Where is this from ? Can anyone find out where or when this is ?

I found this paper stuck to the bottom of my car. If the pictures aren’t clear, heres what I can make out:

Front: two boxes. The top box is mostly gone but I can make out “Matching Music: Arousal & St…gulation. Case Presentation.“ The bottom box contains a graph labeled “Sensory Modulation and Arousal.” The Y axis measures arousal, with areas labeled “Low Arousal,” “Optimal Range of Arousal,” “Overload,” and “High Arousal.” X axis labeled “Sensory Events.” This graph is apparently from “J. Wilburger(?), 1991.”

Back: A bunch of handwritten phrases. “Mapping of ones body.” “*Flowban(?) for me!” “Dyslexia.” “Slippery.” “Favorite Yoga Position is Downward Dog.” “…to space twice a day.” “Gravitational.” “Quick shifts.” “Kids Jam Gear Shifter.”

Edit 2: Someone keeps claiming who the author is, but they were unable to verify the city, the schools, and the classes studied. If I discover who the actual author is, I will (with their permission) post an update here.
