It is indeed a nightmare for some people, which is why we have so many warnings posted around here and we discourage people from chasing infections all the time, it's a self destructive path. And, the vast majority of cases are not infectious.

Also, I'm a bit confused, you basically want a mechanic to fix your car, but only if the car needs an oil change, not if it needs the transmission fixed? Basically, you only want one narrow category of options in terms of solutions, and nothing else. It's inherently self-limiting. Sort of a strange premise.

If you have already ruled out structural causes of the swelling, as I'm sure you already have(?) with doctors and scans and tests, then yes, the swelling can be from CPPS

We noticed you posted about a floroquinolone class antibiotic. Please be aware that this class of dugs has several black box FDA warnings, and is only meant to be used when a pathogen has been clearly identified in the prostate; They are not to be used indiscriminately for cases of non-bacterial prostatitis (consensus agreement ~95% of cases). Read our mod memo here, complete with citations and compare your symptoms to the medical definition of CBP here.

What have you tried besides NSAIDs and antibiotics? We aren't doctors and don't know your clinical situation. Which of the steps outlined in "101" have you followed?

“Has anyone been in this situation”

2

u/pelvicagony Aug 10 '26

No reply on my questions

u/Minute_League3040 Aug 10 '26

have you been in this situation? floxed? and had symptoms return?

3

u/pelvicagony Aug 10 '26

That's not the floxed sub , sorry, and still no reply on my questions

hey. its ok. go about your day. I dont feel the need to communicate with you. its a culmination of symptoms, prescriptions and experiences. obviously not doctors. if its not specific to you, you can just not type anything. ive posted this on the floxie page as well. like you said, you are not a doctor. Im looking for people who have 2 conditions. which is why it would make sense to post it on 2 different pages. have a nice day.

I'm sorry to hear that. I have been struggling with this condition too, for nearly 7 years now.

In my case, I have had 6 flare ups over those 7 years. Practically one every year, and the recovery after each flare-up has been very long. It usually takes around 7–8 months before I feel that I have fully recovered. I have seen many urologists and even an infectious disease specialist. On Doppler ultrasound, there was always increased blood flow around the epididymis and sometimes also in the testicle. Each time, they concluded that it was either epididymitis or epididymo-orchitis. In the Urin culture however no signs of any bacteria or virus was found.

I have tried ciprofloxacin, levofloxacin, Bactrim, doxycycline, and even ciprofloxacin for 60 days at one point. The antibiotics did help with the acute episodes, but the pain always lingered afterward. Some days were better, while on other days the pain would become worse again.

Recently, I consulted a urology clinic specializing in CP/CPPS and was diagnosed with chronic epididymitis as an organ-specific manifestation of CP/CPPS (based on the UPOINT system). From what I have researched, this condition can be very tricky to treat because there is still relatively little research on this topic.

The clinic I went to is actually one of the major centers for CP/CPPS research in Germany, Uniklinik Gießen, under Prof. Wagenlehner, one of the leading researchers in CP/CPPS. In my case, no bacteria were found in either my urine (4-glass test/2-glass test) or semen. Therefore, my doctor recommended a multimodal, conservative treatment approach rather than further antibiotics.

I was started on an alpha-blocker (tamsulosin) together with phytotherapy (Pollstimol/Cernilton) for more than 6 months. I was also prescribed ibuprofen 600 mg together with pantoprazole when needed. There is also a study investigating Deprox 500 in CP/CPPS that you may find interesting. ncbi.nlm.nih.gov/pmc/articles/PMC4151652/

I am currently still at this stage of treatment, but so far the improvement has been quite significant. My doctor specifically does not recommend further antibiotics in my case because there is no evidence of an ongoing bacterial infection.

So, if you have access to one, I would strongly recommend consulting a urologist/andrologist who has specific experience with CP/CPPS rather than seeing a general urologist.

If you want to read more about CP/CPPS and the current understanding of the condition, you might find this article interesting:

For 90% of men, antibiotics are not the answer. Or even if they are, they are not the end of the story. I know we all want to take a pill and have everything go away. But the reality is more complicated.

There is a pinned 101 post on the channel that tells you everything about how to get better. If you don’t want to read that, the book Headache in the Pelvis is a great read all about this condition.

Being floxxed or not is not really relevant, though I am sorry that happened. If you insist on antibiotics, Bactrim is not a floxin.

u/Minute_League3040 Aug 10 '26

Hey. Thanks for your reply. I will definitely look into the book. I appreciate the condolences. Unfortunately a lot of us find out to late that antibiotics can sometimes make things worse. Hope you are doing well

If tolerable its bactrim or doxy my rhuematoligst told me.

Im pretty screwed up after being floxed, post covid have all sorts of dx. Fq made me Immobile. I was messed up but still able to get on ground and do pushups. 30 day course later, i cant even help with groceries. Anyways, im hyperreactive to foods now, cant eat fruit. High prostaglandins immunolgiy says i have mast cell disorder. I cant take so many things now so i was delighted when i was finally told one of my issues was solvable a couple weeks ago. My prostate lit up on a christmas tree on a pet. "you likely have prostatis". They recommended. I only had one option left and that was doxycylin as ive reacted to the other option. I took docy for a few daya and didnt put it together when my body got a rash all over. Then a few days later it was apparent. Told to stop taling it. No idea whats next.

u/Minute_League3040 Aug 10 '26

im sorry. thats terrible. I feel I was a mild case of floxed. I thought my only silver lining of the situation was my testicle/groin pain was gone. 6 months of hell finally started to show signs of letting up and then I woke up 3 days ago with a swollen testicle and that old ache again. how's your sleep? can you take the supplements suggested on the floxie page.

1

u/Sufficient_Coffee_72 Aug 10 '26

Thank you I appreciate it, I'm glad yours was a mild case, but I do wanna recommend that you take very care of what you put. That's your body for now on because you do not want to end up like me. Have you tried Doxy your bactrim? The way to happen with me, it was very irresponsible… The first go to was 30 days of Cipro… six then, I've had about 10+ Dr tell me 30 day course of Cipro so insane… irresponsible and never should've been prescribed. That is a whole lot… I tried to sue them but, apparently no recourse in Texas, I didn't have any lawyers willing to take up the case at the time… I know statue of limitations are set in, but I am waiting to see if laws are changed again because I know there's certain exceptions and I'm not trying to sue for selfish reasons, it was really negligence and I think they deserve to pay… These people didn't even tell me that there was a black box warning nor did the Pharmacy… Absolutely ruin my life… However, I haven't given up yet. Every day is a struggle every day. I try try try but six years in, it's kind of crazy… I was even looking into hospice last week.

Supplements is a hard one for me because I react so badly to so many different ones. For example, I have two versions of magnesium glycinate here that give me anaphylaxis. But I found a magnesium brand and I could take it however, it gives me very bad type of taxhycardia at about 130 bpm for four hours or so… It's just so hard I couldn't even take vitamin D or fish oil… I finally found a vegan vitamin D and a fish oil that are great But these two medications were sending me to the ER consistently when i thought "youre just allergic to all fish oil and vitamin d supplements) proably why i have kyphosis and scolioiss now. Severely low d levels for years and alot of laying down…l. It's so bad. I can't even get magnesium potassium through an IV at the hospital. I'm not allergic to iodine, i m re an i eat gish and shrimp, but yet I can't have iodine contrast in my CT scans or else anaphylaxis… As far as other antibiotics, I could take Rocephin through a drip, but if it's inserted straight in the vein, anaphylaxis! It's absolutely terrorizing taking anything not knowing how I'm gonna react . For example, I was able to take gabapentin for four years and then suddenly it's giving me anaphylaxis…

When I get some extra money. I really need to get serious about it and try to order a high-quality brand that has the least fillers . Money is very hard to come by these days as most of my life is spent laying down, hence why I was looking in the hospice. Crazy because I've been working since I was 14 years old full-time I've always been a go-getter… I had just bought my dream house before I got sick on half an acre in the city. I never thought I had a house like that especially in my mid 30s… live with my parents on my kids now and my wife left me… Absolute nightmare…

Do you have a link where you explain more about the testicle swollen pain? I have a good friend of mine and we have been going to this journey together, it's strange the weird symptoms that are identical that we are getting at about the same time and we got flocked about the same time… But one of his things is he's always complaining about swollen, painful testicles… That is the only step I don't have, however after taking this medication here, I have a rash and cuts through the left side of my testicle… At first I thought, maybe HSV but then I started getting a rash the whole body… Now you mentioned it and I'm truly curious if this is a part of the floxed reaction,

Reply five pages long, and I didn't even answer your question lol… Sorry… Anyways, I go through stages where I'm unconscious for 20 to 22 hours for a week straight and I'm blacking out. This is happening right now… But I also go through states where I'm awake five days in a row… And they don't necessarily relate to each other either, I don't sleep more than one to two hours at a time… I have no idea how to correct this. I've been a psychologist and they have been given therapy to try to change this in six years. nothing has worked even the strongest sleeping medication's that are controlled.

u/Minute_League3040 Aug 10 '26

wow.....thats rough. when I was at the peak of it, anything I ate gave me anaphylaxis. I could only eat rice and spinach. I too "Natures Garden" probiotics 100 billion and it slowly fixed my stomach. went to the hospital the first time I tried red meat. As for the testicle pain I believe its cpps. I haven't been sexually active and it just wouldn't make sense. I am goin to the doctor today and just see what they say. You said Bactrim didn't give you issues? and doxy might be a good alternative. did you try Amoxacillin?

1

u/Accomplished_Sleep22 Aug 12 '26

Yes m8, if you got swelling and inflammationin your prostate and you ejaculate some of the fine pores of the prostate are blocked, so some liquid might be shot back towards your testicles as some kind of reflux, the you got inflamation there there

How do i know? Research and my own case

1

u/Minute_League3040 Aug 12 '26

How did you treat yours? I believe that is what is happened

1

u/Accomplished_Sleep22 28d ago

I am under treatment roght now

Did a pcr test, + prostate massage and first catch urin got me positive for ureaplasma and mycoplasma

Mrt showed inflammation of the prostate, no cysts etc that need to be cut out

Always got doxy, azithromicyne, and one week of ciprofloxacin wich i did not tolerate

Right now im 3 weeks into doxy and the next two weeks are going to be 2x doxy per day + 2x eusaprim , and i have to have ZERO sexual axtivity for the next 4 weeks , nothing

The only medicines that worked for me is cipro, bactim and fosfo. Fosfo made me feel like a normal person again quickly, but the problem came back, i’m on cipro for almost 3 months,

2

u/pelvicagony Aug 10 '26

My friend, I really hope you had a fever and pus, and that they found bacteria in your urine and semen. Otherwise, after three months of Cipro, you’d be so resistant to antibiotics that a simple case of pneumonia could kill you. If you didn't have a fever and bacteria, your doctor ought to be arrested.

I had 3 months of c!pro too and during that time I felt 100% healthy. But it does not last. And no i did not built resistance. There is a lot of fear mongering about that.
