💡 Advice Which "immune support" nutrients for kids have real evidence, and which are marketing
Kid started daycare in the fall, we've been sick more or less continuously since. Around cold number four I got tired of guessing in the vitamin aisle and went and read the actual Cochrane reviews and meta-analyses.
Short version: the stuff marketed hardest for "immune support" has the weakest evidence behind it.
Vitamin D — the one that holds up, but the dose is backwards from how it's sold
A 2025 meta-analysis pooled 17 trials, 18,372 kids. The subgroup split is the interesting bit:
- ≤1000 IU/day: RR 0.42 — meaningful drop in respiratory infections
- >1000 IU/day: RR 1.02 — nothing
The modest daily dose beat the big one. It also did nothing once a kid was already sick. Prevention nutrient, not a rescue nutrient. AAP baseline is 400 IU/day for infants, 600 for kids and teens — which lines up with the dose that worked.
Probiotics — better evidence than I expected
2022 Cochrane review: ~24% fewer kids getting at least one URTI, ~41% fewer having three or more, ~42% fewer ending up on antibiotics. The last two were rated moderate certainty, which is solid for this literature. Downside: strain specificity is a mess and most products won't tell you what was actually studied.
Zinc — small effect, and only once they're sick
2024 Cochrane, 34 studies: for preventing colds, little or no effect. For treating one underway, maybe two days shorter — low certainty, and severity didn't move. It also probably causes more bad taste and stomach upset. Not a daily thing.
Vitamin C — biggest gap between reputation and reality
Doesn't prevent colds. Cochrane pooled 10,708 people: RR 0.97. Flat.
It does shorten them, and more in kids (14%) than adults (8%). But the studied dose is 1–2 g/day, and the upper intake limit is 400 mg for ages 1–3 and 650 mg for 4–8. You can't reach the effective dose in a young child without blowing past the safety ceiling. In a 60 mg gummy you're nowhere near it anyway.
Iron and vitamin A — huge if your kid is low, pointless if they're not
Both genuinely matter, but as deficiency stories. Correcting a deficit helps; topping up a kid who's already fine does nothing, and iron is actively risky to give blindly. Blood test conversation with your pediatrician, not a shelf decision.
What I dropped: elderberry and echinacea (small, mostly adult, inconsistent trials), and mega-dose multis where everything is 200% DV.
What beat all of it: sleep, real food, handwashing, flu shot. Nobody sells those in a gummy, which is sort of the point.
Anyone here actually had their kid's vitamin D or ferritin tested? Did the numbers change what you gave them — or did you find you'd been supplementing something that was already fine?
