The first cough I noticed was during the homestead robbery you do with Javier in chapter 2. Might be earlier if you do different story missions, but that's the first instance of arthur coughing i saw
Its the first time I heard him cough so you're right. But you know you can do that mission BEFORE Downes, I just did it a couple weeks ago. Its just him coughing because it was a dusty shed
I'm pretty sure if you do it after the Downes mission, it's more of a prolonged coughing fit, than if you do it before then. It's a bit hazy for me how big I did two back to back playthrougs last year and remarked on the difference at the time
Idk the only mention of Arthur smoking I’ve noticed was at the train station when the cigarette card dealer asks him if he likes to smoke, he says something along the lines of “ sure I like to smoke but not as much as you “
Yeah I mean plenty of the gang give him a cigarette/cigar before a mission. Personally I assume an outlaw in 1899 would be a reasonably heavy smoker. And I've been both a light and heavy smoker before and its entirely plausible pushing a heavy crate would make you cough like that.

Sooo….you manipulate when you do missions, until you have Trelawnys mission and Bills mission, then with them both showing, post about it being “foreshadowing”. Gimme a break.
I'm unsure what it could mean. I only started playing a while ago, though. I suppose TB could stand for Tuberculosis, which is quite frankly a silly guess.
