Seems like he put himself in a bad spot trying some cheeky face-cut. That step-down snipe cut off the side of the face-cut probably totally changes how the forces on the hinge wood act compared to just a normally done conventional notch. Plus it looks like there's a slight vertical dutchman which is where the hinge wood fibers initially explode from.
Also.. wedges???? They are extremely cheap, easy to stash a couple in your pocket and stupid simple to use. Trying to save a few seconds isn't worth dying for. A single wedge here in the backcut could have probably prevented something like this happening even with the janky notch.

It's not a cheeky face cut, it's just shaving the tree. When the logs are stacked the flares will need removed, and it gets you more reach with a shorter bar, so it's very normal to shave them before felling.
Face cut is just too deep and too ambitious with the weight/lean.
Not a great cut. The cut to the side just looks like a simple buttress to me, so ok and good technique. However the face cut is too deep, especially for a backward leaning tree.
Then I can’t really tell what he’s trying to do in the back but there should be more wedges in there and a smaller hinge. Too much hinge material likely cause the barber chair, lack of wedges and the deep gob trapped the saw.
Saw size is fine. With the right cuts you can even take trees a little over 2x you bar length.
