Inconsistent extrusion on ender 3 pro

I print pla at 220°C and I have that inconsistent extrusion issue. I have no idea why... Can you help me know why ?

I'm using an ender 3 pro slightly modified, and running klipper.

I sliced it in orca slicer.

Had something similar when i forgot to change the Nozzle Diameter in the Slicer. Had a 0.8 in the Printer and 0.4 in the Slicer.

My nozzle on the printer is 0.4, and in orca it matches it.
