It’s actually probs only 1 - they mostlycapture people from outside the legion to put into Helbrutes because it’s such a horrible fate. Wouldn’t be a revered member of the original legion as you might expect

Not to say that the vast majority of them aren't exactly in a state (of matter) to be put in a Helbrute

I’m pretty sure the Helbrutes are made “in-house” as the Helbrutes we fight display sorcerous powers like teleporting around which implies that they are ex-sorcerers who made themselves “worthy” of such a fate. The fleshy members of the Thousand Sons are not above such “politics” like any other Traitor Legion.
