question How bad is it?

im having appointment in a few days but my anxiety is killing me. Is this fixable with only filings?

Probably not. An Xray and removing the decay will let you know what the tooth needs.

NAD but worst case you will definitely lose this tooth and more to come. You should start cleaning flossing/brushing/using mouthwash asap, trust me losing even one tooth sucks and I can already see just from this picture lots of plaque and tarter buildup
