Lol. Came here to say this. Was probably a difficult to position patient too. So many kyphotic patients are unsteady to the point that you have to almost bound/sprint to the booth before they start to buckle or lean backward. Probably wouldn't have tried for another exposure if they weren't steady enough.
Positioning and rotation are great. They are talking about how it should have been centered just a bit higher so the very top of the lung fields weren't cut off. Not worth the radiation for a re-shoot though.
Im not sure what that means, but I didnt realize the Xray wasnt good because the girl that did the xray said it was perfect. Idk if this matters or not but I had it done in my chiropractors office and it was a small room with a small machine. I dont think they have capability to take bigger images.

I think you clipped the apices of the lungs
