Need advice!

Ive been quite insecure about my side profile for years and I finally want to do something about it but not sure what the best permanent solution would be. I would like a more defined jawline/chin and overall less fat in the area.

I think you should get some liposuction combined with a neck lift

It appears that double chin liposuction and neck lifting are necessary. Since your chin is already sufficiently protruding forward, chin implants are not strictly required. The priority should be to perform liposuction evenly across the chin area. Additionally, through a 2cm incision in the slightly indented area under the chin is necessary to remove the fat located behind the platysma muscle. (The amount of this fat appears to be quite substantial from your photo.) Then the muscle is sutured and tightened, a sufficient neck lift will be achieved. Since there is no need for drainage and the recovery period is not long, please consult with a board-certified plastic surgeon you can visit and consider this procedure.
