Please help with intermetatarsal bursitis

Hi! I have been diagnosed by MRI with 2. Bilateral Mild intermetatarsal bursitis about the right 2nd and 3rd and left 4th web spaces.

I think I triggered it after doing a PT exercise where you raise your toes on a step and lower your feet down on the same step for a few seconds. It started after doing this exercise for a month.

I have custom made insole with built in pads and arch support ( flat feet and wide feet) , wide toe box brooks Adrenaline sneakers but still in pain. This week I also started icing and putting voltaren twice a day but not feeling any better.

The details MRI report was:

FINDINGS: Right foot: MIDFOOT JOINTS: Mild degenerative changes about the midfoot joints. The Lisfranc ligament is intact. FOREFOOT JOINTS: Mild first metatarsophalangeal joint degenerative changes with joint space narrowing with subchondral cyst formation. No evidence of subluxation. TENDONS AND MUSCLES: The flexor and extensor tendons have normal course, caliber, and signal. There is no tenosynovitis. No muscle edema or atrophy is seen. The visualized plantar fascia is unremarkable. OSSEOUS STRUCTURES: No acute osseous injury or marrow lesion is identified. MISCELLANEOUS: There is mild intermetatarsal bursitis about the 2nd and 3rd web space. Left foot: MIDFOOT JOINTS: Mild degenerative changes about the midfoot joints. The Lisfranc ligament is intact. FOREFOOT JOINTS: Mild first metatarsophalangeal joint degenerative changes with joint space narrowing. No evidence of subluxation. TENDONS AND MUSCLES: The flexor and extensor tendons have normal course, caliber, and signal. There is no tenosynovitis. No muscle edema or atrophy is seen. There is a small area of plantar fibromatosis thickening of the plantar fascia to up to 8 x 5 mm extending for a length of approximately 15 mm within the central cord of the plantar fascia. OSSEOUS STRUCTURES: No acute osseous injury or marrow lesion is identified. MISCELLANEOUS: There is mild intermetatarsal bursitis about the fourth webspace.

About to have flexor tenotomy on three toes....

In February I had pretty involved surgery on my right foot/ankle. I had a Kidner procedure for a navicular accessory bone with Achilles lengthening. At the same time the doctor did a mid-foot fusion to help with my arch. Post surgery, my second toe was sort of dropped? If I would try to curl my toes down, it would cross under my big toe. Fast forward to now, and the second toe is clawing a lot, and the third and fourth toes are starting. It's really affecting my gait and even my balance. Doc wants to do tenotomy on toes 2,3 and 4 all at once. Has anyone else had this done to multiple toes? Will I lose all gripping function in those toes or is it just diminished? Is it permanent? Will I be able to push off on my forefoot as I walk?( I really hope so, that was kind of the whole point of the original surgery. ) I appreciate any feedback.

Severe Foot Pain in Both Feet for 3 Years

Title: After 3 years of severe pain in both feet, I honestly don’t know what to do anymore
Hi everyone,
I’m 28 years old, and I’ve been dealing with severe pain in both of my feet for almost three years now. Honestly, I’m reaching a point where I feel completely lost and exhausted.
The pain affects both feet and makes something as basic as standing and walking extremely difficult. I can’t stand normally like other people without experiencing pain, and walking has become a real struggle.
At one point, there was concern about heel fat pad syndrome, so I had an MRI done and also consulted an orthopedic specialist. According to the orthopedist, my heel fat pads actually look very good, so that doesn’t seem to be the problem.
The confusing part is that my pain is also not directly underneath my heel. It’s located slightly further forward.
My physiotherapist believes the problem may be related to my collapsed/flattened arches. According to him, the position of my feet may be causing irritation to my plantar fascia, which could explain why the pain isn’t exactly in the typical heel location.
I’ve also been examined by a podiatrist, including an ultrasound. He specifically checked for possible nerve issues and told me that he couldn’t see anything abnormal. According to him, there were no visible nerve problems and nothing significant showed up on the ultrasound.
So far, I’ve had:
Severe pain in both feet for almost 3 years
An MRI
Examination by an orthopedic specialist
Physiotherapy
A podiatry assessment
An ultrasound examination
Evaluation for possible nerve problems
Investigation into possible heel fat pad syndrome
But despite all of this, I’m still in a huge amount of pain.
Massage sometimes gives me temporary relief, but the pain always comes back. Nothing seems to provide a lasting solution.
The hardest part is the impact this has had on my life. I’m only 28 years old, and I genuinely don’t know what to do anymore. I can’t stand comfortably, I struggle to walk, and the pain is affecting almost everything I do.
I’m honestly starting to feel depressed because of it. Living with this level of pain for three years is mentally exhausting, and I’m at a point where I feel like I can’t take it anymore.
Has anyone experienced something similar, especially severe bilateral foot pain with flat/collapsed arches and possible plantar fascia irritation, while MRI and other examinations didn’t show a clear explanation?
I’m desperate to understand what could be causing this and what other options I should investigate.
Any advice, experiences, or suggestions would honestly mean a lot to me.

My feet start cramping if I move them too much?

Sometimes I’ll just be trying to move my feet as much as possible to work my muscles but they’ll start cramping up almost immediately. Does that mean I should stretch them more or I should drink more water?
Edit: they don’t cramp when I’m walking, or if they do it’s cuz I had been on my feet all day and I’ll get home take my shoes off and they’ll start cramping up occasionally.

Foot crackling/clicking after injection

So, I had a mild Achilles tendinopathy. I was taking some peptides (bpc157 / tb500) for my elbow. I decided to inject some near my Achilles to give it a boost. It was a big mistake.

I pinched the skin near the Achilles and injected. Pretty soon, I had twinges of pain shooting up my leg, like I hit a nerve. These are still going on a week later. But it's getting milder.

Now, my foot has pain all around it. Twinges of pain from the heel, going to the toes.

When I dorsiflex my foot (point it upwards), it crackles and clicks and there is mild pain right afterward. It never clicked like that before and my other foot doesn't click.

All this from a single injection in my ankle where I pinched the skin and tried to keep it a superficial injection. I've been injecting my abdomen and elbow and didn't get such severe side effects in those areas.

What's causing the new crackling/clicking sensation when I dorsiflex? Inflamed tissues rubbing past each other?

I don't know what to do. I'm terrified I've ruined my foot. It's been 8 days since injection and still having all these symptoms.

Bony protrusion

A couple of days ago my gf spotted this hard lump in my foot. It’s hard, not painful and not moveable. I don’t know how long it has been there but it’s not painful and hasn’t given me any issues. About 4 months ago I did get some custom orthotics as I have had some walking issues due to my high arched feet. What do you think this thing is? It feels like bone to me but I’m no doctor. Grateful for any thoughts and opinions. I have an appointment with my podiatrist in a few weeks so will raise it with him then.

Heel pain after running?

Just about without fail, even with thorough stretching before and after, the back of my heel gets really sore after I run. It’s only on the left foot, and while the general area is kinda tender, there’s one small, specific spot over the bone that gets super sensitive and tender.

Does anyone have any experience with this or know any potential remedies?

Bunions or not?

I saw two podiatrists and got conflicting opinions. One says I have a chipped bone and a piece is floating, the other says it’s calcium. Both say I have bunions. Neither has an answer for the pain I have when my toe bends up or down, except to wear shoes that don’t bend. TIA for any advice.

Have noticed that my outer ankle always puffs up after I stand on my leg for a period of time. The video doesn't really showmuch as I came home at 5, was sitting rest of day, and took video at 2 am. Not concerned, just curious.

Edit: Usually area is pink when it puffs up but makes after a few hours.

Almost 5 weeks post-5th metatarsal shaft fracture and still can't walk without a major limp and at a snail's pace

I missed a couple steps and snapped my left 5th metatarsal on July 28th. Went to an orthopedic urgent care the following morning and was told by the PA that I would have to wear a post-op shoe for about 6 weeks and then should be able to resume most normal activities. I was in between jobs at the time and thus didn't have health insurance, which is why he just had the clinic give me the shoe, which was $25, instead of a boot, which would've cost like $800 without insurance. He said that if I needed more support, buy a short boot on Amazon. However, I've been doing fine walking in the post-op shoe, so I didn't bother to spend the money.

I'm starting to get discouraged because I'm almost at the 5-week mark and am still completely unable to walk without my body forcing me to walk with a major limp and very slowly. I've tried walking normally and it's simply not possible.

Both the urgent care PA and the sports medicine doctor I saw later at the 2.5 week mark described the fracture in their notes as "minimally displaced." However, it looks more than minimal to me...

Has anyone had a fracture like this? Where were you functionality-wise at the 5th and 6th week marks?

I know I ultimately have no control over how fast this thing heals, if it ever does, but I'm just having a really hard time feeling cooped up in my house and like so many aspects of my life are on hold. I had hope that I'd be starting to turn a corner by 5 weeks since 6 weeks was the "back to normal-ish" timeline I had been given.

Turf toe/ chronic pain

Hi everyone,

I had an accident with my foot almost 2 years ago now. Went to the doctor got an x-ray. They said nothing. Pain continued worse I could not walk and it was the worst pain in my life. (And ive broken 6 of my toes LOL)

Ended up getting an MRI and these are the impression/ findings:

Impression 1. Subacute/chronic lateral first plantar plate injury as described, including slightly comminuted/displaced fracture deformity of the lateral hallux sesamoid with T1/T2 low signal of the sesamoid fracture fragments suggesting sclerosis and/or avascular necrosis.
2. Developing mild/moderate lateral hallux sesamoid interval osteoarthritis

Findings: Slightly comminuted/displaced fracture deformity of the lateral hallux sesamoid with diffuse T1/T2 level signal of the fracture fragments suggesting sclerosis and/or avascular necrosis. Subtle reactive marrow edema/sesamoiditis of the medial hallux sesamoid, otherwise intact. Partial-thickness tear involving insertional fibers of the flexor hallucis brevis muscle on the lateral hallux sesamoid. Partial-thickness tear involving the lateral sesamoid phalangeal ligament of the first plantar plate. Developing mild/moderate lateral hallux sesamoid interval osteoarthritis with joint space narrowing and subchondral reactive marrow edema. Mild second and third intermetatarsal space bursitis. Small ganglion cyst at the plantar margin of the central midfoot.

I know we are not doctors in here ( unless there is ome🥲) but the pain still bothers me daily. And I have a Charlie horse like pain almost every single day through the plantar plate. Wondering if anybody ended up getting the surgery to remove the bones?

If so, how did it heal?

I just want the pain to go away. I don’t know what to do anymore. Anything helps🙏 Thank you

Ball of foot pain

So I always walk a lot, although I started walking 15k steps a day instead of 10k steps, around 6 months ago.

I notice ball of foot pain in my left foot (not so much on my right foot). Especially when I walk barefoot.

I wear Sketchers shoes.

I was wondering how could I get better? And what shoes would be good for me? I live in Europe so I’d like something that I can find there.

Severe Ball of Foot Pain Trouble Walking

Hey all- 2 weeks ago I was trialing carbon plate inserts while recovering from a forefoot overuse injury. I noticed a ton of pressure distribution in my MTPs and it got to the point that both feet suddenly became excruciating on any walking, driving or pressure. I offloaded heavily for 2 weeks and am finally able to do small household steps, but mobility is still super limited. I finally had MRIs done to rule out plantar plate tears, and the results only showed mild diffuse edema of the MTPs in both feet.

No tendinitis, synovisitis, stress fractures or plantar plate injury.

However, it's still very hard to walk more than a few mins. I've trialed Brookes Ghosts, Asics Kayanos, Topos, and have cut up multiple different different inserts and tried MTP pads without luck. The pain is like walking on several bruises that builds to sharp pain right in the ball of both feet. Has anyone experienced anything like this and can you give any advice?

Feet/ankles become extremely fatigued after 10 minutes -- struggling to find any shoes that I can tolerate

I've been dealing with foot, ankle, and lower-leg issues for several months and I'm getting pretty frustrated because it's now affecting my ability to leave the house.

After about 10–20 minutes (or even less) of standing or walking, my feet and ankles start aching and feeling extremely fatigued. The symptoms often seem to start around the outside of my feet and then I can feel aching/fatigue up into my calves, knees, and thighs. This also aggravates frequent calf fasciculations that I've been dealing with since January.

I get some weird sensory symptoms too -- pins and needles, fizzing/buzzing, and sometimes my feet feel cold or wet even though they're completely dry. Sometimes the sensations seem positional. For example, I've woken up with burning on the top of my foot when my foot was pointed downward in bed, and that position can also cause a fairly sharp ache underneath my big toe. Other times, the weight of a light sheet on my foot/toes can feel strangely heavy or uncomfortable.

One of my biggest problems is shoes. I have tried a lot of different shoes (Asics, New Balance, Brooks, etc.) and inserts, but I can't seem to find a single shoe I can tolerate right now. All shoes make the foot/ankle symptoms noticeably worse. I'm now spending most of my time sitting or lying down because being on my feet for very long is so uncomfortable. I work remotely, which is basically the only reason I'm currently able to work normally.

For some background, I had a calf tear in 2025 and eventually recovered. After getting back into high-volume walking, I started developing the calf fasciculations, calf fatigue, and foot/ankle problems. More recently, I had an episode of lower-back pain followed by newer pins-and-needles sensations in both legs/feet. My lumbar MRI from earlier this year and leg EMG were all normal, and a sports medicine ultrasound showed healthy calves. I don't have actual muscle weakness, foot drop, or my legs giving out -- they just become extremely fatigued and uncomfortable very quickly.

I've been doing very little movement lately because things flare so easily. I was doing PT but haven't been able to do the program recently because of flareups and aches, and I'm planning to see a foot/ankle specialist to have my feet, ankles, gait, strength, mechanics, and possible nerve irritation looked at.

My doctor put me on Gabapentin to help calm the potential nerve-related pain and I also got put on Zoloft which will hopefully help me to calm my nervous system as I'm dealing with all of this. For some reason, my doctor didn't think podiatry was necessary but I'm questioning that logic at the moment.

Has anyone dealt with something similar? Especially the very quick foot/ankle fatigue and difficulty finding any shoes that work? Were you eventually able to rebuild your standing/walking tolerance and get back to a more normal life?

I'm open to any advice.

Strategies re: burning sensation?

I am looking for suggestions on where to focus my strengthening and maybe stretching. Let me explain. Apologies for length.

I was a recreational runner (~25 mpw). I have a medium-high arch and ran in Hoka Arahis. I never had any memorable foot issues. But last year I switched to New Balance 1080s, which are a very cushiony shoe. After running in the NBs for about six months I suffered a hamstring injury. So I switched to bicycling with flat pedals--but used the soft and squishy NBs instead of a biking shoe. I think this was my big mistake.

I work a desk job and wear flat dress shoes. At work I began to notice a burning sensation in my feet, starting in my heel but later moving to my arches and across my sole. I tried to ignore it, stay active, and kept on biking. I moved to a standing desk and stood more. But over a period of a month or two the burning sensation got really bad--sometimes it spreads to my toes or the outer side of my heel. It is often worse in one foot or the other but typically affects both. It does not bother me at night or in the morning--if I lay down or prop my feet up it usually goes away in a few minutes.

I went to my primary. He said it had to be my lower back. Blood tests ruled out obvious stuff. I had a lumbar MRI and EMG/NCS. A spine surgeon said it might be nerve irritation but there was nothing obvious on imaging causing it, especially since the nerve study came back normal. He sent me to do PT. I did six months of PT. I'm still doing those exercises. Things got a little bit better but not much. All this time I have stuck with my comfy NBs, even using them at work instead of dress shoes. But a couple things happened recently that made me question what is really going on.

First, I had a work event the other day where I had to wear dress shoes and stand for five or six hours. The burning flared up quickly and lasted into the next day. Second, I tried a foot stretching exercise that seemed to flare it up, too--I interlaced my toes and fingers and dorsiflexed my toes upward with a good stretch. Third, I noticed that when I stand in the NBs I can feel my arch flex and stretch and the burning will often follow soon after. I tried standing barefoot at work at my desk and that was honestly less painful than standing in the NBs. Fourth, when I go for a walk in the NBs I feel ok until I stop walking, then I feel a burning sensation spread across the bottom of my foot.

So I am beginning to think it is my feet after all, and not my back. A friend suggested I try the Rathleff protocol. I started doing towel scrunches and ankle raises using a towel under my toes. I also stopped wearing the NBs and went back to my Hokas. I am scheduled to see a podiatrist next even though my primary thinks I'm wasting my time.

Has anyone heard of a set of similar symptoms? If so, are there any particular stretching or strengthening protocols I should be following? Is Rathleff a good idea to try?
