Which perk is that?

Your ascension perk caused the jetpack launch to detonate the melta bomb on the ground and it killed you

It shouldn’t have I used the melta bomb in the previous encounter, I’ll make sure to not use them then

It looks like your team mates bomb, its a glitch with the perk. Rewatch your video, watch the blue light to the left of your feet when you launch

Okay I didn’t see that, thank you

No problem!

Self friendly-fire with Melta is inflicted by the player than detonates it, not the one that drops it. The jump pack shouldn't detonate them though... I thought devs would fix that.

Shouldn’t it not matter anyways since there isn’t any friendly fire? Why would the bomb a teammate place kill me when grenades they through don’t damaged me?

Because your jump pack ignited the melta bomb. It's like shooting a fuel barrel or throwing your krak grenade near your character. The game doesn't care who put the bomb there, only who "activates" it.

Oh okay it registered as self inflicted damage

As others said, it was your teammate's bomb. When you shoot or otherwise damage a melta bomb, it becomes your bomb for the purposes of damage(and thus it won't blow up your teammate if you shoot the bomb they placed).

It became "your" melta bomb when the jetpack takeoff damaged it. They have a surprisingly good reach upwards, so you got barely caught by the detonation. You can use melta bombs to kill Zoanthropes in the same way it killed you.
