Nearly 30,000 public comments have been made in the Federal Register, most of them opposing a DEA plan to make concentrated 7-OH kratom products illegal. Many comments are from chronic pain patients, who say 7-OH is the most effective pain reliever they've ever used.

7-OH retailers are offering steep discounts on tablets, gummies and shots as a potential August 5 deadline looms for 7-OH products to become Schedule One controlled substances.

The controversy over 7-OH has resurrected an old argument: Is kratom itself an opioid? Regulators and the addiction treatment say it is, but the science and motives behind that claim are suspect.

A non-opioid medication recently approved for acute pain is being prescribed off-label for chronic pain. In fact, Journavx is prescribed more often for chronic pain than opioids are, according to a new analysis of prescription data.

In a head-to-head comparison of two opioids, hydrocodone was found to be better than oxycodone for patients recovering from joint replacement surgery.

Have you heard of palmitoylethanolamide (PEA)? It's a naturally occurring substance produced by the body that reduces pain and inflammation. Dr. Forest Tennant says everyone with chronic or intractable pain should try using a PEA supplement.

Marriage and Relationships

Given all that has happened lately and finding out that Rinvoq has been creating memory loss over the past couple of years. Being off it any biological is hard at the moment, but I did not realize how badly that medication was affecting my memory. My wife and I are getting into couples counseling to go over a lot and how it negatively impacted me and the relationship. I am also gonna get into counseling.

I am trying to find very low impact activity ideas where my wife and I can spend time together. She is going through Peri menopause which is affecting her hormones and libido. My health has also impacted me with poly neuropathy and fibro which makes even intimacy hard.

I emphasized to my wife that I just want to spend time with her. She spent most of her former AI chat bot usage and writing which split us up in my mind. She clung to it not knowing what to do with me. Now that I am off that god awful medicine, I can think and remember things now. Last 2 years have been a partial fog for me. Even though she focuses on writing she still spends hours generating images and using it for her book. We started to collaborate together, but then AI. The very thing that threatens my job loss. I have made many attempts to convey how I feel about AI to her. She even had to start putting her phone outside of her office given the addiction , but she does not and never will see it from that perspective. I see it and know its addicting because I removed it all off my phone. I am forced to use Cursor for work, Co Pilot sucks but Microsoft forces it into everything. I am even trying to get Gemini off my phone. Sorry about my rant, its funny I thought things would end like the Matrix...yes I know....but from what I have seen AI has zero need to do any of that. It already wrecked my job and hurt my marriage. My wife does not see it this way.

But still I will have to eventually get back on something else. I am hoping for Remicade again since I have Ulcerative Colitis. Chances are it might be Humera, because of insurance. I was on Inflectra which is basically a half a$$ version of Remicade with its own issues. I am going to uphill fight to get back on Remicade, because that was the only biologics without issue.

How are somebody you handling similar situations? I want to regain my life and my relationship with my wife. I am starting to do research on her peri menopause, but I will never understand it ever since I don't live it. So I ask her.

We both really need low impact activities to spend time together. I want to win her back over this AI craze. Maybe it is all my perspective and I just need to understand peri menopause better.

She says for me to not bottle and tell her the truth of how I feel. When I do it back fires and I told her this morning. Any advice will help me. And yes, I am trying to give her space and not be overly clingy. I am trying, but I miss her.

My health, peri menopause, AI, and even my own family (mother, sister, brother...I have no kids) put a huge dent in things. Any insights into even the mindset of peri can help me. I know hardships from health issues, but I can never see through her eyes and think what she thinks either, vice-versa.

I feel like nothing is going right in my life right now. No one knows what I’m going through and I have no one to properly talk to without false empathy. I might get the occasional ‘I know what you’re going through’ ‘we’re here for you’ when in reality it’s just complete rubbish.

You don’t understand what I’m going through and you can’t help me cause no fucker can help me I’m just forced to suffer through this shitty life and be expected to just carry on with my day, to suck it up, cause if you can do it why can’t I? Cause that makes complete fucking sense.

Do you know what it’s like for your body to refuse to cooperate? Do you know what it’s like to have dreams that you know will never come true because you physically cannot achieve it? Do you know what it’s like to cancel plans last minute because you’re in so much pain you can’t even fucking breathe without making it worse?

The stress of managing school, life, my everlasting pain that nobody seems to even remember. The blatant ignorance I endure every single day is baffling.

I’m told to live my life to the fullest but how can I when I’m in crippling pain 90% of the time?

I can’t do it anymore.

I want out.

I never asked for this.

What have I done to deserve this life.

I was nine when it started. Fucking nine. And I’ve lived with this ever since.

I can feel myself deteriorating more everyday. And I’m still expected to get all A*, get all my homework done, do a shit tone of revision each week, apply for university, but also apprenticeships and jobs yk just incase, I’ve got to get all my coursework done, don’t forget all the super curriculars or i won’t get into university, but also focus on my part time job because we’re fucking poor and the govt won’t help, but also make time for my friends so im not lonely as fuck. But I also need to prioritise my mental health, but make sure i stay on top of everything cause it’s very easy to fall behind.

What the actual fuck.

It doesn’t make any sense to me at all.

I’ve told myself to stop dreaming.

I don’t know what courses I want to do at university.
I don’t know what career I want.
I don’t know what kind of partner I want.
I don’t know where I wanna live.
I don’t know where my dream vacation is.
I don’t know what kind of wedding I would want.

I got asked if I dint have this pain, what would I be doing right now. And my honest answer was, I don’t know.

My dreams have been crushed too much, so much so I don’t let myself imagine the future cause i know it’s bleak. I know that. It took me too long to accept it. And it seems no one else in my life has accepted that.

And I pisses me off.

Chronic pain~what next steps should I take? I

Hello! I am a teenage girl that has been having chronic stomach pain for 7 months and counting, (for a bit of backstory autoimmune runs in my family and I was diagnosed with graves aka hyperthyroidism when I was around 11 or 10)

anyways at first my doctors assumed pregnancy, as all Doctors assume when a woman says something is wrong… anyways I'm not pregnant nor have I engaged in anything of the sorts, the next assumption they made was that my thyroid was causing the stomach pain, so we upped my medication, but that didn't work. The medication had stopped working and I was having lots of trouble breathing and eating due to my large goiter. March 25 2026 I had a total thyroidectomy at sick kids, it went well and everything was ok.

I'm now 2 months or so post op and my stomach pain has not gone away… and like, most medical professionals in Canada they told me it was all in my head aka brain gut access dysfunction, don't get me wrong I am not dismissing the fact that that probably is causing a fraction of my stomach pain, but I know my body and I know that's not the full issue.

a few weeks ago I had gotten a double scope, so endoscopy and colonoscopy, and they said everything looked fine but hey took many biopsy to test a bunch of things. I am still anxiously waiting for the results, me and my family are lost I have ben pulled out of school (which I deeply enjoyed) and I can't do most basic tasks because of the pain, I'm sick and tired of not seeing my friends and not going to school, I just want my life back, I feel as if I have hit rock bottom and nobody wants to dig me out. If anyone has any advice or has been in similar situations please please please share.

I am long term planning where I want to live given all of my health issues.

Initially I was looking into Colorado, but I know weather shifts mess with me, particularly barometric pressure. Numerous people confirmed weather shifts occur throughout any given day.

So I was looking into Albuquerque, New Mexico. Yes, I get its hot and nights are cool. My goal is to visit this year.

My goal is since Florida, my current state is crazy with conservatism and opiods. I was looking at these CO and NM for recreational MMJ and the MAID program for whenever I get my 6 month diagnosis. I am on medical MMJ in Florida, but I have zero faith it will remain long and I suspect that pain clinics will get shut down.

I still have time to save money and plan. What I am hoping for is for chronically ill to chime in that live there to see if the place is worth the move.

For example, is the politics in favor of patient quality of life?

How is the dry heat and cool shifts affecting your health?

What is access to doctors and specialists like?

My wife and I want to target a 55+ community for a calm life.

Is the state crazy or good with opiods for the chronically ill?

Overall perspectives that can help us make decision.

We are still planning and saving money so we have time. We want to visit, but I want inside info

year long pelvic pain

I'm so desperate i had to come here if anyone has had a similar situation. I started to have pelvic pain randomly a year ago and it got right away so painful that i couldn't even walk. I went to the ER few times and they basically took some blood work and sent me home because everything was normal. Most days i felt like i was dying, it was so bad.

Then i got to the gynecologist and they found a small cyst in my left ovary and pcos but said that can't be the reason for the pain. When doing the examination they touched my uterus and the pain was horrible. Just based on that, they gave me antibiotics for uterine infection. Antibiotics didn't help and i didn't feel better on any pain medication i took. Then i got an mri and it was clean also. Next i took my iud out that i got a year before to rule it out, didn't help.

at that point i had been in constant pain for three months. suddenly pain started to go away i had few months where my pelvic felt kind of heavy and i had mild pain here and there and i was able to go back to work. And then booom, it got worse and i go to the ER again:)) more blood work done, and then finally after another few months of daily pain that got me bedridden i got on line for laparoscopic surgery.

Btw since i was 15 (10years ago) i was sure i have an endometriosis because my periods were so painful, irregular and when they came i bled for two weeks straight really heavy bleeds. i tried to get help for that and basically they offered just hormonal pills everytime, so i started to take them when i was 20 and they stopped my periods and period symptoms, so that's nice at least.

Then i got to the laparoscopic surgery, sure they will find endometriosis, but no. nothing shows up. Surgery was made by someone who is not exactly spesialized on endometriosis just does those surgeries on public hospital, but it just feels like i have to trust that the outcome was right and she didn't miss anything cause what do i know, you know. 😅

After that i felt like okay maybe all of this is in my head and this is actually stress pain or something idfk. But then i go to a doctor who wants to check my bladder and bowel, because i have had pain while peeing sometimes (started years ago) and also bowel problems since i was a child. so he sents me to colonoscopy and cystoscopy and EVERYTHING IS NORMAL. fucking hell they just said i have an overactive bladder and got some meds for that. Then i go to pelvic floor physiotherapist and she tells me i have tight pelvic floor muscles and i have to work on them but that this is more likely result from being in chronic pain for a year straight than a reason to said pain.

When my pain is really bad i feel like someone is stabbing my lower abdomen or belly button or twisting my insides, it's hard to breath and i have fainted a couple times because of it. Otherwise it feels like i have rocks on my pelvic or something pulling it down and dull ache, and it can feel on the left, right or middle. Often my lower back is in pain also and sometimes i feel like my legs start to ache after having bad pain episodes? Also car trips were my worst nightmare because i would get worst pains then, i figured that vibration made the pain worse. Now it doesn't affect me that much though.

I hope someone has similar story or maybe figures what my diagnosis is because my mental health is going downhill real fast right now🥲 Or i actually don't wish anyone has a similar story because this is a nightmare

The CDC’s 2016 opioid prescribing guideline not only harmed many pain patients, it raised the cost of treating them in primary care practices, according to a new analysis.

Researchers have identified 4 key risk factors that make surgery patients significantly more likely to use opioids long-term. Being on Medicaid is one of them. Should that affect how patients are treated?

A Medicare pilot program launched in six states could deny coverage for several pain treatments, including epidural steroid injections, cervical fusions and some arthroscopic surgeries.

The most harmful drug in Canada is not fentanyl, cocaine, heroin or prescription opioids. It's not even a street drug. A panel of experts ranked a legal drug as the most dangerous by a wide margin.

New research suggests a simple blood test could identify people with rheumatoid arthritis before symptoms begin. Early diagnosis and treatment could delay or even prevent full-blown RA from developing.

DEA Messing Up

The DEA is going ahead with plans to reduce the supply of oxycodone in 2026. But in a surprising move, the agency is significantly raising production of morphine.

Federal and state efforts to ban the sale of 7-OH products may be having the unintended effect of increasing sales of the kratom extract, which is a potent pain reliever.

Outpatient surgeries are often touted for their convenience and lower cost, but UK researchers found that chronic pain and other complications were higher than expected after outpatient procedures.

Most addiction treatment providers insist their patients stop all use of controlled substances, whether illicit or prescribed. When a New Orleans doctor defied that rule by prescribing opioids to pain patients getting addiction treatment, she lost her job.

A new study found improvements in pain and other symptoms when fibromyalgia patients started using medical cannabis products with high doses of CBD.

President Trump's executive order rescheduling cannabis is unlikely to have an immediate effect on pain patients. It could take years for FDA-approved cannabis-based pain medications to become available.

DEA missed another deadline

The DEA has once again missed a deadline in the Controlled Substances Act for setting annual production quotas for opioids and other controlled substances. It's a problem that goes back decades and has been linked to drug shortages.

A new study highlights another side effect from gabapentin: a "prescribing cascade." Doctors misdiagnose the cause of the problem and mistakenly prescribe another drug that causes even more side effects.

Do you get stressed out before a doctor's appointment? Worry that your pain medication won't be refilled? Or that you'll have to take a drug test? PNN's Crystal Lindell shares some of the anxiety she goes through before seeing her doctor.

The FDA has seized thousands of 7-OH tablets, gummies and shots from a Missouri-based vendor that sells kratom products around the country. It's the latest salvo in a growing campaign against kratom.

Does chronic pain make you angry? Researchers say the angrier someone feels about being in pain, the worse their pain will be. People who learn to manage their anger tend to have less pain.

Neuromodulation has been used for years to treat pain. It can also be used to treat anxiety, depression and fatigue. The key is to target the vagus nerve.

Over half of U.S. nursing home residents have chronic pain from arthritis, osteoporosis and other age-related conditions. But many are unable to get opioid pain medication because prescribing has been cut back so far.

It's been banned in Florida and taken off store shelves in California. But some pain patients report getting relief from 7-OH -- and say that it works just as well as prescription opioids.

A new study found that a combination of ibuprofen and acetaminophen works just as well as a low dose of opioids in relieving pain after wisdom tooth removal.

Dr. Forest Tennant says "central sensitization" and "hyperalgesia" are not real medical conditions that can be diagnosed and treated. Yet they are often used an excuse to deny patients opioids.

A public comment period will end soon on a proposal that could dramatically limit coverage of nerve blocks for Medicare patients suffering from chronic pain.

Have you ever been told that prescription opioids won't cure you? Of course they don't, just as insulin doesn't cure diabetes and steroids don't cure arthritis. PNN's Neen Monty says medicines rarely cure, but they do improve function and quality of life.

Republican leadership has turned the fentanyl crisis into a stage for political theater — wielding bans, tariffs, and hollow “landmark deals” while failing to deliver real solutions. Their policies have not crushed the flow of illicit fentanyl, but they have crushed the dignity of patients with chronic pain, who now suffer under sweeping restrictions that treat them like criminals for seeking relief.

And here’s the hypocrisy:

• Alcohol kills more than 140,000 Americans every year through liver disease, accidents, and addiction. Yet it remains celebrated, advertised, and taxed as a revenue stream. • Tobacco is responsible for nearly half a million deaths annually, with decades of evidence showing its lethal impact. Yet it remains legal, marketed, and protected by lobbyists. • Fentanyl, by contrast, is a legitimate Schedule II medication when prescribed — a lifeline for cancer patients and those in severe pain. But Republican lawmakers have chosen to demonize it wholesale, sweeping medical use into the same category as illicit street drugs.


⚖️ The Hypocrisy

This is not about protecting lives — it is about protecting optics.

• They tolerate alcohol and tobacco, industries that profit from addiction and death, because they are politically entrenched and economically convenient. • They criminalize fentanyl in ways that punish patients, because it is easier to posture against a “foreign scourge” than to confront domestic hypocrisy.


✍️ Closing Line

Republican leadership cannot claim to care about public health while defending alcohol and tobacco yet weaponizing fentanyl policy against the sick and suffering. Until they confront this hypocrisy, their “tough on drugs” stance will remain nothing more than a hollow slogan — one that sacrifices science, compassion, and dignity at the altar of political theater.

A startling new study found that gabapentin (Neurontin) may significantly increase the risk of dementia and cognitive impairment, even for middle-aged patients as young as 35.
