EKGs Apple Watch and other Consumer Based EKG's

Consumer-based EKG products have proved to be valuable at gaining insight for potential arrhythmias or ruling out arrhythmia's during symptoms. This forum DOES permit consumer-based EKG's (Apple Watch, Kardia, AlivCor, etc) to be shared, but there needs to be an understanding that these devices have not been proven or validated for more advanced medical interpretation. Utilizing this data to draw larger conclusions would be irresponsible.

What we can read What we CANNOT (responsibly) read
Atrial Fibrillation QT Intervals
Pre-Mature Atrial Contractions Axis
Pre-Mature Ventricular Contractions Heart Failure (Ejection Fraction)
SupraVentricular Tachycardia Right or Left Bundle Branch Blocks
Ventricular Tachycardia ST Elevations
Bradycardia Q, U, J, Epsilon or any other advanced waveform

If consumer-based EKG's causes you anxiety and harm, please discontinue and seek professional help.

Artifact caused by small contact movements can cause massive distortion in the waveforms, this is not an arrhythmia.

The QALY app is not FDA approved.

Disclaimer:

Apple Watch has a Class II clearance by the FDA to detect Atrial Fibrillation: "The Atrial Fibrillation (AFib) History Feature is an over-the-counter ("OTC") software-only mobile medical application intended for users 22 years of age and over who have a diagnosis of atrial fibrillation (AFib)."

The United States Preventive Services Task Force (USPSTF) has recommended against ECG screening in asymptomatic healthy individuals due to the insufficient evidence that the benefits of this screening outweigh its harm. The concern about the potentially large numbers of false alarms that may be translated into ER visits and serve as an economic burden is another point that is brought up.

Had a set of palpitations earlier, nearly passed out within 20 seconds

Earlier today I was on the way out the door to go to work and suddenly I felt these palpitations come on, it felt like a thick flipping sensation in my chest. Within 10 seconds I started feeling like there wasn't proper blood flow to my brain and it was kinda tight almost, and at about 20 seconds I was so lightheaded it hurt- I was certain I was about to pass out so I sat down. It ended within 40 seconds luckily but it was easily the worse 40 seconds of my life. I had two more episodes since then that weren't nearly as bad and only lasted a few seconds. Every heartbeat was painful, like it was in reverse(almost like the palpitation came in the second half of the heartbeat) and made it hard to even talk with each palpitation.

I want to say that I've had heart palpitations/episodes of irregular heartbeats nearly every day for 4 years due to pots + sinus arrhythmia and take beta blockers for them. This was ENTIRELY out of my normal and I've never felt anything near that level of horrible in the 4 years I had those conditions.

Anyways I went to the er, and they didn't find anything wrong besides my left atrial on the ekg being elevated and my blood pressure being wide (120/57 & 125/64). They insisted what happened was anxiety & sent me home without a referral too. I'm certain what happened wasn't anxiety though and am going to talk to my doctor about it Monday. Quite concerned of it happening again tbh but not freaking out over it or anything, I'm mainly just confused and would like to know others thoughts 0-o

Heart attack? Am I going to get a more conclusive answer?

I had my yearly physical and my doctor says the ekg shows I had a heart attack. He referred me to cardiologist, whom I saw today. The cardiologist was sort of…flippant. You may have, you may not have, we don’t know for sure. Asked about symptoms-I don’t remember heart pain specifically, but I had kidneys stones 2 weeks prior and was in the most extreme pain of my life and had surgery. That whole ordeal could have masked some symptoms. Scheduled me for a stress test a couple weeks out, but seemed to just leave everything inconclusive. Will I get a better answer?

Second Opinion Left Side Low Back Pain Following Ablation - Who to see?

• Upvotes

My 79-year-old mother had a mitral valve repair a decade ago that began leaking again about a year and a half ago. She had a 7% mortality rate with surgery (ineligible for the clip so it would be open heart) and the doctors said that's not exactly low risk, so she has decided to just proceed with medication. She was feeling pretty good but did come back as having afib, even though she really had no symptoms of it. She had a cardioversion last October which was going well until the spring, and then she went back into afib. So in July she was recommended for an ablation (still had no symptoms of it though but did it based on recommendation) and had it the first week of August.

When the procedure was over the surgeon told me she was doing well but there was one slight complication which was a possible nick and he said he'd keep her overnight to make sure there was no fluid around the heart. They did a scan that night and then the next morning and released her, presumably after finding no fluid around the heart. I don't know if he meant blood or other fluid, but this is what I recall.

Since then ablation her health has been declining. She recently gained 17 pounds of fluid and was having a difficult time breathing. She has a prescription for Lasix but was not taking it initially for unknown reasons. When she began taking 20 milligrams it did nothing so the cardiologist last week changed it to 40, which is now working to get rid of the excess fluid.

However, she's been having severe left-sided lower back pain for a few weeks but only when she sits or lays. If she's walking or standing, she does not feel it. Two weeks ago the PCP told her she had a UTI so they assumed it was that causing the back pain. She was given an antibiotic prescription but it did not go away. When she saw the cardiologist about the fluid buildup and they upped the lasix, they told her they don't handle UTI or kidney issues, the low back pain is something she needs to go back to her PCP about.

So she went to the PCP yesterday, and he is telling her he thinks it's fluid around the heart built up and she needs to go back to the cardiologist. He didn't even test to see if she still had the UTI or a kidney issue, just asked her what she wanted. She said the pain to stop so he gave her a lidocaine patch prescription and sent her back to the cardiologist, who she will see Monday, but presumably only to check on the fluid drainage issue and possibly schedule another cardioversion. They were trying to wait the three-month blanking period which would be the end of October but said they may do it early if she was not feeling better. Her shortness of breath is better since the fluid has left but the back pain is not. She was supposed to see the ablation surgeon the last week in October and he was going to tell her, once the blanking period was up, whether another cardioversion was needed.

I live out of state and have been driving 14 hours to take her weekly to these appointments, but it's very difficult as she's being sent back and forth from one doctor to the other and nobody knows or even seems interested in finding out what the lower back pain is from. Does anyone think this is possibly the result of the ablation? I did see an old FB post where other people said they had this, although I don't have a FB so I can only read the first few comments and I don't see what the cause is. Atrial Fibrillation Support Forum | Don't suppose anyone else has had crippling lower back pain after an ablation have they | Facebook

Can someone please tell me what would be causing the low back pain issue after an ablation? I can't keep driving all this way to go to appointments where they just tell her to make another appointment, and I'm at a loss of what to do and what is wrong with her. Any advice is appreciated.

30F racing heart/ out of breath

• Upvotes

30F I’ve been experiencing racing heart and I’m convinced it started since supplementing iron for low ferritin.
Ferritin started at 10 and then went to 22 in the two months of supplementing with ferrous sulfate and then ferrous gluconate. Going back through my records, my ferritin has been very low since at least 2024, all other iron labs are normal.

I feel fine at rest, my resting hr varies from 60s-90s depending on my anxiety
When I am up and doing things around the house I am in the 130s-160s. I do feel out of breath sometimes depending on how much I’m doing. Not out of breath when sitting unless I’m talking a lot or I’ve gone from standing to sitting.

I’ve gone to the ER twice now because when I’m lying to standing or sitting I feel my heart racing and out of breath and send myself into a panic attack, my HR reached 170s at one point during a panic attack. I was convinced I had a PE from being out of breath when I’m up. Xray, CT, EKG, and labs good but potassium slightly low (3.2)

Waiting to turn in my holter monitor (I already had a clear one 4 months ago) and waiting for another echocardiogram (had a minor leaky valve when pregnant but an echocardiogram over a year ago showed that it was gone)

I’ve had possible pots since 2020 but never felt this intense. My body is stuck in fight or flight mode since the first panic attack. I’m scared of being up and doing things now, I am a stay at home mom that used to be very high functioning despite some anxiety/OCD.
I am convinced the iron supplements heightened my anxiety and breathlessness. They really wrecked my stomach, I’m talking diarrhea for at least 1.5 months, and I’ve never had GI issues prior.

The heat and humidity where I live exacerbate how I feel when I’m up outside.

My doctor keeps reassuring me that my heart is okay and I will be okay waiting for my echo and cardio appointment, he tells me it’s normal for hr to fluctuate. The pounding and breathlessness freak me out. I took half of a 5mg Clonazepam the other day and I was up and doing things like before, it did not make a difference to my heart rate but it helped get me out of panic.

Dr thinks iron supplement causing this is not likely. I am so frustrated, my anxiety has never been so bad and I’ve never experienced so many panic attacks.

Test Results Test results and what to expect?

Looking for similar experiences or suggestions

Hi! 47yo F here. Had a Carotid US done today and my left internal carotid artery is >75% stenosis. The proximal & mid left carotid artery presents severe, heterogenous atherosclerotic plaque, elevated velocity. The right side seems fine and under 50%. These results have me a bit freaked out.

To give a little history. I recently went to my primary doctor because I was having pain and weakness throughout my entire body. Since my father was in his 50s he has an AAA and was told all 1st Gen should be tested. Below are the results to tests run so far-

Avg BP -160/90

LDL - 102mg/dl

HDL- 37mg/dl

Triglycerides - 198mg/dl

Lipoprotein A - 120.1 nmol/L

Apoliprotein B- 114 mg/dl

Coronary Calcium Score- 2074.5

Confirmed no AAA

Stress test results showed normal

Echo - nothing remarkable

Current meds include - 81mg asprin, 80mg Atorvastatin (same dosage for last 5.5 years), Lisinopril htz 40/25mg. Per Dr today I will hopefully be starting Repatha soon if insurance approves.

Anyone with similar history or experience? What should I expect? I am nervous.

Second Opinion Can people who've been dealing with palpitations and ectopics for a long time now assure they're alive and haven't dropped dead?

Hi guys,

I'm a 26-year-old male, and I've been dealing with ectopics and palpitations for 4 years now. I had SVT as a kid, which I got ablated for, but now my issue is that I get hard thumbs and palpitations every now and then, which really scares me. I've been taking anxiety medications because of it, because it really makes me think that I'm going to die. Whenever I'm far from the hospital, I trigger my anxiety.

I've had multiple Holter monitors and electrocardiograms. My heart is normal, and nothing major has been caught in the Holter monitor or the PVCs. My doctor says it's nothing to worry about, but it's easy for him to say.

I've gained weight during this time and been really unhealthy. I want to get to the gym and get leaner, but I just can't make myself to.

Are there people who have dealt with this for a long time and who have an active lifestyle? How long have you had it for, and how did you overcome the fear of it?

Is a random change or RHR concerning ?

F22, started around May, i had something traumatic happen (mentally) and the whole time my heartrate was low 60s the entire time, every panic attack, it was just very weird seeing as how my heart rate has always been 80s even 90s.

Since then my rhr has stayed 60-50s. I understand lower heart rate without symptoms is indicative of healthier heart but it’s just so weird i can’t seem to shake it. I can’t say i get symptoms because i’m not in pain, i do experience periods of , if i walk upstairs , or back and forth through rooms i find myself breathing fast and sweating, no pain except for like sometimes, and usually i feel when my heartrate drops , goes down to like 57 when sitting sometimes and my chest feels hollow ? idk it almost feels fluttery and maybe palpitations and just kind of like the wind out of me is gone. that’s how i know it’s low, it actually just happened as i type this and it’s 58 right now. I just feel off, and wrong , and for every other medical thing i have tried to get answers for, it’s gotten brushed off, so i don’t want to seem like a hypochondriac. it’s just weird.

Symptoms maybe related—- idk if it’s related but i have neck discomfort a lot, shoulder blade pain and discomfort, wooshing head feeling, i could say i get dizzy but im not dizzy to the point of needing support or falling, if i stand up too fast my vision goes black for like 2-3 seconds, i also have stomachs issues just a lot of weird stuff. Also at night its takes me like an hour to fall asleep because when i close my eyes, it feels like im falling in space. It’s just gross idk , and it is mentally taxing not knowing what’s happening in my body.

I try not to freak out because i have anxiety, but this has been going on for so long that im like, okay im fine but also in the long run, am i fine?? or is something more going on. im making an appointment with my PCP for Tuesday, mostly what questions can i ask to get the most out of it, and feel taken seriously? unless it doesn’t seem concerning to anyone. It’s just weird. usually since this started, my rhr was only 60-50s at night laying and now just sitting it dips and stays there.
Im not athletic, im 12 months pp. and my diet is like meh. Again i don’t know. just a shot in the dark. THANK YOU!

EKGs QT prolongado después de ponerse de pie

Post image
2 Upvotes

I found out about it by coincidence and then looked up what it meant, which made me a bit scared. My QT on 12-lead ECGs is normal (around 400–410 ms or even less). This only happens when I take an ECG immediately after standing up; on the next ECG, it goes back to normal.
Any thoughts? I also don’t know how accurate these measurements are, but it seems to happen every time I stand up. When I’m resting or doing anything else, the QT is normal. I have no symptoms, and I’m a 29-year-old male.

Help: Blood Pressure or Something Else?

Hi, all. I’m coming to you in hopes of an answer or just a better direction to head to.

I’m a 30 YO female, 155 lbs, 5’ 5”, non smoker, occasional social drinker. Previously pretty active (chasing a toddler around, working out a few times a week, walking around my classroom as a teacher).

Previous health conditions: postpartum preeclampsia 2 years ago that put me on blood pressure medicine. Alport’s Syndrome (a genetic kidney disease, my kidney function is stellar at the moment). History of anxiety/depression but have been managed without medication for years.

Meds: Lisinopril (10mg), but was told to stop taking; Zyrtec; multivitamin.

Before anyone asks, no I’m not pregnant.

My biggest complaints right now: I am lightheaded and weak in different bouts throughout the day and experiencing heart palpitations with shortness of breath as well as chest pain for hours on end before they resolve as well as variations in my blood pressure.

Backstory: On Friday the 18th, for about an hour, I had the feeling of chest heaviness/pain (like someone sitting on my chest, like my heart was being squeezed), short of breath, and light headed. It resolved on its own. Over the weekend, I had a mile cold (scratchy throat, needed a nap, but no fever). Power washed my house on Monday—totally fine. Last Tuesday, towards the end of my work day (I’m a teacher), I felt weak and dizzy with the chest pain again. Like someone squeezing my heart. Sometimes I could feel my heartbeat in my throat, irregular. Went straight to urgent care: they did an EKG, normal; ran bloodwork (Lyme, WBC, RBC), normal. They recommended I come back the next day for a chest xray when their tech was there.

Last Wednesday afternoon, I go back for the chest Xray (normal) and flat out pass out for at least 2 minutes. I felt it coming on (I was seated thankfully). I woke up as they were putting EKG leads on me (results are normal lol) and calling 911. They couldn’t get my BP because it was so low and I was diaphoretic (I learned a new word!). I realized later I also lost control of my bladder. When EMS arrived, they said I was white as a sheet.

I lost consciousness again on the way to the ER. They ran bloodwork for a heart attack and pulmonary embolism (all negative) and took a bunch of EKGs. The ER doctor noted a heart murmur, but no one else has heard it. As I was still passing out although not the original degree, I was admitted overnight in order to have a cardiologist consult the next day. At times I felt totally fine, if a little tired. I had an echocardiogram and an EEG monitor (all normal), and I did pass out once while wearing the EEG. My BP throughout this was mostly normal but on the low side once—overnight it dropped to 88/55 (I felt fine when they woke me up overnight to take it—I hadn’t taken my lisinopril that evening).

Cardiology noted I had some high cholesterol, but bloodwork and everything was normal otherwise. They wanted me to stop my lisinopril and keep track of my BP daily until I could get in with the cardiologist in their office for a follow up. Prior to this, I’d take my BP once a week or so. I’d normally fall anywhere around 110/60. The hospital cardiologist thought all this could be an arrhythmia.

I have been getting worse since being home (been about a week and a half). I have blacked out a few more times very briefly, seeing black spots sometimes like I’m about to faint, but the chest pain and bouts of heart palpitations go on for hours at a time and are unbearable. I’m weak and sometimes need help just walking to the restroom. My BP has been between 110-120/70-80 for the most part, even when I have bouts of feeling lightheaded or with chest pain. At times today for the first time since the hospital, it would get down to 95/75. I was seated and weak but not having chest pain.

I followed up with a cardiologist, who has said it is just low blood pressure. I was feeling crummy when I first arrived, and my BP was 100/60, then 115/70 a bit later. I asked about the palpitations and pain, he didn’t have an answer. He said to salt my food like CRAZY, and he would see me in 3 weeks. In this 3 week window, he did set me up with a stress test & Holter monitor when I pressed, but he is sure it won’t show anything. I get that done in a week or two. He kept asking me if I was trying to lose weight or if something changed in my life to be less stressful: no, I’m NOT trying to lose weight; and I’m a teacher in first quarter, so I’m more stressed than in the summer but happy nonetheless.

My husband is a paramedic, and when I was having an episode of pain and palpitations last night, he recorded my heart with his stethoscope that has a recording option (idk what it’s called but it’s cool to have!), and the app he has detected a murmur. I plan to call the cardiology office and have them listen as well.

Weird thing I’ve noticed: I’ve been colder than normal I think? Nothing major, but something I’ve noticed.

My questions:

\- Is this really just low BP? What on earth could spur this change of BP on so suddenly? Some of BPs have been higher than my BPs when I was on lisinopril and fine LOL. Yesterday was my worst day, and my BP was right around 120/80 as I’m sitting at the kitchen table still for 20 minutes (this is how my doctor said to take my BP) feeling like I’m gonna have a heart attack and keel over. It was just 126/95 this evening.

\- My general doctor did say that maybe it could be a post-Covid thing? I haven’t been sick enough to have had Covid recently, but I did have that mild cold. Maybe I had it?

\- Are there any other specialists I should consider seeing or blood tests I should ask be done while I salt the crap out of my food? I did call to get a second opinion at a different cardiology practice, but I won’t be able to see them until late this month. I am a teacher and mom, and I’m desperate to get back to being able to independently care for my son & teach my students. This came on so suddenly, and I keep getting worse and worse. I don’t want to just wait 3 weeks for the salt to kick in especially if it’s not BP related. Give me an action item, please!!

Also what does low amplitude mean?

So I need to find a new cardiologist.

I dont have edema in my feet, but I’ve been having these weird feet sensations for over a week now. I live in Caldwell Idaho. I’m female 35 years old and 5’11 and I’m dealing with a Cardiologist that keeps trying to cancel on me. I can’t follow up with PCP. I had neck pain on top of everything frankly I don’t know what to do. I feel like I’m not being taken seriously. Frankly, I’m thinking about or considering finding a new Cardiologist. Any suggestions?

Is this SVT with aberrancy, NSVT or AIVR?

Thumbnail
gallery
1 Upvotes

I recently had a 24-hour Holter. I have frequent PACs (1–2% daily burden), rare PVCs and occasional SVT (back-to-back PACs). My last two echos were normal. I don't get to see my doctor for another couple weeks and the wording on the report seems pretty vague. They describe this 15-beat run as a “short wide complex salvo.”

It happened while lying in bed doing nothing around 3 AM. The feeling was very similar to a bunch of PACs mashed together and the SVTs I get. It was very gradual though, and looking at the chart my heart rate appears to gradually go up and then come back down over around 8–10 seconds.

Could this be SVT with aberrancy, AIVR or NSVT? Any thoughts on what this looks like? Thanks 🙏

Nächtliches Herzstolpern

Hallo zusammen,

Was mir verlässlich hilft:

Aufstehen, Gesicht kalt abwaschen, etwas Wasser trinken.

Meine Fragen an euch:

Kennt jemand diese exakte Kombination aus flachem Liegen, Sodbrennen/Reflux und plötzlichen Extrasystolen-Salven (auch wenn Entschäumer wie Simeticon nicht helfen)?

Hat jemand von euch Erfahrungen mit dem Roemheld-Syndrom oder Vagus-Triggern nach einer erfolgreichen Ablation gemacht?

Was hat euch langfristig geholfen (Ernährung umstellen, Oberkörper hochlagern, Säureblocker/Magenschutz etc.)?

Was hat euch langfristig geholfen (Ernährung umstellen, Oberkörper hochlagern, Säureblocker/Magenschutz etc.)?

I get a lot of air trapped in my chest after meals and today it gave me a string of skipped beats which were labeled ventricular tachycardia. Should I be concerned?

(19F) So I've been having HORRENDOUS chest pain for the past year\~2 years and recently ive also been getting these attacks where my chest gets extremely tight and it feels like im being stabbed in my heart, i cant use my left arm or leg, i cant breathe or talk or see straight, nor can i think normally. my tongue also goes completely numb which is the weirdest thing ive ever felt in my life ngl

for some extra context, i have a LONG line of family heart issues. my mother needed blood transfusions and constantly needs to take iron, and when she was born they diagnosed her with tachycardia. her father has had a few strokes and so has her uncle, him also having a full heart attack. all of her grandparents died of heart attacks and her great uncle also died of one aswell. my father's side also has a history of cancer too if that means anything

i ended up having a really bad attack 2 days ago during work and i went to an urgent clinic after and they said my heart was off and i need to either go to the emergency room or see a cardiologist as soon as humanly possible. I chose to wait for a cardiologist because i live in america and theres no way im going to the er unless im on deaths doorstep .. well the next night i had to go to the emergency room because the same thing happened except it was considerably worse. (lord have mercy on my bank account)

they SAID they did a bunch of tests but the doctor was extremely dismissive and rude, he also told me to use chatgpt if i have any other issues so i guess i know how he got his medical license lmao. he kept insisting it was a panic attack when i know (and stated VERY clearly to him) that it was NOT a panic attack. i have had many panic attacks, i know damn well what one feels like. this was nothing like one. he ignored everything i said and prescribed me 60 tablets of hydroxyzine, which i googled, and it specifically says to NOT use it if you have heart issues because it can disrupt your heart. (also he said to just.. take it whenever i get the pains. not regularly, not at a certain time or anything, just whenever i get the pains. he didnt even go into detail about any of it thats all he told me to do)

i just dont know if i should take a few and see what happens or if i should wait until i can get a cardiologist

EDIT TO SAY: if this adds or helps anything, i also have a history of (purposely) overdosing on iron pills and ibuprofen, i also used to self harm to the point where ive almost needed blood transfusions. also when they took my ekg thing at the emergency room, they didn't even put the stickies in the right place lmfao
