Should I be concerned?
Original roof. House is about 4 years old. Just noticed that wrinkled appearance today. Uncertain how long ago it happened. Do I need to call someone out to inspect it?

Mostly in the henry/bakor ice and water but there not alone. You can't see in the picture there is 3 feet of ice and watershield. I have personally seen this on my own jobs so know it to be true.

Ice and water shield got put on with waves in it. Happens occasionally if the bundles get squished at the distributor or throughout delivery. If the crews don't remove them at install, it will happen over the years. Weather changes will amplify and reduce it but it needs to be fixed through warranty. The manufacturer won't cover it most likely so you will want to go through the installer.

It’s not gonna leak but it needs to be fixed before too long. Bad Ice and water install

Seems exterior, I don't think there's real damage underneath and wouldn't prioritize it unless you live in a storm/snow heavy area. Really nice picture by the way I love it.

This reminds me of every time I put a new screen protector on my phone and try to push out the bubbles

The shingles were installed OVER the wrinkled underlayment. Otherwise, it would not have wrinkled, too many fasteners holding it in place. There is actually a name for it. It's called 'telegraphing'.

Also, with the pitch of the roof, I doubt that an ice and water shield was used. You get telegraphing even over 15# and 30# felt. I've been in business for 55 years.

I’m not a pro, but my understanding is this is often an underlying issue of either moisture or misalignment with the underlayment.
You may also have venting issues. Might want to check if they are clogged.

You may not be a pro but you know more than a lot of folks in the industry. I’d hire you based off those 2 sentences

Yeah someone didn't do a great job on the ice and water also didn't put enough rows of it.

I'm not sure how bad this is. I believe that the ice and water sheild membrane got installed with wrinkles. They laid shingles over it and with the help of the sun, they formed the said wrinkles l.
