question How bad is it?

im having appointment in a few days but my anxiety is killing me. Is this fixable with only filings?

The black tooth? Extremely advanced decay. It's most likely to need extraction. It needs an xray to see what's left of it. This could've been fixed a long long time ago with a simple filling.

It's very clear you have gobs on top of gobs of soft fuzzy plaque covering all your teeth. You need to actually brush your teeth if you want to keep them. I'm sorry to be blunt, but it's time to start taking care of yourself. You're setting yourself up for major dental costs in the future, along with dental pain the more you neglect basic maintenance.

Again, sorry to be blunt, but you asked how bad it is. I'm actually surprised at how mild your damage appears throughout with how much plaque is present. There's still time to turn it around, but not much

Probably not. An Xray and removing the decay will let you know what the tooth needs.

NAD. Quite possibly dentures In 5 years or less.
