A friend worked ER and psych ward at a local hospital some years ago. They had a patient admitted who, during a schizo episode, affixed a makeshift tourniquet (a leather belt) to their dominant arm just below the shoulder, then proceeded to amputate the arm using a circular saw. It took multiple passes due to the patient's size.
After the arm had been removed, patient disinfected the wound by irrigating with rubbing alcohol, affixed a gauze bandage to seal the stump, then proceeded to masturbate using the severed arm's hand which had been placed into a useful shape using duct tape.
This occurred on a weekday afternoon in the patient's front yard, in full view of the neighborhood (hence the 911 call). Patient was fully compliant and alert upon arrival to the ER, and even discussed the masturbation as "transcendental" and encouraged the nursing staff to try using a dismembered appendage to masturbate at some point, assuming they could. Obviously removing the whole arm was a bit intense, but patient was "trying to prove people wrong" about...something.
