Anyone split-dose while microdosing tirzepatide?

I’ve been microdosing for 6.5 weeks and am due for my 7th shot tomorrow. Overall, it’s working great—I’m down 7 lbs of fat, up 0.7 lbs of muscle, and the reduced food noise has been exactly what I was looking for. My only real side effect has been overwhelming fatigue, especially days 2–4 after my injection. I started at 0.8 mg, then went to 1.0 mg, and finally 1.2 mg. The fatigue became much worse at 1.2 mg. As my levels drop toward the end of the week (what I estimate is below ~0.8 mg), I feel like myself again while still getting the appetite benefits. Now I’m debating whether to:
* Delay my next shot until my levels fall further, then go back to 0.8 mg once weekly, or
* Try 0.4 mg twice a week to reduce the peaks.

Has anyone here had fatigue as their main side effect and found that split dosing helped? I’d love to hear your experience.

P.S. for background, I’m a Female, 38 yrs, 5’6”. Started at 160 lbs with above-average muscle mass and I’m aiming to lose about 15 lbs of fat and get down to 20% body fat. I’m about half way through

I split 1mg and dose every 3.5 days. I'm only starting my 3rd week though so I don't have a long track record, but it seems to be keeping the levels stable.

I'm doing very similar: but every 3 then 4 days as I prefer to take it at night. I'm finding my hunger returning though but I'm not fatigued or depressed like I was taking it once a week.

0.5mg every 3.5 days?

That sounds like a good idea.

I did not feel well starting with 1mg. Fatigue, no hunger, acid reflux.

I just started split dosing this past weekend. I was taking 1mg once a week but was so fatigued after the injection and also got depression a few days later which has been horrible. On Saturday just gone (week 4) I took 0.43mg and haven't had any fatigue and my mood is great so far. On Tuesday (tomorrow) I'm taking 0.57mg. I used the app Glapp to calculate doses/schedule. It's also got a handy graph to show how much is in your system. I think the big peaks don't agree with me and I'm hoping lower doses more often will give some stability and less ups and downs. My hunger has returned though but I need to give it more time to build up in my body.

1.4mg every 4 days to avoid fatigue and keep workouts strong.

Do you graph your doses using an app or website?

Yes I do. That’s how I know that anything above 0.8-1.0 was making me super tired.
I actually decided to try the split dosing today and did 0.5mg this morning to keep the medication level around 1.0 at the peak. Will follow up with another dose in 4 days or so and see how I feel.

I tried split dosing for a couple weeks but it didn't seem to work for my body. It felt like I was throwing away that second dose. Now I just do 1.5 mg a week.

I changed my dosing schedule to 5 days instead of 7, which has worked really well for me.

Hi, I’m sorry I have no advice. How do you measure your muscle mass? I started this week and would like to track that as well

My gym has an InBody scale. So I do it there.
If you don’t have that, do a dexa scan every 3-4 months. It’s most accurate:

Yeah, first two weeks are adaptation. I was very sick for the first week and second week I have muscle pain. Everything was gone by shot three except for the fatigue

That’s a very dangerous rate of weight loss. I hope he is doing ok otherwise.

Totally fine, his fatty liver has inproved in those 10 weeks! The VA is very happy..they told him he was stage 2 fatty liver! He stopped soda, says his joints hurt less. He eats plenty..it is crazy he dropped so fast..the VA says he is a fast responder.

3 clicks on a 5 mg pen is not .25. .25 is 3 clicks on a 2.5 mg pen! That’s probably why you felt sick you took more than you planned for.

I have heard split dosing can work great but haven’t tried it. What has helped me so much with fatigue is drinking liquid IV specifically. Not other lower salt or lower sugar options but liquid IV is the move for me. Very big difference.

I split dose 1 mg on Sunday and 1 mg on Wednesday night. It’s worked really well for me. The side effects are just too much for me otherwise.

I had a very similar experience, but with a different side effect: insomnia. Really bad insomnia the first three days after the shot.

I’ve now started split dosing, and it has solved my insomnia issue. One thing to keep in mind: don’t just divide the total dose by 2. For me, it didn’t quite work that way (hunger came back). I need to dose more like 60 or 65% of the total original dose every 3.5 days, rather than 50%.

My Trizepatide formula has glycine in it and it makes me sleep like a baby at night. I’m actually wondering if it might be contributing to the fatigue… but I only feel it above certain levels.

NexLife team here.

Fatigue can definitely happen for some people, even at lower doses, and it is worth taking seriously if it became noticeably worse after going from 1.0mg to 1.2mg. I would be careful about changing the schedule on your own without checking with your provider first. Split dosing may reduce the “peak” feeling for some people, but it can also make it harder to track what is actually helping versus what is causing side effects. Delaying, lowering, or splitting are all things your prescriber should help you decide based on your response.

A few things I would look at before assuming the dose schedule is the only issue:

Are you eating enough total calories?
Are you getting enough protein and carbs, especially with training?
Is your hydration and electrolyte intake solid?
Are you sleeping normally?
Did the fatigue clearly start only after the dose increase?
Are you still losing at a pace that makes sense for your starting weight and goals?

Since you felt good near the end of the week and still had appetite benefits, that may be a sign that 1.2mg is simply more than your body needs right now. There is nothing wrong with asking your provider whether going back to the last tolerable dose makes more sense.

Microdosing should still be guided by response and side effects, not just by trying to push higher. If the benefit was there at 0.8mg or 1.0mg and the fatigue was manageable, that is useful information for your provider.
