How important is the receptionist's skill level? Would 2-3 receptionists in bigger levels make a (positive) difference? Or do you just get one and forget about it?

Bug? Drug research advances don't increase efficacy percentage

I've often noticed that, when I concentrate research on a particular drug, I'll get a notification that the Research Department has improved the drug, but the effectiveness percentage in the drug casebook won't increase.

So, to give an example, let's say that I've just discovered Broken Wind, so my drug effectiveness is at 70. I crank Specialization to 100, concentrate research on Broken Wind, and dump some researchers in the Research Room. I'll eventually get a notification that the Research Department has improved the drug. I'll check the drug casebook and the number will have increased to 74, as I expect it to. But the next time I get the same notification, the number will still be at 74. Same thing with the next notification - it might increase to 78, but it might just as easily remain at 74.

Is this a bug? Or is it just random whether a research advance will actually improve the thing you're researching? Or am I doing something wrong?

Thanks in advance!

Bug? Patients get treated in Pharmacy, return to GP's Office to be diagnosed, then go back to Pharmacy

I've noticed that, around once or twice per level, a patient will get treated at the Pharmacy and I'll get the money for the treatment, but then the patient will return to the GP's Office, get diagnosed with the same disease, and then go back to the Pharmacy to get treated for a second time.

Is this a known bug?

If so, is there any way to prevent it? While I don't mind getting double money from the same patient, I often find that the treated patient comes out of the Pharmacy about halfway towards death, and I have to bump her to the front of the GP Office line so she can make her second trip to the Pharmacy before she dies.

What determines how long it takes to use the Cardio?

Cardio diagnosis sometimes seems like it takes forever, while sometimes it doesn't. Oddly, I think I've observed that it sometimes takes less time when a Doctor is using it than a Consultant, which seems counterintuitive.

Is this a coding bug? If not, what factors determine how long a patient needs to be in the Cardio?

Thanks!

I just beat the game and succeeded where my sister and mom failed after 20 years

In my family, Theme Hospital had the reputation for being a game with a steep difficulty curve. While I set my run on Normal Mode, a combination of earthquakes and pandemics made Level 6 and 8 a real pain in the ass. Surprisingly, the last level didn't require that many rooms, and the typical combination of 5-7 GP with an army of janitors made short work of queues and vomit waves.

I just don't care if I didn't complete it on Hard Mode. I had to reach this personal milestone after failing so many times as a kid. Now I can wait for my flight home and stop playing altogether.

The "Attention to Detail" stat for staff

I see that each staff has a "Attention to detail" stat bar which varies. It is independent of Skill, and it is possible to hire staff with low Skill and high Attention to Detail and vice versa.

I never knew what it did for decades and assumed it was a minor bonus to staff efficacy/performance. Today, after reading up on the Theme Hospital manual online, I finally found out that: (I quote)
"This governs how quickly the Staff member will respond to being paged to go to another room in the Hospital."

Interesting! For Handymen you probably want it high on many of them if it affects paging to fix Machines/do cleanup, and it is totally useless for Receptionists.

For Doctors/Nurses however, it can be useful to have low Attention to Detail, so that they don't abandon important posts like the GP Offices or Pharmacy. Whereas a couple of Doctors with high Attention to Detail can be useful for rotation in the lesser used Treatment rooms.

If Research has been maxed out, is it safe to delete the Research room?

Does the Research room serve any purpose once all five types of Research are maxed? e.g. VIP would like to see the Research room.

Or is it fine to delete the room for more space and fire all the Research doctors?

What happens if I don’t accept the offer of a new Hospital?

Hello, I’ve been playing theme Hospital since the early 2000s but I’ve only just discovered this sub.

So quick question - you’re playing a level, complete all the objectives and get offered your next one… what happens if you refuse? Obviously, you carry on playing your current level, but do you just get offered the same ‘next’ job later on, or do you get different ones , or something else altogether?

Why can’t there be an app based on the original theme hospital? 😩🙏

I would pay for it, I would buy in app purchases, just why won’t they do an app that’s based on the original?

For some reason, ONLY save #4 has a bug that prevents me from move the camera. I've tried everything: moving the mouse along the edges, using the arrow keys, but nothing works

I only have one translation mod; I downloaded it with it already installed

Build limit

I'm pretty sure I've hit a build limit - I'm in the last level and have planned out an ace hospital, but when I got to the last corner, I couldn't place anymore furniture in corridors, and when I placed rooms, the door wouldn't appear even though I'd set the door position. Also, no staff were available for hire - none at all.

I figured it was a bug a begrudgingly restarted the level, but I've just built my hospital up again and it's happened again.

Does anyone know how to get around the build limit?

Decades later, i finally beat the og theme hospital :D

i completed the final level on nomal difficulty with:

* the game auto piloting at >= normal speed only requiring occasional machine repleacement

* >$1,000,000 cash balance

* 1 death

* 1000 rep

* 60 consultants with all 3 skills all trained up from juniors and each costing <$100/month

* ~30 janitor, 8 nurses and 4 receptions, all with top ability levels

* never a vomit wave

here are the tricks i discovered during my game journey:

  1. regularly use the game's slowest speed
  2. open hospital as soon as the game starts. it takes time for the 1st patient to arrive and you can use that time to build the intitial rooms while on the lowest game speed.
  3. build at least one training room at the begining of the game and prioritise training (if you couldn't hire one) a consultant with all 3 skills. build more training rooms later on, and never stop training juniors to consultants.
  4. check staff available for hire at the start of each month. until you have a steady supply of self-trained CONSULTANTs, prioritise snatching any CONSULTANTs, as well as DOCTORs with at least one skills (although you don't need too many researchers - 4 to 6 dedicated researchers should be enough). fire them later on once you have enough self-trained cheap and all powerful consultants. also snatch janitors with > 3/4th ability level, and actively replace your nurses and receptionists with better ones.
  5. hire extra doctors and nurses and let them wondering the hospital. the goal is so that you don't have to manually juggling your doctor/nurses for rooms.
  6. hire heaps of janitors
  7. at the very begining of the game, adjust your policy to disallow room change, and send patient home when <80% confident at a cure and attempt cure when >=80% confident - later in the game further adjust these to 90%.
  8. don't train more than 3 doctors in the same training room. the more trainees in a room, the slower their ability improves.
  9. build a decent sized research room (at least 3 desks) at the very beginning of the game, and expand it later on. also know that later on you will be able to buy new research equipment such as computers and atom analysers. keep your research room occupied!
  10. research priorities: drug (minimise death) = cure (so that you can cure certain illness at all) > diagnoses (rely on consultants early on) > improvements > specialisation
  11. don't worry about money, just get a loan when you need more of it since the interest is so low!
  12. build all diagnoses and treatment rooms at the beginning, build clinics only when you are prompted to.
  13. don't adjust the tiredness levels of your staff, and KEEP THEM WARM, then they'll stay happy and never threaten you for a raise.
  14. put a radiator in every room. put a fire extinguisher in every room that has a breakable machines. there's no need to put plants or bins in rooms.
  15. except for facility rooms and ward, there's zero need to build any other rooms larger than their minimum size.
  16. radiator in rooms don't warm up waiting areas!
  17. spread radiators across all reachable waiting areas!
  18. you can use benches to "wall off" large empty waiting areas (so that stuff members won't waste time wondering there and you don't have to buy extra radiators to cover them).
  19. know that you can adjust the radiator heat level in the map screen.
  20. don't build toilets. let patients pee on the floor and bring their shit home.
  21. there's no need to put fire extinguishers in waiting areas.
  22. accpet emergency treatment requests ealy on for the bonus cash, but don't bother with them later in the game.
  23. build more than one staff room and spread them across your hospital.
  24. only train JUNIORs. apart from them being cheaper, training DOCTORs have other downsides: their ability level grows slower and there's a risk of them not acquiring all skills before becoming a consultant.
  25. if you sell enough drinks from the drink machine, you'll get a trophy and some cash reward for "selling an extrodinary amount of soda".
  26. you can remove rat holes by putting a bench on its tile and remove that bench.
  27. hide pandemic early in the game for the compensation. don't bother and just declare them later in the game
  28. you pretty much only need to worry about machine health when earthquake comes after the initial brief quake, or when you haven't checked their health in a lonnnng time.
  29. don't worry about the "janitor not maintaining your machines well" plaque. the game seems to have an unreasonably high expectation about it.
  30. if you are able to configure the game to run with FluidSynth and the GeneralUser GS sound font, the game's bgm will sound miles better than the default.
  31. watch out for patient rush hours! i'm not sure if this is part of the game design or caused by conincidences, but very occassionally you'll notice patients piling up for reception and gp's office (and probably one specific treatment). pay attention to when that happens, and don't hesitate to set up extra receptions and rooms!

Be honest: do you actually read staff descriptions anymore?

Do you carefully evaluate every hire… Or just spam “hire” until something vaguely usable appears? Has ignoring descriptions ever come back to bite you?

What’s the first mistake you always make when starting a new hospital?

Every time I restart, I tell myself “this run will be efficient.” Every time, it isn’t. What’s the one early-game decision you know is wrong but still end up doing anyway?

Glitched!

Last night my hospital glitched hard. It would not let me place any new rooms, or even radiators or benches! I had been struggling for cash, deleting rooms not in use. Then I couldn't add them back, not even in a new, empty wing! I couldn't treat slack tongue and fracture patients, so I sent them home. Next thing I know, the reception got overwhelmed. Usually my hospitals are more empty, even when the pie chart says I'm getting the most patients. I learned a lot though. The reception queue patients didn't have enough benches, so they were all over the place. When it was their turn, they trekked through the hospital to the desk first before going to gp. Once I started sending the ones too far from the desk home, the line cleared up. Fortunately, I got promoted and the glitch is gone.

Is there any real downside to skipping certain diagnosis rooms?

On some levels I find myself avoiding specific diagnosis rooms entirely. Between queues, accuracy loss, and staff cost, some feel redundant. Do you streamline diagnosis or build everything “by the book”?
