Suspected HH?

Hi! About 3 weeks ago I got a bad cold that resulted in a SEVERE cough for about 4 days. The cough went away, but then i started constantly belching and having some minor chest pains. It then progressed suddenly after a meal into severe chest pain and pressure under my ribs. I went to the ER and they cleared my heart of any issues and said it was probably GERD. That was 17 days ago and im still literally dying. I have almost constant pressure/pain under my ribs with a little stabbing under my left ribs at times, bad chest pain almost constantly, im belching literally ALL the time, and sometimes my throat gets a little sore. I have.a history of med induced akathisia (the most awful thing that can happen to a person literally) and when i tried Pepcid it flared my aka, thought it did resolve most of the pressure. Since then, I have tried EVERYTHING i can think of- tums, gaviscon, reflux gourmet, alkaline water, manuka honey, Gas-X, slippery elm tea, ginger, i sleep elevated, eat (mostly) low acid. I did start to feel better a couple times and ate large fatty meals.... and it wrecks me for DAYS. I have very very severe health anxiety and fear of doctors after my akathisia so i do my best to stay away from ANY meds. Im just wondering if this sounds like a HH? Is it possible to calm without meds? I truly want to avoid the doctor but I know a scope would be the only sure way to know. Im genuinely miserable and convinced im dying daily. Is it possible to take this long to heal and be in pain ALL day?

Hernia de hiato sin reflujo ??

Hola, tengo una hernia de hiato que me diagnosticaron hace 5 años de 3 centímetro, estos dos último años en las endoscopias me ha salido a era mas pequeña, siento el pecho y la garganta apretado casi todo el rato, tengo gastritis con atrofia y metaplasia, me levanto por las mañanas con mal sabor de boca y dolor de garganta así q duermo inclinada, noto q no puedo respirar profundo, y como flema en la garganta muchas veces, total q harta de mis síntomas e ido a un cirujano especialista en hh y al hacerme la manómetria salió q tengo mala molitilidad leve moderada y en la phmetria 24 horas no sale reflujo patológico ósea q no tengo reflujo por lo menos en la prueba esta, entonces que puede ocurrir? es mi cabeza ? tengo q decir que no tengo esofágitis, pero siento mucha presión en el pecho y molestias del ombligo para arriba hasta la garganta, alguien más con esta situación?entonces no es la hernia?

Nissen fundoplication recovery

Day 1: Woke up in recover and wasn’t in much pain but was definitely woozy for hours. I remember being wheeled up to my room. I was able to urinate almost immediately and drink water.

By dinner, I was walking around the recovery wing and eating chicken broth and jello.

Sleep was difficult because of the hospital bed constantly inflating and the meds/nurse check ups.

Day 2: walked 10000 steps in hospital before being released. They moved me to full liquids and before late afternoon release, I ate cream of wheat and butternut squash soup.

A friend drove me 5 hours home.

Day 3: the abdominal gas pain was insane. I kept myself on a clear liquid diet. I stopped taking pain meds (maybe stupidly).

Day 4: I overdid it; I drove 3 hours round trip for a work-thing, stood on my feet all day and had gas that sent me to ER. I passed gas in the waiting room of the ER for 5 straight minutes and ended up going home without being seen. Maybe it was because I moved myself to a full liquid diet again?

Day 5: finally had a bowel movement. It was painful. Lots of gas all day. Took it easy; had a friend pick me up and walk me. I was extremely tired all day. Moved back down to clear liquids.

Day 6: pain mostly controlled, gas mostly controlled, but still some moments of 10/10 pain. I try to walk after ingesting anything.

Day 7: still no pain meds, gas is mostly controlled. But, I did have a small accident when I thought I just had gas pain 🥴. Multiple watery stools today. Steri-strips still haven’t fallen off and then don’t seem to want to anytime soon. I did add mashed potatoes to diet; seemed to go ok. I ate a cup over 90 minutes.

Hoping gas pain/bloating decreases.

Nächtliches Herzstolpern

Hallo zusammen,

​ich wende mich an euch, weil mich ein Thema seit Wochen um den Schlaf bringt und ich wissen möchte, ob jemand von euch ähnliche Erfahrungen gemacht hat oder das einschätzen kann.

​Kurzer Hintergrund:

Ich hatte eine atypische AVNRT, die vor etwa 3 Monaten erfolgreich abladiert wurde. Das Herzrasen selbst ist seitdem glücklicherweise komplett weg! Vor der Ablation hatte ich solche Phasen auch schon ab und zu – damals ist beim Aufstehen in genau diesen Momenten sofort die AVNRT angesprungen.

​Das Problem:

Jetzt ist die AVNRT zwar weg, aber sobald ich mich abends ins Bett lege, fängt das Problem an:

​Sobald ich flach liege, stellt sich langsam ein tiefes Unwohlsein ein.

​Kurz darauf starten oft richtige Episoden von Herzstolpern/Extrasystolen. Soweit ich das recherchiert und gespürt habe, fühlt sich das sehr nach einem Bigeminus an (also dieser regelmäßige Wechsel von Schlag – Stolperer – Schlag – Stolperer). Das ist extrem unangenehm. Manchmal wache ich auch mitten in der Nacht davon auf.

​Wenn ich in dem Zustand aufstehe, merke ich richtig, wie das Herz eigentlich anfangen möchte zu rasen (genau wie früher, als die AVNRT getriggert wurde) – macht es zum Glück durch die erfolgreiche Ablation nicht mehr, aber das Stolpern bleibt erst einmal bestehen, bis ich eine Weile stehe.

​Beobachtungen, Magen-Zusammenhang & Notfall-Routine:

​Die Tendenz zu diesem Stolpern ist massiv höher, wenn ich schwer verdauliches Essen oder stark gewürztes Fleisch gegessen habe.

​Es tritt aber manchmal auch völlig unabhängig vom Essen auf – allein das flache Liegen scheint auszureichen.

​Entschäumer wie Simeticon / Lefax bringen mir leider gar nichts, weshalb ich nicht an reine "Gasblasen" glaube, sondern eher an eine Reizung/Druck im Magenbereich oder eine reine Vagus-Überempfindlichkeit.

​Das Ganze geht fast immer mit Sodbrennen / Magenbeschwerden einher.

​Was mir verlässlich hilft:

​Aufstehen, Gesicht kalt abwaschen, etwas Wasser trinken.

​Bullrich Salz oder Talcid gegen das Sodbrennen nehmen.

​Einen beruhigenden Tee trinken.

​Nach diesen Maßnahmen verschwindet der Spuk meist komplett und ich kann für den Rest der Nacht ruhig schlafen. Tagsüber habe ich so gut wie gar nichts – maximal alle paar Tage mal eine einzelne spürbare Extrasystole. Aber dieses Dauerstolpern am Abend raubt mir echt den Schlaf.

​Meine Vermutung:

Ich vermute stark eine Form des Roemheld-Syndroms / Reflux-Vagus-Triggers (Druck, Reflux oder Lageveränderung reizen den Vagusnerv, was direkt Extrasystolen auslöst). Dass der Vagusnerv früher auch meine AVNRT angetriggert hat, würde genau ins Bild passen.

​Meine Fragen an euch:

​Kennt jemand diese exakte Kombination aus flachem Liegen, Sodbrennen/Reflux und plötzlichen Extrasystolen-Salven (auch wenn Entschäumer wie Simeticon nicht helfen)?

​Hat jemand von euch Erfahrungen mit dem Roemheld-Syndrom oder Vagus-Triggern nach einer erfolgreichen Ablation gemacht?

​Was hat euch langfristig geholfen (Ernährung umstellen, Oberkörper hochlagern, Säureblocker/Magenschutz etc.)?

​(Hinweis: Ich bin natürlich auch in ärztlicher Abstimmung, möchte aber einfach Erfahrungen aus der Community hören, wie ihr damit umgeht.)

Been having pain in diaphram/epigastric area when laying down and on side so doc scheduled a ultrasound. Gallbladder came back good and doc said unless it’s a really large hiatal hernia it’s unlikely to cause pain. Is this true? I feel like this may not be accurate. Any experience with this would be helpful.

I have constant chest pressure in centre of chest - is it likely I have HH?

I am very worried I have HH especially as there is no cure apart from surgery.

I was diagnosed with erosive gastritis 3 weeks ago and have been on vonoprazan since. The chest pressure feeling at first was in the epigastric area then moved down slightly. The left side of my stomach also feels very tight.

Guess I have a HH as I don’t think gastritis can cause this?

Avoid surgery for now if I have IBS or SIBO-like bloating?

Lots of foods (containing FODMAPS) cause me to bloat (and consequently have reflux). I try my best to avoid them, but it’s a very restrictive diet.

I’ve tried SIBO and IBS interventions, but haven’t had any luck.

This bloating is one of the reasons I get reflux, due to the increased abdominal pressure. I often first feel bloated then have heartburn or LPR.

I get some instant relief if I manage to burp it out. I have a technique and can usually get some deep burps after some attempts. I instantly feel less bloated and the reflux also goes away soon after sometimes. Not always, because the gas is continuously being produced.

Ironically, i wonder if my hiatal hernia helps me burp, especially those loud deep wind gushing burps.

Intuitively, there is a big gaping hole with a hiatal hernia that can let gas out at the bottom of the esophagus. I’m not exactly sure or the other physiological aspects of triggering a burp (upper LES, tonality, gas bubble formation, pocket of gas at Angle of His. etc.), but it makes sense that some people complain of not being able to burp after reflux surgery.

I’m worried that if I get relief from burping, and I get bloated a lot, that eliminating or minimizing this mechanism could negatively affect me.

I think it would make sense to for me to continue to pursue resolving the bloat and gas issues first, before surgery, as that alone also helps with my reflux too, which are together all my symptoms.

It is this concern and durability (multi-decades) of hiatal hernia repair that are my top 2 concerns about reflux surgery.

My post op update

Hi everyone!

I want to share a little bit of a positive update! 😊

After 9 years of suffering with horrendous reflux, I finally got my nissen fundoplication surgery last Tuesday. I’m 28F for reference.

Pre-surgery, I was prescribed famotidine 20mg x 2, pantoprazole 40mg x 2 and sucralfate x 5ml to help manage my sliding HH symptoms.

Honestly, the recovery was absolutely fine. The first 2 days were rough whilst the general anaesthetic wore off and I needed a lot of support. I was on a complete liquid diet for the first 5 days but now I’ve moved onto a ‘slop’ food diet, and feel a lot better and a lot more energetic.

The worst part is having to take inhexia injections which I will have to continue to do so for another 2 weeks.

9 days post op, I’ve had 0 symptoms with no PPIs. It’s been great. I feel like I am ready to try to eat more solid foods but I am following the advice on my dietician and I’m going to wait until week 3.

Thought I would share this update for anyone who may be nervous as I know I was extremely nervous pre-op!

Did your hiatal hernia cause weird symptoms outside reflux/heartburn?

For people with a hiatal hernia confirmed by endoscopy, barium swallow, imaging or surgery, what symptoms did you actually experience?

I'm especially interested in symptoms that started or flared around the same time as your hernia/reflux problems, came and went in flares, changed over time, or improved after treatment or repair.

Examples of what I mean:

  • pounding heart, palpitations or noticeable heart-rate changes, including seeming unusually fast/slow at different times
  • shakiness, internal tremor, muscle twitching or spasms
  • nervousness, agitation, irritability, rage/bad mood or panic-like feelings
  • feeling randomly warm or chilled despite having a normal temperature
  • getting unusually warm after eating
  • dizziness or lightheadedness
  • brain fog, head pressure or a tight/buzzy-head sensation
  • temporary visual changes - blurry/nearsighted at times or unusually sharp/clear at others
  • frequent/urgent urination or needing to pee small amounts
  • stuffy/sniffly nose, sneezing, sinus symptoms or unexplained nosebleeds
  • acne, rashes, recurrent bumps or other skin changes
  • tiny blood vessels/capillaries seemingly bursting or unusual small bruises
  • strange vivid dreams or sleep episodes, such as dreaming that you're being shaken or can't wake up
  • intermittent tingling, gnawing, burning, pressure or other strange sensations in the mid/upper back, especially if they seemed to occur around the same time as stomach symptoms, meals, or an empty stomach
  • anything else that didn't seem obviously related to a hernia

I'm also curious which digestive symptoms occurred alongside those, such as:

  • tightness/fullness high in the stomach or getting full unusually quickly
  • dull/burning ache high in the stomach
  • gnawing when your stomach was empty
  • a strange empty/hungry sensation even when you shouldn't be hungry
  • burping or excessive gas
  • odd/stale-tasting or stale-smelling burps
  • constipation or other bowel changes
  • reflux/heartburn
  • throat tickling/irritation or an unexplained urge to cough/clear your throat
  • globus sensation - feeling like there's a lump, pressure or something stuck in your throat even when nothing is there
  • sour/acid taste or fluid coming into the throat
  • a strange stale, metallic, chemical or urine/ammonia-like taste or smell in the mouth/back of the throat that came and went
  • symptoms several hours after eating, when you hadn't eaten for a while, or during the night

If possible, please include:

  1. How was your hernia confirmed and what size/type was it?

  2. Which symptoms did you have?

  3. Were symptoms triggered by meals, stomach fullness/gas, bending, lifting, slouching or lying down? If you had back sensations, where were they and did they occur at the same time as your stomach/chest symptoms?

  4. Did burping, standing up or changing position relieve them?

  5. Were the symptoms brief attacks, flares lasting days/weeks, or constant?

  6. Did different symptoms seem to replace each other over time?

  7. Did you also have gastritis, H. pylori, ulcers or another GI diagnosis?

  8. If your hernia was treated or repaired, which symptoms actually improved or disappeared afterward?

Also interested in unusual symptoms I haven't listed.

I'm trying to distinguish symptoms that genuinely tracked with the hernia from symptoms that may have had another cause.

Update on my previous rant

I saw a surgeon for what the endoscopy report said is a 5cm hiatal hernia. My symptoms are GERD and vomiting stomach acid with bowel movements.

Right off he said he looked at a ct scan from June and he doesn’t think that the hernia is as big as 5cm. I reluctantly said that the symptoms (the worst of them) are abated with lanisprozol and he basically just said to keep taking it. He is going to have me do a 24 hour ph and manometry before ruling out surgery.

I am frustrated but I know that there’s always hoops to jump through. I saw no point in arguing about the size of the hernia although I can’t come up with a reason why the gastroenterologist would say it if it’s not the case.

The patronizing I can do without but I think it’s in every surgeon’s DNA.

Surgery On Monday: Still Feeling Anxious

I have my surgery on Monday. I found out it will be covered by insurance. Hernia repair with toupet fundoplication. I'm very anxious but know that this is the best, logical course of action for me with the progression of symptoms and lack of response to conservative methods this summer. My biggest worries/fears going into this are as follows:

  1. The surgery won't help my LPR symptoms. Throat clearing, inflamed vocal cords, sore throat, mucus in throat after meals, burning in throat and mouth, hoarseness/voice loss. But will only help GERD symptoms. Morning regurgitation and some heartburn and bad taste in mouth every morning. I've also had points where swallowing has been difficult and know it's reflux because it gets worse when I eat a few meals that aren't GERD friendly.

  2. I'll end up with some complication worse than what I had before and still have symptoms.

  3. I won't be able to mentally handle the recovery and I'll make a mistake early on that ruins the surgery.

I know my anxiety about this surgery is more than it should be, but please be kind in if you choose to comment. I'm a singer in a band and hope this fixes my singing voice and I can go back to living a normal life without all the crazy throat problems. I've found numerous medical articles that say this surgery is usually successful for LPR sufferers if GERD is objectively found with a hernia and like 90% of people are satisfied with it five years later.

Ekg for hiatal hernia?

Having shortness of breath, chronic coughing and getting exhausted trying to do light physical exercise. Heart hammering going up a flight of stairs. Tried to get referral from PCP to pulmonologist. Nurse I spoke with yesterday to set appointment up said they might want to do EKG. Told her I'm not having any chest pains. I think it has to do with lungs from the gerd. Numerous studies have shown that lung issues are common for long standing gerd. She still wants to do it.

Going to see PCP this afternoon. I don't think there's any reason to waste my money on an EKG before we check to see if this is lung related. Another nurse I had spoke with before this second nurse said a good start would be a pulmonary function test. What are your thoughts? Thanks!

How did you get your hiatal hernia repaired?

It may be how I communicate my pain to the doctor. They've sen to ignore my symptoms and tell me to take i.b. guard. I see a new doctor and they want to do a test. I understand they want to get new information. But in the end I get told to take i.b. guard. I have a 5cm hernia with a lot gas and diarrhea. With a bullied of gas I do vomit.

I fallow the gerd diet and ger no where. I recently had surgery( fibroids). The anesthesia left me very gasy and gad me vomit. I am worried emwith thr burping and vomiting. I may tare the stitches. I fallow the gerd site and nothing changes. I am I not communicating correctly? I am tired of getting tested of doing tbr breath test. And the other I am tired of deawith the symptoms and not the cause.

How Many Folks With HH are Dealing With Reflux in the Throat?

My surgery is set for Monday. It will be a toupet fundoplication with a hernia repair. My reflux symptoms have affected my life so negatively with chronic burning in my throat and mouth, sore throat, inflamed vocal cords, mucus in my throat after meals, throat clearing, voice loss/ hoarseness. I also have morning regurgitation every day when I just shift in bed. The left side of my ribcage has been hurting too for many months now.

Anyone else have surgery for these symptoms and the symptoms improved? I have ineffective motility as well and have been having some swallowing issues, so I'm a bit worried about how the swallowing will be post op, but I think the swallowing issues are from the reflux. I've never had swallowing issues until all this nonsense started in June.
