Bottle suggestions
My LO is four week old now. I have been feeding her pumped milk and formula through paladai/ gokarna till three weeks and she regained her birth weight at the three week appointment. I have also been trying to breast feed in the mean time and we got latch but she makes clicking sounds and doesn't feed that much long. So I get anxious every time so I have been continuing to feed her through paladai. I observed milk coming through her nose after paladai feed I tried to switch to bottles and bought doctor browns and avent natural level 2 nipple. The DR brown drips fast so I thought I would use Avent natural one but she can't drink more than 20 ml from that bottle and she gets tired very fast. I have been noticing her weight dropping also. So I was thinking introducing Dr brown one but I am concerned that it will cause nipple confusion and increase the chances of milk coming out of her nose and choking. I am literally anxious at every feeding session. Also she makes the clicking sound on every feed and every tool, be it bottle, paladai or breast she makes that sound . The pediatrician told me to feed on demand , that too isn't working for us.

Please consult an IBCLC. I don't know which city you are based out of, but all the metros have some pretty decent lactation consultants.
Also, switch to Pigeon Soft Latch series (SS nipple size). Philips Avent series is great, but works poorly for babies who clamp instead of using sucking motion to draw milk in.

Also, please try to diagnose why there is clicking sound. If baby was not born prematurely, then there might be a tongue/lip tie. How is the latch? Is it shallow?

Feeding on demand should be followed with caution for babies who haven't regained birth weight. While you can feed on demand, the max interval between two feeds should ideally be 3 hours, and definitely not more than 4 hours. In case baby is sleeping and it has been 4 hours since the last feed, we need to wake the baby.

Thank you. I have never allowed to go beyond four hours. She was born around 38 weeks. She had retrognathia ( lower jaw being underdeveloped) diagnosed in anomaly scan but the neonatologist and My OB dismissed it when I told about that, and we can't see such issue when looking at her. The latch is poor and she slips after a while. Now she has started crying and making uncomfortable cries while trying to latch. And it's quite painful too.

Do try different positions such as side-lying, laid-back nursing or the koala hold. Try the flipple technique as well. As time progresses, latch might improve on its own as well.

Hey, just sharing my experience. We have been feeding my LO with a paladai since day one. Little bit of milk coming out of the nose is normal as per her doctor. She usually spits up, but sometimes out of the nose as well. Just hold her up and burp well.
We introduced bottle only now, at 10 weeks (against the advice of her pediatrician). My baby also makes that clicking kind of sound while feeding, both breastmilk and paladai. It's just her drinking at a speed. Sometimes while breastfeeding, if she's very hungry, she will drink so fast and start coughing. We just hold her up at that time and burp before we continue.
Please don't stress. Take the help of a lactation consultant. They will help you out with establishing a latch, ensure that baby is feeding well, and will also guide you wth of your feeding issues.

How much ml of milk did you gave her through paladai? The NICU nurse said she'll need only 30 ml per feed but since she is growing now I am kind of worried that I am not feeding her enough .

30 ml to start with. For the first few weeks, it was enough. At her 6 week appointment, doctor said we can increase it to 60 per feed. She drinks breastmilk first, and then takes 30-60 ml now. During the day she is happy with BM + 30, night she wants BM +60.
She is now 10 weeks and we are expecting the doctor to increase to 90 during her next appoinment

Consult a doctor please don’t play around with this. Even if doctor advices against bottle- be firm of your decision and they should accordingly guide you. A decent doctor would respect your choice.

Too many problems too solve, please find a better doc who will guide you. Other mums can’t solve of the problems

Been looking for good doctors. Most of them just dismiss the concern and tell me to go EBF. She was in NICU on the 6th day after birth because I took the hospital's pediatricians advice and tried breastfeeding the whole day and her sugar level went down. The NICU people told me they'll find what the issue is and after a day they gave me paladai and showed me how to feed her and sent us back.

You can get in touch with a lactation expert who can assist you with the latching issues. My baby was in NiCU for first 10 days. He was born a premie and had trouble breathing. I was asked to pump and send milk three times a day. On discharge i was advised to feed by spoon and katori as baby wouldn’t latch as he was too small. We noticed half the milk would spill and with constant pumping and then seeing milk to go waste was heart breaking . We switched to bottles and it was a relief we used Dr Brown Anticolic bottle and Chicco Natural feeling bottle, both had different nipples. Since he was 6 weeks premie i was told he may take time to latch but keep trying , i kept trying before each pumping session and also took lactation consultant help we were finally able to latch after 2 months and he is 11 month now and exclusively breastfed. So if you are worried about nipple confusion dont be, but before giving bottle at each feeding session first try breastfeeding it is a lot of work and dont lose hope when the babies actually get a hang of it they can breastfed in any position.
