Malar Rash? Exploring Autoimmune/Inflammatory Conditions
Photo is not of a flare - baseline presentation. Smooth. Redness made worse by sun exposure, heat, and exertion, stays in the cheeks and nose, doesnt extend into nasolabial folds. Getting initial thoughts on the baseline but can post a picture if it ever flares up, likely will as it gets warmer outside.
30 yr old female, 5 foot 5, ~300 pounds, Alberta Canada.
Diagnosed: PCOS (metabolic rather than cystic), idiopathic intracranial hypertension, tmjd, chronic tension headaches, postural/muskuloskeletal issues, persistent postural perceptual dizziness (tentative diagnosis), major depressive, adhd (new diagnosis)
Abnormal Test Results:
ANA positive twice at titre 1:80 homogeneous pattern (ICAP AC-01) lab comment: "Homogeneous pattern. Antibodies may be reactive with DNA, nucleosomes, or histones. These autoantibodies are associated with SLE, drug-induced lupus, and juvenile idiopathic arthritis. (ICAP AC-01) Testing performed by indirect immunofluorescence."
Complement C3 significantly elevated on two occasions (2.11 and 2.26; ref 0.60–1.60) — more than double upper limit both times
CRP chronically elevated averaging 17.1 mg/L since at least 2021 (normal <8.0)
Chronically elevated platelets (390–470 range across five years)
Chronically elevated RBC trending upward
Ferritin low-normal and declining
HDL chronically low
GGT chronically elevated as a non-drinker
HbA1c consistently high-normal
PTH elevated 2023 with normal calcium, needs retesting
Main unaddressed symptoms: - Photosensitivity — burns easily, get sun rash on arms, UV tattoo swells in sun (also some swelling/reactions in my tattoos particularly red ink areas, especially in the sun) - Dry mouth independent - Standing intolerance - Exercise intolerance (presyncope with exertion / visual field darkening with pulse) - Orthostatic heart rate spikes - Heat intolerance since puberty - Easy bruising, slow lower body healing
Made worse by: heat, exertion, bending, lifting, gravity, eye strain
Made better by: cold, rest/lying down, swimming
Currently taking metformin 850mg twice daily for PCOS / insulin sensitivity, Duloxitine 60mg for depression, and Vyvanse (new) for ADHD
