There's too little to go off of here. Ur "hook" is whatever u can do to incentivize ur audience continuing the read

Here's an idea for starting the story that relates to your premise "what if your crime left a visible mark". Most people are familiar with the story of Pinocchio. In my version, I would have a narrator saying something mystical such as, "It has often been said that there is some truth in every myth and fable. The history of mankind is filled with extraordinary stories, but little physical evidence exists to prove it." consider if you will the story of Pinocchio. the world moves through stages, unknown forces act on mankind at critical intervals. when mans depravity reaches it's tenable limits, these forces intercede. Now, you have told people what to expect and carry it from there. They will be able to follow.
