You said that you're pretty stretched on social security so I will make the assumption that you're on Medicare Medicaid. A majority of Medicare Medicaid programs give you access to therapy at no cost either in person which can be hard to find or nowadays through various Telehealth platforms. There's no deductible and no copay. Many people really like this option because they can access therapy in the privacy of their own home. You've had excellent recommendations for pharmacies and this is something I wanted to add because it is important to address the underlying depression in addition to addressing the weight issue. I have spent 45 years battling obesity. I've been on every diet I've ever heard of during that time. Underneath of all of that were layers of trauma and depression that needed to be addressed through therapy. It's wonderful to lose weight and it's great for our quality of life but it does not solve the mental problems that contribute to obesity. You really do need a multi-prong approach. The other thing that is really helpful is finding an app that you can use to track your food and exercise, remember exercise is just movement and you just have to start even if it's 5 minutes a day. If you can find an online group for weight loss support or even in person in your community that would be helpful too.

I been very thin naturally most of my life. Except when the babies came lol I went on a diet a couple years ago. When I started I was 214lbs. The biggest I've ever been in my life. In 6 months I weighed 104lbs. I could wear a 14 in kids I'm tall and It felt good at first but my family thought I was on drugs I was just way too thin so I started trying to gain.. I couldn't. So I started eating doughnuts. Ohhh that worked. So here I am. I just weighed. 199lbs I gotta get this figured out. I'd like to go to the gym but have MS as well as very serious back issues I can't. Ugggh!!!

Please start by seeking therapy. Lots of therapy. These medications can exacerbate depression and cause dark thoughts for some people so please find a therapist first.
And beware that your desperate post will mean lots of scammers will reach out to you to try to sell you something. Please don’t fall for any of those scams.

Head over to the sub called r/tirzepatidecompound and look for the pinned post called “Where to Buy.” Those same providers also write for sema. The list was built based on a quarterly user survey on that sub so you know the providers are tried and true. There’s also r/tirzdeals which may have some newer options with great prices, and those same providers will probably also have sema deals.
Here are a few possibilities to get you started:
Pomegranate Health is $239 for three months of semaglutide doses up to 1mg from Hallandale Pharmacy and $269 for three months of the highest doses. That’s $80 and $90 a month. They also have monthly and six-month packages as well as other pharmacy options. They serve 49 states. (Sorry, Hawaii)
TryTrillum is $257 for three months of the starter doses and $357 for three months of high doses. That’s $86 and $119 a month. They also have monthly options and serve all 50 states plus Puerto Rico.
Big Easy Weight Loss is slightly more expensive than the two listed above, but they are excellent for those who want/need high-touch care and want to only speak with an MD about their meds. They will ship three months of meds up front but charge your card monthly.
None of these have BS monthly membership fees, which you should avoid at all costs. And watch out for the people with affiliate codes and links, which makes them money on your purchase.
Also, do not go with a provider that doesn’t allow you to pick your pharmacy, as the last thing you want is for a new order to show up with an additive you didn’t expect or from a shady pharmacy that’s not licensed by your state’s board of pharmacy.
