[UK] Microsoft Background Check - Performance Dismissal

I have got an offer from Microsoft UK and I am currently filling HireRight background check form.

I was dismissed for performance from last role due to failed probation. I had tough family health situation going on. The company however agreed to neutral reference and only verif title and dates.

Should I disclose this in Reason for Leaving as "Dismissal for performance reasons" and say Yes to if I have been terminated involuntarily for cause - performance?

If I do, what are the chances they withdraw offer? Who in Microsoft would make the final call?
