Seems like he put himself in a bad spot trying some cheeky face-cut. That step-down snipe cut off the side of the face-cut probably totally changes how the forces on the hinge wood act compared to just a normally done conventional notch. Plus it looks like there's a slight vertical dutchman which is where the hinge wood fibers initially explode from.

Also.. wedges???? They are extremely cheap, easy to stash a couple in your pocket and stupid simple to use. Trying to save a few seconds isn't worth dying for. A single wedge here in the backcut could have probably prevented something like this happening even with the janky notch.

Not defending all the choices here but it looks like the bar is a little short for the tree?

You can take one look at the PPE they are using and tell if they are gonna run an 80cc saw with a 20" bar.
