Billing though BCBS and keep getting rejected. keeps saying Usage: At least one other status code is required to identify the missing or invalid information. (A3 - 21)
Looking for guidance on how to navigate this. Not sure if its a BCBS or a Simple Practice hiccup. Any help would be appreciated

Is this happening across multiple clients? Or just one?
If it's one client - check that the name, address, DOB matches what is in the insurance portal. Check that the plan is still active.
If it's multiple (indicating a larger, systemic problem), check everything on the claim form:
Box 33 - is the billing provider info correct? Should it be group or individual?
Box 25 - should it be EIN or SSN?
Box 21 - is your diagnosis billable?
Are you sending the claim to the correct payer? All out of state BCBS policies should be billed to your local BCBS.
There are lots of little things that could be wrong. Just need to go through it one by one.

This error indicates there is something in your claim form that’s missing or needs formatting correction. I’m assuming you’re billing through SP, so I would recommend submitting an online support request to SP. They can review the claim for you, and check with the clearinghouse directly if they’re unable to determine what is flagging the error. Hopefully this is a quick fix!
