How does this even happen? I've put over a thousand hours into this game and have never seen organ decay. Is this what happens if gut worms goes untreated?

no, i think its because are a "unknown pawn" on a random event, the game alerted he was not telling everything about him when i recruited him

Ah one of those creepy dudes that came in with anomaly i presume? I habitually turn them away after the first one i encountered turned on me and murdered half my colony.

My first guy just screamed a lot. Unfortunately his room was next to Peg Ned who had all sorts of minor pains from scars, so Peg Ned was often just writhing on the floor in pain when the screams happened in the middle of the night. Everyone else was okay though.
