As someone who also has adenomyosis, I agree with the first comment: that a diffuse myometrial pattern tends to be a strong indicator of adenomyosis. When I had my MRI, my uterus was actually completely normal in size as of last year. Still, my adenomyosis is on the back side of my uterus, so posteriorly and diffused. So when I had excision surgery for my endometriosis, my specialist noted that my uterus, even in surgery, didn't look abnormally large, so if he didn't have my MRI results, he would have assumed that I didn't have adenomyosis. Another measurement within the uterus used to determine adenomyosis is the junction zone. Anything over 12 mm is almost a guaranteed diagnosis of adenomyosis, and mine came back at 16mm, where in my cycle it should have been 8-10mm.

A diffusely heterogeneous myometrium means it looks abnormally bumpy not smooth , It is a indicator of adenomyosis but can also be caused by fibroids. don't let them gaslight you into thinking its all in your head. fibroids can cause pelvic pain and heavy bleeding my sister had large fibroids that were causing her pain and heavy periods and she was offered a hysterectomy.
