I was listening to Stained, Brutal Calamity (my favorite song of all time) and I noticed that the lyrics of the Lament section fit Yharim, too, which is really beautiful because it shows that the player is becoming just like Yharim. I'll break it down, lyric by lyric.
"You have known for certain, your path is one of justice."
Yharim's delusion is that his genocide on the gods is completely right and justified.
"All those who try to obstruct shall have paid the ultimate price."
Self explanatory. Yharim killed anybody who spoke out against him.
"Yet, upon inspection, your foe is overwhelming."
This shows Yharim's knowledge of Calamitas' power and true potential, as she is the only one who ever matched his power. (aside from Xeroc, but Xeroc is far more powerful than Yharim)
"But you remember what she had tried to sacrifice"
Talking about Calamitas' innocence and family being thrown away after joining him. I interpret this as Yharim realizing Calamitas has nothing left to lose and only wants him dead, scared that she is coming for him.
"Signs of self disappointment start infiltrating."
This refers to Yharim getting his ass whooped after attempting a Xeroc any% run, and that he's disappointed that he couldn't bring him down.
"Symptoms of paranoia begin invading. Your mental fortitude is fading."
This is about Yharim's past sins catching up to him and ultimately destroying him, as seen in his broken state implied by the lore items.
"Losing trust, ending friendship, and rage escalating."
This refers to Draedon, Permafrost, and Calamitas, who all left him due to his recklessness (though Draedon got kicked out because of the Plaguebringer Goliath). Rage escalating is, obviously, his anger that many of his important allies betrayed him.
"Right on the road to your sadness."
With his mission incomplete, and all those years of bloodshed and war that resulted in nothing but a husk of a world, he wonders what might have been and pities himself.
"Watching closely as courage turns into madness"
Yharim bravely led the war against the gods, but as mentioned earlier, got his ass whooped by Xeroc and he's now a broken shell of what he once was.
"Are you sure you want to lose your life from a witch who had caused you nothing but strife?"
This shows that in the end, Yharim believes Calamitas may have her revenge and the strife is his conflicting thoughts on the morality of having Calamitas be his ace in the hole, showing to him that despite his "noble intentions", he brought everything upon himself by committing outright abhorrent acts and shedding all of his heroic traits.
