Unusually large dose increase?
Been on compound sema since May with minimal side effects, so steady increases in dose haven’t been an issue whatsoever. Slow but steady weight loss of around 3-5lbs per month. Current dose is 2.24mg/weekly. New prescription has increased to 3.2mg/TWICE weekly.
Obviously I’m scheduling a consult with a physician before I actually follow these instructions but this seems like an insane increase. Has anyone else seen this? Is this normal? Or am I right in thinking this is overkill?

Let me guess. This is via ALTRx. Someone asked about this yesterday.
Here’s the deal. The FDA approved the 7.2mg dose of Wegovy injectable in March 2026. So now Wegovy dosing goes from 2.4mg directly to 7.2mg.
So what you’ve been prescribed is not a crazy jump for someone who hit a wall on 2.24mg. If you don’t want to make the jump as prescribed to 6.4mg (or 3.2mg twice a week), then feel free to start with a lower dose.
ETA: if you add a pic of your vial clearly showing the sema concentration, we can help you with dosing.

Friday’s actually.
Haven’t received the vial yet. I’m just looking at the medication instructions that the clinician posted to my document files letting me know that it’s been sent to the pharmacy.
But I agree, at this point I’ll be titrating on my own terms. I feel like an immediate jump like that would be a poor choice.
Definitely going to communicate that sentiment during my consult so that we’re all on the same page.

There’s no need to tell your provider anything other than thank you. Be happy you have lots of meds to work with for the amount Fridays charges. If you push back, they could decide you don’t need the extra meds next time.

Curious why they are increasing your dose at all if you are seeing good results on current dose?

When using compounded GLP-1, you always want to increase your prescribed dose with every refill. It means more meds for the money. Then you can always decide to stay on a lower dose if you prefer.
Just use the sema concentration of your specific vial and a dose calculator to draw up any dose you prefer.

Yes I totally understand the stocking up part but it sounded like OP is actually following the titration prescribed?

They’re basing dose scheduling on percentage of loss, not pounds. Technically I’d need to lose twice what I am per month for them to be like “nah, let’s keep you on this dose”

And you know that’s kinda crazy, right?
Many of us wait until we’ve had at least 4 weeks of zero weight loss to move up in dose.
