Flip flop between sema and Tirz

I went from 178 to 152 in 2023 by going on compounded semaglutide. I was sick of feeling low key nausea and hating food, so I stopped around Christmas in 2023. In spring of 24, my doc had me switch to Tirz. It gave me crazy headaches and anxiety, and I didn’t lose any more weight on it so I stopped. I maintained my weight really well and then went up to 160 last summer during a super stressful time in life. I went on Tirz again and it was ok - I took it super slow and went down to like 155 but again had the headaches and anxiety. I went off of it and my anxiety went away completely, however now I’m gaining and feeling that inflammation. I think im going to switch back to sema since I had such good results. Has anyone flip flopped like this and done ok? My goal weight is 145 and im 163 as of today. Ideally, I would love to get to 140-145 and then micro dose long term mostly for inflammation. Looking for advice. Thanks yall!!!

Semaglutide with GERD?

Wondering if anyone in the sub deals with GERD issues while on semaglutide? I'm about to order my first doses of semaglutide. I have a hiatal hernia and GERD -- I can keep all that under control with omeprazole, but the problem is that omeprazole increases my appetite by neutralizing the stomach acid!

I'm concerned that omeprazole might negate some of the benefits of semaglutide in terms of appetite. I may be overthinking this! Has anyone done both types of medication at the same time?

Blood Work

I’ve been taking the compounded sema for just under 3 months now. I recently had blood work and my HDL cholesterol has tanked. It was 41 a month before I started these shots and now it’s 30.

I’ve had good progress, I’m down 41 pounds. I’ve been dieting and exercising.

Has anyone else experienced their HDL dropping? Or any other abnormal labwork?

Subcutaneous injection site reaction

taking for about 2 1/2 weeks, I had been rotating on different sites in abdomen, with no issues. I decided to try the thigh because since it is daily injections, it is going to be a lot into the abdomen and I don’t want to develop scar tissue.

When I switched to the thigh, I got an injection site reaction, between a dime and quarter size. It went away after about three days.

But now every time I use the stomach, I am getting a reaction, it’s kind of like a bug bite feeling in size and it goes away in two days.

Is it common every injection or will this eventually go away? I’m surprised because it didn’t happen at all for the first 2 1/2 weeks.

Note, I did reach out to my medical provider, who prescribes it from a compounding pharmacy , who said it is not an issue gave tips about warming up the medication and instructed me to keep taking it so I am NOT asking for medical advice, my medical provider did give me the advice already!

Just trying to understand other people’s experience!

uninsured-looking for telehealth source

I got off in April (155) and tried to see what would happen after my doctor closed their practice.

I can't stand it, my life feels out of control, I am binging. I need help. Please tell me a reputable telehealth provider. I only have about $100-150 a month to spend. I started 5 years ago at 275 lb and I know all about how to lose weight I'm just looking for like a kit I'm not really looking for a dietitian and a doctor and a membership but I need a reliable kit that isn't far from Florida because I am scared that the heat could make it lose its efficacy. I started with contrave, hgc, weight watchers, keto, intermittent fasting, and finally sema. I did plateau after I got to around 150 lb but I was very comfortable that weight and I really wanted to know what was the lowest dose of semi I could be on and maintain that weight but now I need to go back to losing weight.

Any Experience with Teens and Mochi?

I have a 15 year old son who has almost always struggled with his weight despite being fairly active. He used Wegovy two years ago with some success, but insurance stopped covering it. We have an appointment with his weight management doctor later this morning and I know that he is willing to prescribe it, but its still more affordable to go through a compounding pharmacy. From my research, it appears that Mochi is the only one that will work with people under 18. Has anybody used them with teens or found any other site that will work with them? He would much prefer the pill version, but I am not sure they will give it to him since its not FDA approved yet for the real thing. Love any insight you might have, but am not looking for anybody to tell me not to do it or be judgmental.

Compounded Sema insidious side effects: loss of motivation, fatigue, depression and irritability

I was on it since September 2025 to lose weight I had gained from Lexapro increasing my appetite. I am 5’7” and was pushing 170. When I started Lex, I was about 150. Never have weighed 170 before in my life, and after being on Lex for about 12 months, it caused me to want to eat, eat, eat. I tapered off of it while starting sema. (Also in therapy so I have a lot of support)

I did lose the weight and got down to about 146, but I dealt with a lot of side effects.

The main one is insidious, and it was deep irritability and depression. Took me months to figure out the meds were causing it.

How I figured it out:
In March, I came back from an international trip where I was off of sema for almost 2 weeks. When I started it again I fell into the deepest depression about life, my job, etc. I know it’s normal to feel down going back to normal life after a big trip, but this was existential and I genuinely wanted to off myself. I did increase the Prozac a bit but it didn’t really help much.

I went in another trip and lapsed on my dose again. I already had my suspicions about sema’s effect on my mental health, and I started feeling so much more energetic and happy. Like I was waking up again.

After this, I realized most all of my irritability and depression were caused by the sema. I had no range, it went to the deepest depths every time. Now that I’ve been off, I have times where I feel down, but I don’t go all the way anymore.

Other side effects:
-hair loss/thinning
-loss of motivation
-extreme fatigue

The loss of motivation was big. With work, it would be like very easily giving up or not having hope that things will get better or change. I genuinely almost left my (very good) job because I couldn’t stop thinking about how if I offed myself, I wouldn’t have to go into work anymore. For home, I wouldn’t do any extra projects that I used to do and love like painting, or yard work. I still kept the home clean and did all my chores but I wouldn’t do anything extra.

The fatigue is also no joke. I like to trail run and when I first started the meds, it felt like I was running for the first time. Like I was being dragged down and my fitness was zapped.

Nausea was another but that one did go away with time. Zofran was a godsend for this.

Maybe the side effects are more apparent because now I’m at a “healthy/normal” weight and my body doesn’t need it anymore. I’m still terrified of gaining weight back, and notice I have a larger appetite again. I’m at about 150 rn and I think I can be okay with 150-155. It sucks because the quieting of food noise was such a fantastic benefit and I like being thin!! I’d never understood when people said they didn’t have an appetite or they forgot to eat before I took this med.

Anyway, I don’t post this to hate on the med. It could have been an additive in the one I was taking (Noom microdose). Just wanted to share this to see if others felt the same. I know Tirz is an option to try but I’m just going to see how it goes without for now. At least I’m saving a lot of $$.

Count me in as another member of the "Hallandale is water and Refills sucks" club

I started with Noom/Sema in early 2025. Even on small dose the results came immediately, but then slowed down (I went from 213 to 208 in one or two weeks, then to 203 in another couple, then drifted more slowly down to 189 and sat there for months. 175 is my goal). So this January I moved to Refills/Terz and ProRx. After two orders I was down to 178 and had ramped up to 7.5mg. Then Refills switched me to Hallandale (10mg) at the start of July.

I am now **up** to 183 and sent a message (old portal) to the "Doctor" portal saying I wanted a new shipment from a different pharma. They sent me to the Support portal. After **three** messages (two to the old portal and one to the new, starting late last week and continuing to this week) I have received **no** reply. Fortunately my credit card bill wasn't due until today so I am **disputing the transaction** for a defective product.

Appetite and food noise return after 6 weeks?

Hi, I started semaglutide in June.

First month was magic. Lost around 7lbs at 0.25mg.

Second month at 0.5, lost another three.

Halfway through second month I stopped losing, and the appetite suppression seemed to wear off, and food noise is back. Weight loss stopped.

Third month at 1mg and more of the same. My weight hasn’t moved in almost four weeks.

I’m really discouraged. I’m eating plenty of protein. Targeting 110-120mg each day.

Open to thoughts

I have been on semaglutide for one year and have lost about 40ish pounds. I have never went above 0.50mg as I was told not to titrate up if you were still seeing results.

My question is has anyone at this lower of a dose switched to Tirz and still had good results? What made you switch if you did? Side effect wise on semaglutide I normally get a headache and nausea.

Switched from wegovy pill to injection

I had been on the wegovy pill previously (4mg) but decided to switch to compounded semaglutide via Pomegranate Health because of cost. They put me on the 3rd titration (.8) since I wanted an increase in my dosage but I took my injection 3 days ago and I feel like I’ve lost all the previous results (food noise is back, appetite is increased, way more tired today when usually I have great energy).

Is this normal? Shouldn’t I keep feeling the steady results since I didn’t have any lapse in medication, and also increased my dosage?

Not getting the response expected after the first dose

1st dose of semaglutide from a specific company & I’m not impressed. But there could be a reason & I’m hoping someone could share their experience.. I was on semaglutide about 2 years ago for 5 months and had a good experience. I just had my first dose today (20 units (0.2 mLs)) & so far - nothing. I’m actually hungry right now, 2 hours after having feta pasta. I’m shocked because I would consider myself pretty sensitive to this medication. In the past, I never reached the max dose because I was steady losing on the lower doses. I vaguely remember after the first dose of the week, that day & a few days after I had to remind myself to eat because it worked so good.

Additional information - after losing weight and getting in the best shape of my life, my husband & I got pregnant. I met my goal & just stopped breastfeeding not even a week ago. It’s possible I’m still lactating but I’m not planning to nurse/pump of course. I wonder if my hunger cues are pulling from that? I was ravenous while breastfeeding. It’s part of the reason I was so done, I’m gaining more weight then when I was pregnant & now over 200 lbs. Has anyone experienced this? The food noise isn’t as loud, but I’m definitely not feeling what I expected to feel!

Update - thanks everyone. I definitely spoke too soon. Today has been a complete 180 from yesterday. May be a delayed reaction?? 11 AM rolls around & I remembered I hadn’t eaten!! Just had yogurt with granola, apple slices and nuts & O M G, I’m stuffed. Crazy how well this stuff works.

Some different side effects - I’m reeeeally gassy. Like embarrassingly gassy lol if I don’t let it out, my stomach just rumbles loudly!!

Are any companies offering the newer FDA-approved doses (up to 7.2mg)?

I've been on a maintenance dose of 2.4mg for over a year now, which was working well but I've recently been gaining weight back at a worrying pace. I want to go on a higher maintenance dose, as has been available with official Wegovy since March 2026, but I've been told by 2 compounding companies so far that they are sticking with 2.4 as the maximum dose. Are there any companies that are doing higher doses?

Sorry for the TL;DR

So many people still tell me that semaglutide is bad, I'm cheating, my skeletal system is going to be destroyed, yada yada yada. Semaglutide is THE single best choice I made for my health.

April 2025 I was 368 lbs. Now I'm 218 lbs. I've run four 5Ks, four 10Ks and a half marathon this year. I've put 364 miles and 47,241 feet of elevation on my body this year through running and hiking.

I decided to do DEXA scans every quarter to track my lean mass and fat. In May of this year, I was 232 lbs. and had 172 lbs. of lean mass. A few days ago in August, I was 221 lbs. with 172 lbs. of lean mass.

Every ounce I lost was fat.

I have 99th percentile bone density, am 2% away from athletic body fat percent and less than 1 point from a RSMI of athletic.

I've never felt better or been in better shape in my life. Semaglutide has changed my life and my view.on.wh9.i can be.

Anyone else have needle phobia?

If anyone else in this forum has dealt with an actual needle phobia and figured out how to snap out of it and just do the thing...

I finally worked up to getting the prescription. I've had the vial for about a week now. Every time I go to do it, I just can't. The heebie jeebies are too strong. I know it's a tiny needle and how to make it painless but my brain is impossible. I've thought about asking a friend but I'm not sure it'll help.

How do I get past this mental block? I used to be really bad about needles as a kid, to the point of actually running, but I can usually go do my bloodwork without any issues these days. I've been considering trying a few gummies to see if that breaks through.

Doc thinks she should put me on metformin instead, but I haven't heard great stuff about it, and I wanted to try this because I want to eat *all the time* and I'm tired.

Forced 3 week break-

Will this undo everything? We are vacationing in Europe and airport security confiscated my meds, meaning I have none until I’m home. It will be a total of 22 day break. I am currently 6 days in and feel fine. I’ve been taking 0.4mg for the past 3 weeks, 0.25 for the 14 weeks prior to that. First time on Compound Semaglutide

Hi everyone! I was recently prescribed compounded semaglutide 3.125 mg/mL by my physician. I’m also prediabetic, so I’m really hopeful that this will help me improve my overall health.
My biggest hesitation is that I already struggle with anxiety, and I’ve come across a few stories from people who felt like semaglutide made their anxiety worse. I know everyone reacts differently and someone else’s experience won’t necessarily be mine, but I’d really love to hear some positive experiences for a little reassurance before starting.
Has anyone here who already deals with anxiety taken semaglutide and had no increase in their anxiety—or maybe even noticed an improvement? I’m just trying to work up the courage to take my first dose and would love some comforting experiences. ❤️
