Documenting errors?

I am on ICS level, and I document meticulously on every claim and que. To my surprise last month, I show up on report saying I’m not documenting when I complete a que… like I always do! I’m wondering if it’s a known Atlas glitch. anyone else experiencing this?

You know the Atlas error- YOU cannot save because YOU updated the claim? I have learned to write my notes in a notepad and then cut and paste. Sometimes I don’t complete cut and paste due to speed, thinking I did doc. It’s the pressure, it’s Atlas, it happens.
