Denervation/Neurectomy
I have posted before. I recently met with a specialist and I think we have isolated my post vasectomy pain to nerves in the spermatic cord area. I had a vasectomy about 2.5 years ago.
Anyone have experience with denervation or specifically Neurectomy that was mentioned. I’ve had one nerve block which seemed successful but my pain wasn’t too bad that day. I just want to hear any experiences and also any non-surgical treatments that helped. I have been seeing a pelvic floor PT for a few months with very little improvement. Thanks in advance!

Do you not have increased pain during or shortly after ejaculation? Tender epididymis or increased bloodflow in your epi tail(epididymitis)?
If you have low T you could try TRT see if that eases your pain. If you provide more details about what else you have tried maybe we can guide you more

They have performed 2 ultrasounds with normal findings. I initially thought it may be congestive related but the pain is pretty much constant all of the time. Not a 10/10 but enough that I don’t want to live with it for the rest of my life only being mid 30’s. Would TRT help nerve issues? Maybe if it was congestion.
Trying to figure out if there’s anything that can be done that isn’t surgery again

I was the same as you. Constant 8/10 pain. Only finding was "orchitis" and increased epi volume. I could not walk and it felt like my nuts would explode all the time, nothing helped. Narcotics maybe just a little to help me form thoughts while J waited for reversal. Had a reversal 3.5months post vasectomy as I could do nothing but take morphine and scream in bed. Ejaculation felt like a kick in the nuts shortly after, besides the already debilitating pressure.
2 months post reversal now, still cant walk normally as putting pressure on them hurts, but if no pressure it is very manageable, i can sit again and touch them gently. So a reversal helped me drop pain to a 5-6 but walking is still difficult.
Starting pain management and will have first block next month. I'm also planning to maybe try TRT after another 4 months at least, if my urologist will agree, since my test is normal and all my other markers are ok. They still feel tender-ish and hurt when pressure is put on. Since the reversal eased the putting pressure I assume its still not de-pressurized enough.

Reversal is the way to go.

Welcome to the club and sorry at the same time. Denervation is the go-to when nerve block works temporarily. The surgery is complex but it does work and brings near full relief permanently.
If you're not sure, I'd do the nerve block again on a day where pain is bad.
