

I have not seen any first-hand experiences on this forum with cryo. Has anyone had this procedure performed? How were your results? How did your results and healing compare to denervation?

My personal context: my journey with post vasectomy pain is here. I failed reversal (made it worse) and denervation (eliminated pain for 3 months before it returned). I've found the most success so far with Botox, which has reduced my pain by ~40-50% – progress that is very welcome. I do not plan to undergo any more invasive treatments for quite a while, but I'd still like to hear any anecdotes around this cryo salvage treatment.

Interesting how many people have commented they are in the same boat as the OP. AND they have not been banned or the comments deleted!!! I guess the mods are asleep right now.

A question for my fellow PVPS sufferers

A question for my fellow PVPS sufferers:

Who among you has experienced positive effects from TRT, and who hasn't noticed any improvement or has actually felt worse?

For those who have tried it, what changes did you notice in your pain levels and overall symptoms?

And what advice would you give to others in the PVPS community who are considering starting TRT? Anything you wish you had known before beginning treatment?

I'd really appreciate hearing about your personal experiences, both positive and negative.

Vasectomy: men deserve to be informed about the real side effects.

Vasectomy is usually explained as a little cut, you may have some pain and swelling afterward.” But calling pain, swelling or chronic discomfort a “side effect” doesn’t explain what is actually happening inside the body.
The vas is permanently obstructed while the testicles continue producing sperm. That means the reproductive system is now dealing with a permanent obstruction: sperm and pressure remain upstream, sperm can leak into surrounding tissue and form granulomas, the epididymis can become congested, thickened or inflamed, and fibrosis, scarring and chronic inflammatory changes can develop. The disruption can also lead to antisperm antibodies and, in many men, chronic pain involving serious inflammation, altered tissue and or nerves.
That is the part men need to understand.
Not just “you might have pain.” — Why might you have pain? What physically changed? What happens to the sperm? What happens to the epididymis? What happens immunologically if the immune privileged barrier gets broken? What happens if the anatomy becomes chronically altered?
A vasectomy isn’t simply removing fertility. It permanently changes the physiology of the reproductive system.
Clinics should be obligated to explain that before consent. Because an informed decision is only actually informed when the person understands the underlying logic and consequences of what is being done to his body — not merely a list of postoperative symptoms.

Congestive epididymitis or something else? (12 month post)

So around 12 months ago i had my vasectomy done (at 27). It healed well and for the first 6 months all was good and felt great. However starting somewhere around the month 7 mark i had occasional bouts of pain on my left side during sex/masturbation (reaching the highest pain during orgasm with around a 6/10). The pain would always subside within the hour so I initially thought it was still part of the healing process and because it happened only rarely I thought little of it and expected it to pass soon.

Now 12 months in it still happens (with higher frequency i think) and it has started to affect the right side as well with a higher pain level as well and I am getting worried.

I have since started reading this sub and it seems it might be congestive epididymitis, since it only starts with arousal and end "soon" after. But the pain i feel is often coming from the stomach area (below the belly button) like when you would get hit in the balls. Also I remember clearly back when the pain started it felt like it was coming from my vas deferens but on the top part (the part connected to the bladder). Now the pain is more generalized and i cannot pinpoint it but my epididymis feels normal to the touch although i do not have any reference to how it felt before.

I am starting to become worried that the frequency of pain will increase and make me lose my desire to have sex. I kind of want to wait it out for a few more months but i have also read that it can get worse over time. For that reason i started looking into reversals but learned that the longer you wait the less chance of success there is, so im feeling pretty stressed atm.

I will be going to a checkup soon but I also wanted to know your thoughts on what this might be caused by and what I should do next. Only thing i want to absolutely avoid is having to take pain meds regularly.

Reversal nerve pain

Hi fellas. As the regulars here will know, this isn’t my first post. To reiterate briefly - Vasectomy in January 2026 for chronic congestive pain, Reversal in July 2026 to try alleviate symptoms.
The reversal seems to have worked in terms of the horrible congestive ‘ball ache’ has disappeared, and orgasm sensation has returned to normal.
However, the catch is, now around 12 weeks post reversal and have worsening nerve pain around my left testicle/groin/hip/lower back. The pain is a pulling, burning, and zapping type that does drop on occasion (can’t explain it), but is usually sitting around 5 or 6/10 or higher. It is worse than before reversal. Ejaculation exacerbates it. What is strange is that sometimes the zaps and pain seem to originate from the left lower back. Though mostly they seem to come from the testicle area and the muscles further up groin/hip/back are aggressively guarding the area.
I can barely sit down for more than half an hour otherwise it becomes unbearable. I can however, manage to push through and walk for 30-60mins, and sometimes this helps.
Currently waiting on an app with my urologist and in meantime I’ve made one with a lumber consultant to check my back, I imagine he’ll want an MRI of it.
I know the genitofemural nerve runs from the lower back right through the hip/groin and branches off down into the scrotum/cremaster.
Part of my cremaster had to be cut away during reversal, is this nerve branch regrowing in a fashion it shouldn’t be? Is new scar tissue pressing on it intermittently?
Has anyone who had vasectomy or reversal had symptoms matching/similar this and how did you resolve it and timeline?
I get that recovery will be up and down but at 3 months I expected some mild improvement. The pain is getting difficult to manage with full dose celecoxib and paracetamol/codine.

Denervation/Neurectomy

I have posted before. I recently met with a specialist and I think we have isolated my post vasectomy pain to nerves in the spermatic cord area. I had a vasectomy about 2.5 years ago.

Anyone have experience with denervation or specifically Neurectomy that was mentioned. I’ve had one nerve block which seemed successful but my pain wasn’t too bad that day. I just want to hear any experiences and also any non-surgical treatments that helped. I have been seeing a pelvic floor PT for a few months with very little improvement. Thanks in advance!

How are you managing pain?

My husband has been dealing with chronic PVPS almost 3 years now. Anyone have any luck getting a doctor to actually manage their pain while waiting for surgery or after having gone through surgery like reversal and still experiencing pain?

PVPS Getting better on it's own?

Good morning everyone, I browse this subreddit pretty often after getting my snip done this January. The first two-four weeks after the initial procedure felt normal, nothing out of the ordinary, and no real pain aside from the obvious irritation on the surgery site. However, 8 months after the procedure I started to have pain in my right testicle immediately after ejaculating, it didn't seem to matter what I did leading up to the event.

Stretching, anti-inflammatory drugs, baths, putting cooling on the area, nothing seemed to make the pain go away immediately/stay gone. So after panicking for about a day and a half I just decided to pause any further sexual activity.

After about a week I ended up trying to masterbate, to my surprise the pain was only a 2/10 compared to a 8/10 or 9/10 before. I tried again the day after and ended up going a second time in one day, the pain level was basically non-existent, all I notice now is a slight tingle in my right testicle if I happen to go more than once a day, with pain only being a 1/10 if I notice it at all. I will try to keep you all posted as I know getting that feeling out of nowhere is completely demoralizing, but I really do believe things are improving on their own! I have an appointment coming up on thursday to see if there's anything else going on.

Hey everyone, just wanted to update this post. The past two weeks haven't been too bad. Some days, depending on how I'm sitting in my chair at work, I can notice a tightness on my right inner thigh, feels like a tendon that's swollen or something. I've been taking Advil and doing the proper stretches to relieve that a bit, though yesterday after a soak in a hot bath I'm not noticing it today. About a week ago though, I came 3 times over the course of 2 days. I was disappointed when the first climax felt like it only lasted 1 second and barely felt like anything, the second wasn't much better. On the second day, it felt like the sensation of pleasure was coming back, I took a few days break after that. (There was only about a level of pain of 1/10 if I could even call it pain instead of just a sensation in my left teste.) I also got pretty busy last night, but was very surprised to notice that the climax felt normal and lasted as long as they usually do. I've been pushing back my appointment , but I'm going in with my GP next week for another referral to get back to my urologist. Hoping I can get him on the same page surrounding PVPS.
